Retarpen is a natural antibiotic from the β-lactam penicillin subgroup. The active element of the drug is the substance benzylpenicillin benzathine (penicillin subtype G), which has a prolonged type of therapeutic activity.
The specified drug component (benzylpenicillin benzathine) is extracted from fungi such as molds. The drug has a bactericidal effect against the cells of pathogenic bacteria. It is used in the treatment of bacterial infections of various forms. [1]
Indications Retarpen
It is used in the case of diseases provoked by the action of pale treponemas and streptococci: scarlet fever, , erysipelas , tonsillitis in an active form, as well as rheumatism and syphilis .
The medication can also be used to prevent the development of complications after surgery.
Release form
The release of the therapeutic element is realized in the form of a suspension for intramuscular injections (lyophilisate inside the vials). The pack contains 50 of these bottles.
Pharmacodynamics
The principle of the therapeutic activity of an antibiotic is realized by inhibiting the binding processes of the bacterial cell membrane. The medicine has a long lasting effect. Demonstrates high activity against gram-negative and -positive microbes, treponema, anaerobes and spore-forming. [2]
Retarpen has no effect on staphylococci that produce penicillinase. [3]
Pharmacokinetics
After the introduction of benzylpenicillin benzathine, it is absorbed at an extremely low rate from the area of the injection, providing the development of a prolonged effect.
Serum Cmax values are observed after 12-24 hours from the moment of administration. The long term half-life leads to the establishment of long-term and stable values of drugs inside the blood: by the 14th day from the moment of application of 2,400,000 IU, the serum level is 0.12 μg / ml. The indicator of protein synthesis is about 55%.
The drug substance in small volumes crosses the placenta and enters the mother's milk. The metabolic processes of drugs are rather weak.
Excretion is mainly carried out through the kidneys, in an unchanged state; over a period of 8 days, up to 33% of the applied portion is excreted.
Dosing and administration
The medication must be used only by the intramuscular method. In case of need for the introduction of 2 injections, they are produced in different buttocks.
When treating congenital syphilis in newborns or young children, 1-time use of 1.2 million IU is required (or the portion is divided into 2 injections).
With a seronegative form of primary syphilis: intramuscular use of drugs at a dosage of 2.4 million units.
In the case of a fresh form of secondary syphilis or a seropositive form of primary syphilis: the introduction of 2.4 million units, with a repetition of the injection after 1 week.
With the tertiary form of syphilis: the use of 2.4 million units; therapy continues for 3-5 weeks.
Individuals with flambesia are given 1-2 injections at a dosage of 1.2 million units.
With an active form of tonsillitis, wound infections, scarlet fever or erysipelas, therapy begins with the use of benzylpenicillin, and then Retarpen is injected.
Prevention of the development of attacks of rheumatism in the joint area: i / m application of 2.4 million units at 15-day intervals.

Retarpen is not used in pediatrics.
Use Retarpen during pregnancy
Since benzylpenicillin benzathine crosses the placenta, the medication is allowed to be used only after a careful assessment of all the risks and benefits.
A small amount of the drug is excreted in breast milk (these indicators fluctuate within 2-15% of the drug value inside the woman's plasma). There is no information on the appearance of negative signs in infants, but it must be borne in mind that sensitization or interference with the intestinal flora may be observed. HB should be canceled if the baby develops candidiasis, diarrhea, or a rash.
Children who are on combined feeding, for the period of therapy with a woman, should be transferred to feeding with baby food. It is allowed to resume hepatitis B after 24 hours from the moment of discontinuation of therapy.
Contraindications
The main contraindications:
- severe intolerance to benzylpenicillin;
- hay fever;
- BA.
Side effects Retarpen
Prolonged administration of the medication can provoke the appearance of superinfection. Therapy can lead to glossitis, stomatitis, headaches, arthralgia, anemia, and in addition to signs of allergy, fever, exfoliative dermatitis, hypocoagulation, anaphylaxis and leukopenia.
Overdose
Movement disorders, encephalopathy, seizures, confusion, and neuromuscular excitability are possible.
Symptomatic and supportive actions are carried out, and in addition, hemodialysis procedures. The medicine has no antidote.
Interactions with other drugs
Rifampicin, cycloserine with aminoglycosides, cephalosporins with vancomycin and other antibacterial substances of the bactericidal type have a synergistic effect on drugs.
Lincosamides, chloramphenicol, tetracyclines with macrolides and other drugs of the bacteriostatic type have a powerful antagonistic effect.
Retarpen reduces PTI values, increases the effect of indirect anticoagulants, inhibits the intestinal microflora and insignificantly affects the therapeutic efficacy of hormonal contraception.
Diuretics, NSAIDs, allopurinol with phenylbutazone, and tubular secretion blockers increase penicillin values. The combination with allopurinol increases the likelihood of allergic symptoms in the form of epidermal rashes.
Storage conditions
Retarpen must be stored in a place protected from small children and sunlight. Temperature values - maximum 30oC.
Shelf life
Retarpen is allowed to be used within a 4-year period from the date of manufacture of the medicinal product.
Analogs
The analogs of the drug are Amosin, Ospin with Ampicillin, Bitsillin and Ospamox.
