Pharmacodynamics

Nimesulide is an NSAID from the sulfonamide category. It has antipyretic, anti-inflammatory and analgesic properties. The active substance of the drug slows down the action of the enzyme cyclooxygenase, which carries out the process of prostaglandin (PG) synthesis, suppressing mainly COX-2.

Pharmacokinetics

After oral administration, the drug is absorbed from the gastrointestinal tract, and reaches its maximum saturation in the blood plasma after 2-3 hours. It binds to plasma proteins by 97.5%. The half-life is 3.2-6 hours. The active substance easily passes through the hematoparenchymal barriers.

Metabolism occurs in the liver with the participation of the cytochrome isoenzyme type P450CYP 2C9. Among the decay products, the main one is the active pharmacological substance hydroxynimesulide (parahydroxy derivative of nimesulide). This component is excreted from the body with bile as a metabolite (it can only be detected in the form of glucuronic acid - approximately 29%). Nimesulide is excreted through the kidneys (approximately 50% of the dose consumed).

Dosing and administration

Movalis should be taken orally at a dosage of 7.5-15 mg once a day.

Naproxen is prescribed to adults in a daily dose of 500-1000 mg in 1-2 doses. The daily dosage can be increased to 1500 mg, but such use is possible only for a limited period (no more than 2 weeks). If there is an acute pain syndrome (for example, with bursitis), the first dose is 500 mg, and then 250 mg every 6-8 hours. For children, the dosage is 10-20 mg / kg / day in 2 doses.

Baclofen is taken orally with food. The initial dosage is 5 mg three times a day. Then the dosage is increased by 5 mg every 3 days until the therapeutic effect is achieved. Often in this case the dosage will not be more than 20-25 mg three times a day.

The maximum daily dose is 100 mg (only for a short period and only in a hospital setting). If the patient has hypersensitivity, the initial daily dose should be 5-10 mg, with a subsequent slow increase. In the case of chronic renal failure or hemodialysis, the daily dose should be 5 mg. Children under 10 years of age should be prescribed 0.75-2 mg/kg per day; children over 10 years - 2.5 mg/kg. The initial dose is 1.5-2 mg/kg four times a day, and the maintenance dose (children aged 1-2 years) is 10-20 mg/day and 30-60 mg/day (children aged 2-10 years). The drug is discontinued gradually (over 1-2 weeks).

Use low back pain pills during pregnancy

Since NSAID tablets slow down the process of PG synthesis, they can have a negative effect on the course of pregnancy and/or fetal development. As a result, the Botallo duct may close earlier than expected, blood pressure in the lungs may increase, and kidney function may be impaired, which may result in renal failure complicated by oligohydramnios. In addition, uterine contractility may decrease, the risk of bleeding may increase, and peripheral edema may occur. Therefore, NSAIDs are prohibited for use by pregnant women.

Muscle relaxants during this period may only be prescribed under the supervision of a physician and only in the event of a serious health condition of the expectant mother.

Contraindications

NSAIDs are prohibited in case of ulcers or erosions of the gastrointestinal tract (especially in an acute form), as well as in case of hypersensitivity to drugs, severe disorders of the kidneys and liver, and in addition, in case of cytopenia.

Muscle relaxants are contraindicated if the patient has the following diseases or conditions:

  • Allergic reactions to the drug and its components;
  • Liver failure, peptic ulcer, myasthenia;
  • For children under 1 year old.

Side effects low back pain pills

Among the most common side effects of most NSAIDs is the occurrence of problems with the gastrointestinal tract, in which ulcers can form. Also quite often, hematological disorders become side effects - panmyelophthisis, thrombocytopenia, and neutropenia.

Side effects of muscle relaxants may include headaches and dizziness, sleep problems, muscle weakness, hallucinations, abdominal discomfort and pain, dry mouth, decreased blood pressure, nausea, slow pulse rate and heartbeat, anaphylaxis, and skin itching and rashes.

Interactions with other drugs

Naproxen increases the toxicity of hydantoin, as well as sulfonamides, indirect anticoagulants and methotrexate (prevents the process of tubular secretion). Reduces hypotension caused by β-blockers, and also weakens the natriuretic and diuretic effects of furosemide. In addition, it reduces the rate of excretion of lithium salts, as a result of which the saturation of this substance in the plasma increases. Antacids containing magnesium with aluminum, as well as sucralfate can reduce the absorption of naproxen.

Movalis greatly reduces the effectiveness of hypotensive drugs. It can also cause potassium and sodium retention, and reduces the effect of saluretics. In addition, it can provoke the progression of heart failure and an increase in blood pressure.
