The number of people suffering from bronchial asthma and obstructive pulmonary disease of chronic type exceeds 200 million. Patients with such diagnoses are familiar with frequent bouts of shortness of breath, lack of oxygen, and suffocation. T-fedrin is included in the group of antiasthmatic drugs of systemic use. Pharmacotherapeutic class code - ATS (Anatomical Therapeutic Chemical) R03D B04, according to which the drug refers to substances that affect the respiratory system (the letter R). The abbreviation R03 refers to medications used for obstructive diseases of the respiratory system. The subsequent labeling corresponds to theophylline and adrenergic drugs.
Indications T-fadrin
The drug T-fedrin is an effective tool for the prevention and treatment of bronchial asthma with bronchial obstruction, chronic bronchial obstructive processes in the lungs and other conditions with bronchospasm.
Indications for T-fedrin apply to asthma with viscous, hard to separate sputum, as well as to the chronic form of obstructive pulmonary disease. The medicine allows you to get rid of the accompanying asthma unstable psycho-emotional state, the main signs of which are panic, anxiety or fear.
Release form
The basis of the drug is: theophylline (100 mg), ephedrine hydrochloride (12 mg), phenobarbital (10 mg). Additional components are: microcrystalline cellulose, magnesium stearate, starch, polyvinylpyrrolidone, talc, sodium starch glycolate.
The form of the T-fadrin preparation is a round, white shade of the tablet, bevelled at the edges with a notch.
Pharmacodynamics
The basis of the combined preparation is theophylline with ephedrine, which have a pronounced bronchodilator effect, as well as phenobarbital (barbituric acid).
The medicine leads to a relaxation of the smooth muscle structures of the bronchi with an increase in their lumen, removes the respiratory spasm, activates the center of breathing, increases the strength and frequency of contraction of the heart muscle, causes the expansion of the vascular bed and the diuretic effect.
Pharmacodynamics of T-fedrin is determined by the characteristics of its components:
- Theophylline (refers to the group of methylxanthines) - the bronchodilating properties of this substance are determined by the ability to quench the activity of the phosphodiesterase enzyme and increase the level of cyclic 3'5 'adenosine monophosphate in tissues (AMP - serves to conduct hormonal signals inside cells). The concentration of the latter lowers the calcium content in the myocytes of the muscular membrane of the bronchi, relaxing by acting on the muscles and stabilizing the membrane of the mast cells. In turn, this will slow the rate of anaphylactic reaction, leading to bronchospasm and swelling of the mucosa. By exerting an expanding effect on the vessels in the kidneys, lungs, skeletal muscles, theophylline lowers the pressure in the arteries of the pulmonary system, reducing the overall counteraction at the peripheral level. This activates the center of breathing, increases the blood volume, the frequency of contractions of the heart muscle and energy loss of the myocardium;
- Ephedrine in its pharmacological properties is a close analogue of adrenaline, stimulating alpha and beta receptors. By means of bronchodilation, serotonin and histamine are not produced. Ephedrine activates the respiratory center, excitingly acts on the central nervous system;
- Phenobarbital has a pronounced hypnotic, sedative and spasmolytic effect. Thanks to this component, T-fedrin is characterized by a soft but at the same time long sedative effect, it allows to correct psychoemotional behavior in patients with bronchial obstructive syndrome.
Pharmacokinetics
The components of the anti-asthmatic substance T-fedrin are absorbed to the maximum in the digestive system.
The connection with plasma proteins of theophylline is about 60%. The distribution of the component in tissues is characterized by uniformity, and its volume index has an average value of 0.5 l / kg. Theophylline is biotransformed in the liver by oxidation and demethylation. The excretion of a substance that has become inactive metabolites occurs through urination. Theophylline has the ability to overcome the placental barrier and to penetrate unimpeded into breast milk. The elimination half-life for adult patients varies from 6 to 10 hours. As for smokers, this indicator increases two or more times.
The pharmacokinetics of T-fedrin is determined by two other components - ephedrine and phenobarbital. The last 50-60% binds to the plasma protein, the fourth part of the component is excreted through the urine in an unchanged form, the half-life itself lasts about 100 hours. Ephedrine also leaves the body in its original state with the exception of a small proportion of metabolites formed in the liver. The elimination half-life is from 3 to 6 hours.
Dosing and administration
Patients of adult age T-fedrin take half or one tablet once a day. In especially severe cases, the dosage can be increased to five tablets per day, which are consumed in two to three doses. The dosage and dosage of the drug for children 6-12 years vary from half to three parts of one tablet.
To avoid disturbance of sleep, the drug is recommended for use during the light period of the day. The duration of therapy is determined individually, based on the severity and characteristics of the disease.
Use T-fadrin during pregnancy
It is not recommended to use T-fedrin during pregnancy, and also when breastfeeding.
Contraindications
Persons who have contraindications to the use of T-fedrin, are divided into groups:
- children under six;
- future mothers and lactating women;
- patients with severe sleep problems;
- people with individual intolerance to one of the components.
The drug is prohibited for patients with arterial hypertension, complicated cases of heart disease, glaucoma, liver and kidney dysfunction, hyperthyroidism, atherosclerosis of cerebral and coronary vessels.
Treatment with T-fadrinom of elderly people is recommended under the supervision of a specialist and observance of special care. Long-term therapy can cause pharmacological dependence. The use of T-fedrin is reflected in the results of sports doping tests.
T-fadrin reduces the concentration of attention, which is unacceptable when driving a vehicle or other jobs that require increased response.
Side effects T-fadrin
Some patients have adverse effects with the use of the drug. Dysfunctions arise from the digestive organs, heart, nervous system. Side effects T-fedrin shows in the form of:
- allergic reactions;
- pain in the stomach;
- symptoms of intoxication (trembling in the body, nausea, vomiting);
- instability of the emotional background (anxiety, nervous overexcitation, problems with sleep);
- detection of cardiac arrhythmia, increased heart rate;
- pressure jumps.
The first signs of a side effect of the drug are an excuse for stopping therapy.
Overdose
In cases of excess dosage prescribed by the doctor or not following the recommendations in the insert instructions, severe conditions occur that are characterized by tremor of the muscles, nausea, vomiting. Arrhythmia, tachycardia, acute heart failure, convulsions, and hallucinations are frequent consequences of non-compliance with the rules of admission.
Overdose of T-fadrinom requires gastric lavage, in some situations - symptomatic treatment.
Interactions with other drugs
The drug T-fedrin is not used simultaneously with reserpine, methyldopa, griseofulvin, lithium, doxycycline.
It should be remembered that a number of components increase the content of theophylline in the blood serum. These include oral contraceptives, cimetidine, erythromycin, allopurinol and others. The presence of rifampicin in the drug on the contrary reduces the concentration of theophylline in the blood. It is not recommended to mix T-fedrin with phenytoin, since the simultaneous administration of these drugs reduces the quantitative content of the latter in the blood serum.
