Ibalgin is a typical representative of a group of non-steroidal anti-inflammatory drugs that can affect bone structures and joints. This drug belongs to the group of medicines that are widely used in the presence of pain in the muscles and joints.
The main active substance is ibuprofen, which has a large spectrum of action. Ibalgin is used for local application to the focus of pain in order to reduce the intensity of the manifestation of the pain syndrome.
The method of application is based on the form of release, since Ibalgin is presented in the form of a cream. According to the ATS code, the drug belongs to non-steroidal anti-inflammatory drugs (M02AA), in particular ibuprofen is coded as M02AA13.
The international name of the drug is Ibuprofen. The main physico-chemical features are white or almost white, in consistence - cream-emulsion "oil / water".
Indications Ibalgin
The drug is designed to reduce the intensity of pain syndrome, which develops as a result of degenerative processes and inflammatory reaction of joint structures. Spinal injury and joint may have rheumatic damage or characteristics of rheumatoid arthritis, ankylosing spondylitis, or osteoarthritis.
In addition, indications for the use of Ibalgin include the presence of pain in the exacerbation of gout involving joints, psoriatic arthritis, tendonitis, bursitis, radiculitis and inflammatory process due to traumatic injury.
However, not only the pathology of joints and bone structures is an indication for use, but also the presence of pain syndrome in inflammation of nerve endings and muscles.
Indications for the use of Ibalgina also imply the administration of the drug in diseases of the ENT organs, the cause of which is an infectious agent with a further increase in the inflammatory response.
This anti-inflammatory agent is also widely used in adnexitis, algodismornorea, dental and headaches. Ibalgin can be used to reduce temperature in pathologies of infectious and inflammatory genesis in the presence of hyperthermia.
Release form
The anti-inflammatory agent is presented as a cream for use by application to a painful area. This form of release allows to exert a local effect on the pathological focus, without significantly altering the hemodynamic parameters.
The cream is a substance of pleasant soft consistency, thanks to which it is well applied to the area with painful sensations, completely covering the skin. As the cream penetrates into the underlying layers of the skin, the main active substance begins to show its pharmacodynamic properties.
Ibalgin, the form of which is represented by the cream, is available for sale in the form of filled tubes with a volume of 30 ml and 50 ml. It should be borne in mind that 1 g of cream contains about 0.05 g of active ingredient - ibuprofen. It is necessary to know this in order to avoid an overdose and the appearance of adverse reactions.
In addition to the basic substance, it is necessary to identify auxiliary - solid fat, triglycerides, alcohols, propylene glycol and some other components.

Pharmacodynamics
Ibalgin is a derivative of phenylpropionic acid, as a result of which the main therapeutic effects of the drug are isolated. Among them, we need to focus on the anti-inflammatory, analgesic and antipyretic effect.
Due to the mechanisms of action of Ibalgin, the activity of the inflammatory reaction is reduced by decreasing the intensity of exudative processes and the production of mediators of inflammation. Thus, reduction of puffiness, redness, pain syndrome and recovery of the motor activity of the affected joint is ensured.
Pharmacodynamics Ibalgin consists in inhibition of the activity of COX, which is an enzyme that takes part in the metabolism of arachidonic acid. The latter, in turn, is associated with prostaglandins taking part in the development of an inflammatory reaction.
Due to inhibition of the synthesis of prostaglandins in the central and peripheral areas, the intensity of the pain syndrome is reduced. In addition, the pharmacodynamics of Ibalgin includes a reduction in the process of gluing together platelets.
Thus, the drug with regular external application provides anti-inflammatory and analgesic effect, due to which the motor activity of the joint is restored.

Pharmacokinetics
The drug is intended for external application on the affected joint, therefore, a small volume enters the systemic blood stream, which does not cause significant hemodynamic changes.
The pharmacokinetics of Ibalgin is observed after application of the cream to the skin, after which the drug is rapidly absorbed into the deep layers and begins to provide therapeutic assistance.
The maximum concentration of the main active substance after application of 5 g of cream (in terms of 250 g of ibuprofen) is approximately 100 ng / ml. If you compare these figures with oral intake of the same dose of ibuprofen, you can see that they are only 0.5% of the concentration after ingestion.
A small part of ibuprofen, which is absorbed into the bloodstream, is metabolized in the liver and excreted in the urine - up to 80% and through the intestine - about 20%.
The main effect is on the site of the application of the cream without changing the blood parameters.
Dosing and administration
The cream is intended for application to the skin area, where there is pain syndrome and other clinical manifestations of the inflammatory reaction.
The method of application and dose should be strictly considered in order to avoid the appearance of adverse side effects from the skin or internal organs. So, for a single application you need to squeeze out of the tuba a strip of cream, the length of which depends on the area of the lesion. In general, the length is from 4 to 10 centimeters.
The frequency of use of the drug is 3-4 times a day. Cream should be grinded on the area and slightly rubbed. In the presence of hemorrhage and the need to impose a sterile bandage, you should apply the cream and cover with the material.
The duration of the application of the cream depends on the severity of the pathological process and the rate of recovery of the motor activity of the joint. Most often the duration of the therapeutic course is 2-3 weeks.
Use Ibalgin during pregnancy
Pregnancy all over requires a woman to pay special attention to taking medications. Their independent use without prior consultation with a doctor threatens the life of the fetus and its health. In addition, depending on the period of pregnancy, there may be irregularities in the processes of laying the organs, their development and preparation for an independent life.
Use of Ibalgin during pregnancy should be resolved only after assessing the risk to the fetus and benefits for the woman. So, in the first and second trimesters, the use of Ibalgin is less dangerous than at later dates. This is due to the fact that in the third trimester, the mammary glands begin to be intensively prepared for feeding the baby.
Ibuprofen in small amounts can be excreted in breast milk, so in the last months of pregnancy, and during the period of natural feeding is not recommended to use Ibalgin.
If you can not avoid the use of this drug, then you need to strictly control the dosage. If you exceed the daily dose of more than 800 mg, you should consider stopping breastfeeding while you use Ibalgin.

Overdosage with a cream is very unlikely, since the used amount of the drug is applied to the skin, after which only a small part falls into the systemic bloodstream.
The proportion of the main active substance in the blood can not have a significant therapeutic effect, the more significant hemodynamic changes. Only in rare cases, there may be dizziness, headache or nausea.
Nausea and vomiting may occur if the medication is mistaken for external use orally. In this case, it is necessary to wash the stomach.
Overdose can be manifested by clinical symptoms of adverse reactions if a person has an individual drug intolerance. In this case, on the skin, redness, itching and swelling may appear.
Interactions with other drugs
Due to the fact that the cream is intended for use on the skin, the interaction of Ibalgin with other drugs is possible to a greater extent only with those that are also applied externally.
So, if it is required to apply several different drugs to the affected focus, one of which is Ibalgin, then take into account the time of their absorption. After applying Ibalgin, you need to withstand a break about 3-4 hours before using another drug.
The interaction of Ibalgin with other drugs with a group of non-steroidal anti-inflammatory drugs may increase the likelihood of developing side effects. To avoid this, it is advisable to adhere to a certain time interval between the applications.
In addition, it is necessary to take into account the interaction of Ibalgin cream with drugs that are administered intra-articularly. When using methotrexate, it is possible to intensify the manifestations of its side effects.
With controlled use of the cream, the part of ibuprofen delivered to the blood can not cause conflict with medications taken internally. Care should be taken with oral administration of non-steroidal anti-inflammatory drugs, warfarin, diuretics and ACE inhibitors.
