The dermatologist must prescribe the necessary ointment for psoriasis, but patients who have tried for years to relieve their condition and reduce the manifestations of this chronic disease, it is useful to navigate the names of the means. It is not superfluous to have an idea about the pharmacological characteristics of dermatotropin preparations, including their contraindications and side effects.
Names of ointments from psoriasis
Even specialists to make a rating of the best ointments for psoriasis is difficult, because the reaction of each patient to a given drug depends on many factors, and the means that help one may not give a therapeutic effect to others. Dermatologists admit that often patients are forced to change dozens of medicines before determining a really effective ointment for psoriasis for a particular patient.
There are two main classes of external antipsoriatic drugs - non-hormonal ointments from psoriasis and hormonal. Among the first experts include:
- ointments with salicylic acid and zinc oxide;
- ointments for psoriasis based on solidol;
- ointments based on naphthalan oil;
- ointment based on bituminous anthracene derivative;
- ointments for psoriasis on the basis of tar;
- ointments with calcipotriol (a vitamin D3 derivative);
- homeopathic ointments and combined phytomases (based on medicinal plants).
The main names of ointments for psoriasis that do not contain hormones: Antipse, Psorilam, Kartalin, Daivonex (Psorkutan), Naftalanum Ointment Naftaderm, Acrustal, Antralin, Sophor Ointment, Psoriaten, Psoril.
Any hormonal ointment from psoriasis contains a synthetic substance similar to the steroid hormones of the adrenal cortex - glucocorticoids, the main function of which is the regulation of metabolism and coordination of all parts of the body's immune system.
Ointments with salicylic acid and zinc oxide
Still, it helps to soften skin areas that have become chapped from excess keratinized cells, effective keratolytic - salicylic ointment (2%). In addition, it is part of many combined drugs. The main indications for the use of ointments from psoriasis with this acid include the progressive stages of the disease.
Zinc ointment is an antiseptic and adsorbent, it dries the moccasins and relieves inflammation, like the ointment of Yam (a combination of salicylic acid and zinc oxide). As these agents act on psoriatic rashes, read in the article - Non - hormonal ointments from psoriasis
Apply with psoriasis (as well as eczema, atopic dermatitis and neurodermatitis) ointment with antimicrobial and fungicidally active pyrithione of zinc. Cinocap (Skin-cap) - twice a day treating the rashes on the skin (daily for 4-6 weeks). This fairly effective ointment for psoriasis is good because it has no contraindications (except for early childhood) and side effects (except for possible slight skin irritation).
Psoriasis ointments based on solidol
Medical fatty salt is the basis of such products as Acrythal ointment, cosmetic balm Antiport or Ivanova ointment from psoriasis, Tsitopsor, Psorium, Psorilam. And also home-made ointments, in particular, it's Golyuk's ointment for psoriasis (read about it in the Home Ointment section of psoriasis), Markin's ointment for psoriasis, Rybakov ointment (saltol, Vaseline, boric acid and menthol).
Pharmacodynamics solidol, that is, the biochemical mechanism of its ability to reduce the intensity of keratinocyte proliferation at a progressive stage of the disease, is not described in any instructions to the listed drugs.
Ointment Acrural, positioned as a phytocream, acts due to the presence of oils (eucalyptus, cedar, wheat germ, yarrow), oily extracts (calendula, chamomile, dog rose), extracts (licorice root, aloe, juniper berries). This remedy also contains beeswax and wax. During the first week Ointment ointment is applied to the rashes only on the limbs - every other day (once a day, without washing off for 6-7 hours); after that during the subsequent treatment the agent should be applied daily.
Contraindications Acrustal - individual hypersensitivity; side effects can manifest as a skin allergy with redness and itching.
Storage conditions for this product require a temperature of at least + 5-7 ° C, and the expiry date is 2 years.
In addition to the solidol, the composition of the preparation Antiphore includes birch tar, fir oil, chitin (crushed shells of Kamchatka crabs), red seaweed, and extracts of medicinal plants: strings, celandine, linen, felt burdock and aralia of Manchuria. The pharmacodynamics of these components is not explained, although it is emphasized that it is thanks to the solidolol that essential oils, organic acids, flavonoids and plant alkaloids that contain Ivanov's ointment from psoriasis penetrate well into the epidermis and help exfoliate dead keratinocytes and reduce itching.
In addition, various dermatoses, trophic ulcers, external hemorrhoids, varicose veins, alopecia and other diseases, including gynecological and cancerous, are included in the indications for the use of the Antiprusin ointment.
Kartalinnaya ointment, more precisely, ointment Kartalin has in the list of components of solidol, eucalyptus and lavender oil, retinol (vitamin A), natural honey, salicylic acid, antiseptic enzyme lysozyme, extracts of chamomile and triplicate chamomile.
According to the instructions, this product has anti-inflammatory, antipruritic and keratolytic properties. It is recommended to apply on the rash within two months - at least twice daily. After cleansing plaques from scales - once a day for 30 days. Contraindications and side effects of Cataract are the same as those of an antiperspirant.
