The drug Yutibid with the international and chemical name Norfloxacin (ATC code J01MA06) belongs to the antibacterial substances of the fluoroquinolone pharmacotherapeutic group, effective in the treatment of infections of the genitourinary system and gastrointestinal tract.

Indications Utibid

The drug Yutibid is prescribed for infectious and inflammatory processes caused by pathogenic organisms, the activity of which can be suppressed by norfloxacin.

Indications for use of Yutibid tablets:

  • diseases of the urinary system;
  • intestinal infections;
  • prostate diseases;
  • for prophylactic purposes to prevent recurrent urinary tract infections;
  • in patients with gonorrhea;
  • for the treatment of bacterial infections that occur with a decrease in granulocytes against the background of a decrease in leukocytes (granulocytopenia);
  • for "traveler's diarrhea" (gastrointestinal disorders caused by microorganisms present in drinks and food).

Drops with norfloxacin are recommended in the treatment of:

  • for various inflammations of the mucous membrane of the eye or eyelids (conjunctivitis, blepharitis, etc.);
  • ulcerative lesions of the cornea;
  • meibomitis (inflammatory process of the meibomian glands) of an acute nature;
  • infectious lesions of the lacrimal sac (dacryocystitis);
  • to prevent the proliferation of pathogenic flora after removal of a foreign body;
  • as a result of dysfunction due to chemical eye damage;
  • before/after surgery (eye area);
  • external and middle otitis of acute or chronic type;
  • for the purpose of preventing complications after ear surgery.

Release form

One tablet of the drug Yutibid contains 400 mg of norfloxacin. Additional components include: sodium lauryl sulfate, starch, talc, titanium dioxide (E171), magnesium stearate, propylene glycol, polyethylene glycol, brilliant blue dye (E133), ethyl- and hydroxypropyl methylcellulose (Methocel E 5/15 LVP).

Release form:

  • coated tablets (10 pcs. in a blister, the number of which in a cardboard package varies from 1 to 6 pcs.);
  • eye/ear drops.

Pharmacodynamics

The antimicrobial substance Yutibid is a synthetic broad-spectrum antibiotic. The drug has pronounced bactericidal activity, destroys DNA gyrase (an enzyme involved in supercoiling and stability of DNA bacteria).

Pharmacodynamics of Yutibid is based on high activity against most known gram-positive and gram-negative bacteria. The spectrum of action of norfloxacin extends to Neisseria gonorrhoeae (gonococci that cause gonorrhea), not excluding strains that produce penicillinase. The drug is used in the treatment of infectious lesions of the urinary tract when infected with: Klebsiellae, E. Coli, Proteus spp, Enterobacter spp, Pseudomonas aeruginosa and Serratia marcescens. Norfloxacin eliminates pathogenic flora that causes inflammation of the small intestine (Salmonella, E. Coli, etc.). The antibiotic is ineffective against a number of anaerobic organisms such as: Actinomyces spp, Peptostreptococcus spp, Chlamidia trachomatis, etc.

The clinical effect lasts up to 12 hours.

Pharmacokinetics

Absorption of Yutibid tablets occurs in the gastrointestinal tract and is up to 40%. Parallel consumption of food significantly reduces the absorption capacity of the drug. The peak concentration is observed one hour after consumption. The percentage connection with plasma protein does not exceed 15%.

The pharmacokinetics of Yutibid indicate good distribution of the substance through the cells of the urogenital system, including: ovaries, kidneys, uterus, secretion of the seminal ducts, organs of the peritoneum and small pelvis, bile and kidneys. Norfloxacin passes through the blood-brain barrier and placenta, penetrates into umbilical cord blood, amniotic fluid and breast milk.

The process of partial metabolization occurs in the liver. The half-life of the antimicrobial substance occurs 3-4 hours after administration. Approximately 30% of the drug is excreted unchanged in urine (through glomerular filtration and tubular secretion), bile and feces within 24 hours. Excretion from the body also occurs through metabolic transformation.

Dosing and administration

Yutibid is recommended for patients over 18 years of age. The tablet form of the medicine is taken on an empty stomach with water or with food. The antibiotic is taken once/twice (morning and evening) per day strictly at the same time. Drops are used as a local medicine in ophthalmology and ENT practice (1-2 drops up to 4 times a day, in especially severe cases the specified dosage is used every two hours during the first day).

The antimicrobial drug dosage is calculated based on the degree of damage and the sensitivity of pathogenic microorganisms. Often, before starting therapy, the resistance of pathogenic microflora to norfloxacin is checked.

Use Utibid during pregnancy

The use of Yutibid is prohibited during pregnancy and breastfeeding, since it has been experimentally revealed that norfloxacin provokes arthropathy (trophic joint damage).

