Hydrocortisone ointment 1% - a substance for external use. This is an artificial drug from the GCS group. It has anti-edema, anti-inflammatory and antipruritic effects.
The drug slows down the release of cytokines (interferon with interleukins) from macrophages, as well as lymphocytes; in addition, it inhibits the release of inflammatory conductors from eosinophils, destroys the binding of PG and metabolic processes of arachidonic acid. [1]
If used in the prescribed dosages, it does not lead to the appearance of systemic negative signs. [2]
Indications Hydrocortisone ointment 1%
It is used to treat allergic and inflammatory lesions of the epidermis that have a non-microbial nature (also those against which itching develops): psoriasis, pruritus, eczema , neurodermatitis, seborrhea and erythroderma, and besides that, insect bites and contact or allergic form of dermatitis .
Release form
The release of the therapeutic agent is made in the form of an ointment for external treatment, inside 10 g tubes.
Pharmacodynamics
The drug stimulates glucocorticosteroid endings, which induces the formation of lipocortin. Weakens inflammatory infiltrates in the area of cells and reduces the movement of leukocytes (among them lymphocytes) into the area of inflammation. [3]
Pharmacokinetics
After applying the ointment, the active element accumulates inside the epidermis (most of it is inside the granular layer).
Metabolic processes occur directly inside the epidermis, and then develop inside the liver.
Excretion occurs through the intestines and kidneys.
Dosing and administration
It is necessary to apply the ointment externally - treat the affected areas of the epidermis with a thin layer, 2-3 times a day. The duration of the treatment cycle is determined by the effectiveness of therapy and the nature of the disease; usually lasts 6-14 days. With a severe course of pathology, it can be extended up to 20 days. In the case of treatment of limited foci, sealed dressings can be used to potentiate the effect.
In children over 2 years of age, the ointment can be used for no more than 14 days; also do not perform actions that potentiate the absorption of hydrocortisone (fixing, hermetic and warming dressings).
- Application for children
Cannot be administered to infants under 1 year of age.
When used in children, the duration of the treatment cycle must be shortened, since in childhood the suppression of the adrenal cortex can occur faster. Along with this, there may be a deterioration in the excretion of STH. With prolonged use of the ointment, it is necessary to monitor the indicators of height, weight and plasma cortisol.
It is possible to use the medicine in a child under the age of 12 only under the supervision of a doctor. In the absence of improvement (or deterioration of the clinical picture) after the 1st week of the use of drugs, and besides this, when the signs resume after several days from the moment the ointment is canceled, its use should be completed and the doctor should be consulted.
When treating large areas of skin or prolonged therapy, a diet with an increased K + and Na restriction should be prescribed; also, the body must receive a sufficient amount of protein.
It is required to monitor the level of blood glucose, blood pressure, blood clotting, patient weight, urine output and plasma cortisol values.
Use Hydrocortisone ointment 1% during pregnancy
GCS are able to cross the placenta. The effect on the fetus can be especially pronounced if large areas of the skin are treated with ointment. It is necessary to use Hydrocortisone ointment with extreme caution for HB and pregnancy.
Contraindications

Overdose
The likelihood of acute intoxication is very low, but with prolonged or excessive use of hydrocortisone ointment, chronic poisoning can develop, in which symptoms of hypercortisolism are noted: glucosuria, curable suppression of the adrenal cortex, hyperglycemia and signs of cushingoid.
Appropriate symptomatic procedures are performed; in case of chronic toxic effects, a gradual withdrawal of the drug should be carried out.
Interactions with other drugs
Anticonvulsants, barbiturates and antihistamines weaken the effects of drugs.
Use with paracetamol causes hepatotoxicity.