Psoriasis ointment based on naphthalan oil and bitumen anthracene
Naftalan ointment in the form of Naftaderm has pronounced antiseptic and keratoplastic properties, it also stimulates blood circulation in the vessels of the skin, activates metabolic processes, promotes moistening and softening of the epidermis. Pharmacodynamics of this agent is provided by biologically active cycloalkanes - isomers of hydrocarbon compounds of the higher fractions of naphthalane oil - naftalan, purified from resins. In naphthalane there are carboxylic acids, sulfur-containing compounds, salts of manganese, copper, zinc, cobalt. Their combined effect is manifested not only in the exfoliation of excessive keratinization in psoriasis and other hyperkeratoses, but also in the activation of the growth zones of the epithelium. Experts believe that this is an effective ointment for psoriasis.
Contraindications to its use are functional deficiency of the liver and kidneys, anemia, thrombocytopenia, oncology of any location, dysfunction of the adrenal cortex and pheochromocytoma of their brain substance, children's age.
Only the attending physician, considering the patient's condition, can allow the use of naftalan ointment from psoriasis during pregnancy. Among the side effects of Naftaderma, skin reactions (rashes, congestion, swelling) and inflammation of hair follicles (folliculitis) are noted. This tool should be applied to dry skin in a very thin layer (without rubbing) - twice a day for a month.
But the ointment from psoriasis Antralin (other trade names - Antraderm, Psoriaten, Ditranol, Zignoderm) contains anthracene - a derivative of naphthyde (bitumen) 1,8-dihydroxy-9 (10H) -anthracenone, which refers to polycyclic aromatic hydrocarbons. Under its influence, the rate of mitosis of keratinocytes decreases, which contributes to the slowing down of plaque growth. Antralin is applied to the affected area for half an hour, maximum for an hour, after which the ointment should be washed off, taking a warm shower with soap. The course of treatment is 1,5-3 months.
Contraindications for this remedy include allergies to its components, renal failure, pregnancy, and an active stage of psoriatic eruptions. Antralin tends to irritate the skin and can cause its redness and soreness. The drug also temporarily stains the skin in brown. It can be stored at room temperature, but away from sources of heat, moisture and light.
Ointment for psoriasis on the basis of tar
It should be borne in mind that ointment from psoriasis on the basis of tar should be used during the stationary stage of the disease (that is, when new spots do not appear, and already available become larger in size and much flaky).
Among the tar constituents obtained by dry distillation of the upper layer of the birch bark, there are salicylic acid, methyl salicylate (methyl salicylate), betulin and betulenol, which disinfect and tone the skin, stimulate its blood supply and recovery, normalize cellular immunity, excrete toxins and help release plaques from keratinized keratinized particles.
The known sulfur-tar ointment (5%) or 10% tar ointment is applied once a day, if there are no contraindications: kidney failure, pustular inflammations or wetting rashes on the skin (with exudative psoriasis). Also, do not use these drugs during pregnancy. Side effect is manifested by increased sensitivity of the skin to ultraviolet rays, irritation in the form of dermatitis and folliculitis.
Contains tar Monastery ointment from psoriasis - a therapeutic cream of the series "Monastic". Also in the composition of this remedy there are salts, vitamins (A and D3), oils (chamomile, lavender, soy), extracts (alternating and oats) and beeswax. The cream is applied to diseased skin areas once or twice a day. Contraindications and side effects are similar.
Psoriasis ointment with calcipotriol
Ointment from psoriasis with calcipotriol (1, 25-hydroxyvitamin D3) - Dayvonex (other trade names Psorkutan, Silkis) is intended for treatment of mild and moderate degrees of the disease in a stationary stage.
Pharmacodynamics Daivoneksa is based on the ability of the active substance to bind to the vitamin D receptors of T cells and inhibit the proliferation of keratinocytes by normalizing the synthesis of certain proteins.
Pharmacokinetics: no more than 1% of calcipotriol is absorbed into the blood, the drug undergoes cleavage in the liver, metabolites are excreted in the urine and feces.
Contraindications to the use of Daivonex ointment: renal failure, liver disease, hypercalcemia, age younger than 12 years. Do not use this medication on the skin of the facial part of the skull. The use of ointments with calcipotriol during pregnancy is contraindicated.
Diveonex can accompany such side effects as dermatitis; hyperemia, edema, dryness, hyperpigmentation and increased photosensitivity of the skin; angioedema; the increase in the content of calcium in the blood and urine.
Dosage and administration: the ointment is applied to the plaques no more than twice a day, the maximum daily dose is 15 g, the maximum duration of application is two months.
Overdosing is possible with more frequent application of the ointment and is manifested by constipation, increased thirst, general and muscle weakness.
Interactions with other drugs: Daivonex is incompatible with ointments containing salicylic acid, and diuretics that remove sodium from the body (hypothiazad, furosemide, etc.).