Contraindications

The use of the drug Yutibid has a number of features:

  • prescribed with subsequent monitoring of the condition of individuals prone to epileptic seizures and convulsive syndromes of various etiologies;
  • caution is required in treating patients with liver and kidney dysfunction;
  • the drug affects photosensitivity, therefore patients should avoid prolonged exposure to the sun and visit a solarium;
  • During the treatment of elderly people, cases of tendon rupture (especially Achilles) or tendonitis were sometimes observed, so at the first symptoms of pain and inflammation it is important to consult a doctor in a timely manner.

Contraindications for the use of Yutibid:

  • sensitivity to one of the components of the drug;
  • carrying a baby;
  • breastfeeding period;
  • patients under 18 years of age;
  • congenital anomaly of red blood cells (glucose-6-phosphate dehydrogenase deficiency).

Treatment with an antimicrobial agent should be carried out with diuresis monitoring (the patient should drink enough water). If surgical intervention is necessary, then blood clotting properties are monitored, since during treatment with norfloxacin, an increase in the prothrombin index is often detected.

Side effects Utibid

Therapy with the antibiotic Yutibid may be characterized by unpleasant consequences, which include ringing in the ears, excessive lacrimation, vaginal candidiasis. Clinical practice indicates rare cases of pseudomembranous colitis and anaphylaxis in patients taking norfloxacin.

During treatment, the following side effects of Yutibid may be detected:

  • dysfunctions of the central nervous system – headaches of varying intensity, dizziness, hallucinations, sleep disturbances;
  • digestive organs – bitter taste in the mouth, bouts of nausea/vomiting, anorexia, intestinal disorders, enterocolitis, abdominal pain;
  • cardiovascular problems – pre-syncope or loss of consciousness, vasculitis, low blood pressure, arrhythmia, tachycardia;
  • musculoskeletal disorders – tendonitis, tendon rupture, arthralgia;
  • urinary organs – bleeding from the urethra, urination dysfunction (dysuria), formation of salt crystals, kidney diseases (glomerulonephritis, hypercreatininemia);
  • allergies – itching, skin rashes (eg, hives), swelling, Stevens-Johnson syndrome;
  • hematopoietic system – quantitative decrease in leukocytes (leukopenia), decrease in hematocrit level, eosinophilia.

Overdose

If the patient's general condition worsens and one of the listed symptoms is detected, medical help should be sought:

  • heat, fever, chills;
  • the occurrence of shortness of breath;
  • allergic reactions;
  • a decrease in the level of white blood cells or platelets (leukopenia/thrombocytopenia);
  • gastrointestinal disorders;
  • the appearance of signs of acute hemolytic anemia;
  • kidney dysfunction.

An overdose of the antimicrobial agent Yutibid requires immediate administration of a calcium-containing solution. The combination of the antibiotic with calcium significantly reduces the absorption capacity of the drug in the intestine. The patient with complaints is carefully examined and recommended normalizing, supportive therapy, the basis of which is sufficient fluid intake. Particularly severe situations are resolved by gastric lavage, hospital stay under observation for several days.

Interactions with other drugs

Norfloxacin is an inhibitor of the CYP IA2 enzyme, which determines the ability of the antibiotic to interact with pharmacological substances that form metabolites based on this enzyme.

Interactions of Yutibid with other drugs:

  • a decrease in the therapeutic effect is noted with the simultaneous administration of Yutibid and drugs based on nitrofurantoin;
  • theophylline increases the likelihood of side effects of norfloxacin, the simultaneous use of drugs increases the concentration of theophylline in the blood plasma;
  • Yutibid has an inhibitory effect on the processes of caffeine dehydration, therefore, a reduction in the excretion and an increase in the half-life of caffeine from plasma are often observed. This fact should be taken into account when consuming caffeine-containing beverages and pharmacological agents;
  • the combination of norfloxacin and cyclosporine is undesirable, as it results in an increase in the quantitative content of the latter substance in the blood serum;
  • Yutibid enhances the anticoagulant properties of warfarin;
  • Norfloxacin may reduce the effect of hormonal contraceptives, therefore, during the treatment period, other methods of preventing unwanted pregnancy should be considered;
  • Animal studies have shown that fenbufen, taken in combination with norfloxacin, increases the risk of epileptic seizures;
  • Antacid medications and products containing iron, magnesium, zinc, calcium, and aluminum should not be taken together with a fluoroquinolone antibiotic, as they reduce the absorption activity of the antimicrobial substance (norfloxacin should be taken at least two hours before or four hours after taking such medications).
