Terbinox is an antimycotic for local treatment.

The active ingredient of the drug is terbinafine hydrochloride, which has a wide range of antimycotic activity. The pharmacological effect of terbinafine is associated with its ability to inhibit the early phase of sterol biosynthesis inside the fungal cell by slowing down the cell wall enzyme squalene epoxidase. As a result, a lack of ergosterol develops and the accumulation of squalene inside the fungal cell, which leads to its death. [1]

Indications Terbinox

It is used for the following disorders:

  • fungal epidermal lesions associated with epidermophytes, trichophytes and microspores that are sensitive to the drug;
  • yeast epidermal infections associated with the activity of yeast fungi (mainly candida );
  • pityriasis versicolor .

Release form

The release of the drug is realized in the form of a cream - inside tubes with a volume of 10 or 15 g. Inside the box - 1 tube.

Pharmacodynamics

Terbinafine demonstrates a fungicidal effect against dermatophytes, including trichophytons (red trichophyton, interdigital trichophyton, crateriform trichophyton, warty trichophyton and T. Violaceum), fluffy microsporum, flocculent epidermophyton, as well as certain dimorphic fungi, smoky aspillus and moldy asp etc.). In relation to yeasts (usually from the genus Candida), terbinafine has fungistatic and fungicidal activity (depending on the type of fungus).

Terbinafine hydrochloride has been shown to act in relation to the Pityrosporum subgroup - for example, inhibiting the reproduction and growth of the bacterium that causes the development of pityriasis versicolor, Pityrosporum orbiculare. [2]

Pharmacokinetics

When using terbinafine in the form of a cream, absorption through the epidermis occurs at a low rate (below 5% of the portion applied to the epidermis), so it almost does not cause systemic effects. [3]

Dosing and administration

Terbinox is used 1-2 times a day. Before performing the procedure, it is necessary to clean and dry the infected skin areas. Taking into account the size of the affected area, apply 2-4 g of the medicine (the size of the strip is within 3-6 cm), lightly rubbing it into the skin. Both the infected area and the surrounding areas are subject to processing.

In case of infections in which diaper rash is noted (in the area between the fingers or buttocks, under the breasts, as well as in the groin area), the cream area can be covered with a gauze bandage, especially at night.

Therapy for various diseases lasts on average:

  • dermatomycosis in the lower leg or trunk: 7 days (apply once a day);
  • dermatomycosis in the foot area: 7 days (once a day);
  • epidermal candidiasis: 7 days (1-2 times a day);
  • pityriasis versicolor: 14 days (1-2 times per day).

The main clinical manifestations are often weakened or disappear during the first days of treatment. If there is no improvement after 14 days of therapy, it is necessary to clarify the diagnosis.

  • Application for children

There is not enough information regarding the safety of use and the therapeutic efficacy of Terbinoks in persons under 12 years of age.

Use Terbinox during pregnancy

Terbinox has practically no systemic effect, but there is too little information regarding its use in breastfeeding or pregnancy. Because of this, it is prescribed only in situations where the benefits for the woman are more likely than the risk of complications in the fetus or infant.

Contraindications

It is contraindicated to appoint people with severe intolerance to terbinafine or other elements of the medication.

Local treatment with cream can cause burning, redness and itching, which do not require discontinuation of therapy. Such complications must be distinguished from the development of signs of allergy (they appear occasionally), in the event of which it is necessary to cancel the treatment.