Storage conditions: at a temperature not higher than + 25 ° С. Shelf life - 24 months.
By the way, Radevit ointment containing vitamins A, E and D3 (cholecalciferol), relieves inflammation, reduces prurit and moisturizes the skin. It is recommended to apply it twice a day. In the period of pregnancy can not be used.
Treatment of psoriasis with hormonal ointments
Hormonal ointments containing corticosteroids (SCS) - synthetic analogues of endogenous glucocorticoids, are used in dermatology as topical treatments for many pathologies, including psoriasis. Of course, these are effective ointments for psoriasis.
Their anti-inflammatory effect, as well as the ability to remove the itching and swelling of the skin are associated with immunosuppression, which is provided by the binding of pharmacologically active substances with glucocorticoid receptors on cell membranes. This is followed by an increase in lipocortin production, due to an increase in the level of which in the tissues of the dermis the enzymatic activity of the cytosolic phospholipase A2 decreases. In turn, this dramatically reduces the synthesis of mediators of inflammation and inhibits the exit from the mast cells of the mediator of allergic reactions of histamine.
Here are some names for ointments from psoriasis with SCS:
- Hydrocortisone ointment, hydrocort, acortin, lokoid, laticort, oxycorte are synonymous and contain the same active substance - hydrocortisone-17-butyrate.
- Ointment Beloderm (other trade names - Betamethasone, Acriderm, Betacorten, Celestoderm-B) contains fluorinated GCS of betamethasone dipropionate, which is often more effective than hydrocortisone.
- Dermoveit (Clobetasol, Psoroderm, Cloveit) with the active substance of clobetasol propionate.
- Ointment Sinaflan (Flucinar, Flukort, Sinoderm, Ultralan) contains fluocinolone acetonide; well penetrates the epidermis and accumulates in the dermal layer, providing a prolonged pharmacotherapeutic effect.
- Elok ointment (or cream) contains a steroid substance of mometasone furoate; Other ointments with the same GCS - Uniderm, Avecourt, Momat, Gystan.
There are also a lot of hormonal ointments with a combined composition. Ointment Belosalik - synonyms: Betasalik, Betaderm A, Diprosalik, ointment Rederm and others is a combination of betamethasone dipropionate and salicylic acid (which in ointment is almost 98%).
Components of the Daivobet - betamethasone and calcipotriol (in the ratio of 99: 1).
Chinese cream King of the skin (Compound ketoconazole cream KL) is a combination of SCS of clobetasol propionate with an antifungal agent Nizoral (ketoconazole).
Contraindications to the use of all ointments with corticosteroids are identical and do not depend on the specific GCS. These are viral and fungal diseases, acne (normal and pink), violation of the integrity of the skin, malignant dermatological neoplasms, children's age (up to two years).
The use of hormonal ointments from psoriasis during pregnancy is allowed only in the most extreme cases and strictly according to the doctor's prescription.
Absolutely no different and side effects of ointments from psoriasis, containing GCS. And here we come to the answer to the question: what are the dangerous hormonal ointments from psoriasis?
Their main danger lies precisely in the side effects that can manifest themselves:

- in reducing the synthesis of collagen and the development of atrophic processes in the skin with the involvement of underlying tissues (possibly the appearance of striae);
- in the reduction of muscle mass and myopathies;
- in the loss of strength of bone tissue (osteoporosis);
- in persistent increase in blood pressure;
- in violations of glucose metabolism (the development of so-called steroid diabetes);
- in the suppression of the functions of the hypothalamic-pituitary-adrenal system and insufficiency of the adrenal glands with the risk of the occurrence of Isenko-Cushing syndrome.

Psoriaten - homeopathic ointment from psoriasis on the basis of the bark of evergreen shrub mahonia, papular or holly-like (Mahonia aquifolium) in the form of a homeopathically diluted matrix tincture; ointment base - lanolin. Pharmacodynamics of the drug is associated with the effect on the skin cells of the alkaloids of this plant (berberine, berbamine, tetrahydroberberine, canadine, hydroxyacanthine, hydrastin, columbin) and tannins. According to the instructions, Psoriatin Ointment helps to reduce the itching and desquamation of the skin with insignificant manifestations of psoriasis.
A homeopathic external means Psoril contains oil from seeds of milk thistle spotted, essential oils (eucalyptus, lavender and rosemary), extracts of ten herbs (St. John's wort, calendula, sage, violet, sweet clover, etc.). The therapeutic effect is similar to Psoriaten.
The method of administration and dose are also the same: ointments are applied to plaques (without rubbing) three times during the day. Side effects are manifested by urticaria and increased itching. These funds do not apply to children and pregnant women.
The main component of phyto-means Sophora ointment - oil extract of buds and fruits of Japanese Sophora (Styphnolobium japonicum), containing flavonoid sophorin or rutin, which is an antioxidant, strengthens capillary walls and promotes normal metabolism in the dermis.
