Sinaflan is a medication from the GCS subgroup. This is a hormonal agent that determines its high therapeutic effectiveness. Used to treat signs of allergies, inflammation and itching.

The acetonide element fluocinolone is synthesized with specific ends of the cell cytoplasm, resulting in potentiation of mRNA binding. This influence leads to the development of various processes associated with cellular metabolism. [1]

Indications Sinaflan

It is used in the treatment of a variety of epidermal inflammations of a non-infectious nature:

  • eczema with different localization ;
  • psoriasis affecting the scalp and body;
  • atopic dermatitis ;
  • seborrheic lesions;
  • lichen planus;
  • neurodermatitis;
  • epidermal pruritus of unknown origin;
  • lupus erythematosus of the discoid type;
  • rash of allergic etiology;
  • erythema polyform;
  • small burns;
  • inflammatory lesion in the area of the insect bite.

Release form

The release of the drug substance is realized in the form of ointment and liniment - inside tubes with a volume of 10 or 15 g. In addition, it is produced in the form of a gel and cream.

Pharmacodynamics

Among the clinically significant properties of the drug, acting on the cell, the following are distinguished:

  • stimulation of the binding of lipocort proteins (among them lipomodulin), which suppresses the activity of phospholipase A2, which is one of the important factors in the progression of inflammation;
  • stabilization of the cell wall, which prevents the development of edema;
  • an increase in the rate of protein catabolism;
  • weakening the migration of B- as well as T-lymphocytes and the destruction of the interaction of these cells;
  • slowing down the accumulation of macrophages with leukocytes in the area of inflammation;
  • inhibition of hyaluronidase activity and a decrease in the indicators of formed leukotrienes and arachidonic acid;
  • changing the regulation of carbohydrate metabolism by reducing tissue consumption of glucose, due to which its blood level can increase;
  • promoting the retention of fluid and sodium ions, stimulating the excretion of calcium with potassium, as well as weakening the ability to assimilate Ca;
  • increased binding of erythripoietins.

A similar effect on cellular metabolism leads to a noticeable drug effect in various epidermal inflammatory processes. [2]

Pharmacokinetics

Fluocinolone acetonide freely penetrates into all skin layers, accumulating inside the stratum corneum (it can be noted even after 15 days from the end of therapy). There is a small amount of medicine inside the circulation.

Biotransformation processes are realized inside the liver with the formation of metabolic elements that do not have activity. Most of the excretion occurs through the kidneys. [3]

The absorption of the drug has a higher intensity in children, when processing large areas of the epidermis, skin on the face or damaged skin, as well as with constant use.

Dosing and administration

Schemes for using the ointment.

The ointment should be applied to the affected area of the skin 1-2 times a day, rubbing it in a little. For an adult, the treatment cycle is up to 10 days, and for a child - a maximum of 5 days; facial skin can only be treated for 1 day.

It is forbidden to cover the area treated with ointment with a bandage (except for use with several types of psoriasis; but it is extremely important to constantly change it).

Sinaflan ointment should be used in case of dry type dermatoses.

Liniment application methods.

Liniment processing is carried out 2-4 times per day. The duration of the treatment cycle is similar to the duration of the application of the ointment.

It is forbidden to cover the treated area with a bandage, but it is allowed to use a sealed bandage on a small area of the epidermis.

Liniment is recommended to be prescribed for dry dermatoses.

Ways to use gel and cream.

The affected areas are treated 1-4 times per day. The entire cycle lasts a maximum of 14 days.

The cream is mainly prescribed in the case of wet type dermatoses, and the gel is used to treat lesions in the scalp.

  • Application for children

Sinaflan is not used in persons under 2 years of age. For children over 2 years of age, the drug is prescribed carefully, under medical supervision and for a short time.

In pediatrics, it is forbidden to treat the skin on the face with medicine.

Use Sinaflan during pregnancy

Due to the limited amount of information regarding the effect of the medication on the fetus and nursing infant, it is prohibited to prescribe it to pregnant women and with hepatitis B.

Contraindications

The main contraindications:

  • severe intolerance associated with the active ingredient and additional elements of the medication;
  • epidermal lesions of an infectious nature (viral, bacterial or fungal);
  • epidermal tuberculosis;
  • epidermal lesions, which are a sign of syphilis;
  • application to wounds;
  • diaper rash;
  • neoplasms and precancerous conditions of the epidermis;
  • the presence of a nevus in the area that needs to be treated with the drug;
  • psoriasis that has progressed to plaque formation;
  • acne rashes (when treating acne, which is acne, an exacerbation of the pathology is possible);
  • ulcers of the trophic type in the leg area (associated with varicose veins);
  • application in ophthalmic procedures and skin treatment in the area of the mammary glands.

Side effects Sinaflan

Side signs develop only occasionally, but at the same time they are quite diverse:

  • lesions of the subcutaneous layer and epidermis: hyperkeratosis, urticaria, itching, folliculitis, furunculosis, burning, contact form of allergic dermatitis and steroid-type acne. In addition, there is an exacerbation of epidermal lesions that existed at the time of initiation of therapy, a disorder of pigmentation, irritation of the epidermis, cutaneous atrophy and hair loss, or increased growth in the area of drug treatment. There is also a rash in the form of spots or papules or peeling of the skin in the area of drug treatment;
  • digestive disorders (when processing large areas of the epidermis): steroid-type gastric ulcer or gastritis;
  • endocrine system damage: glucosuria or an increase in blood sugar values. A child may experience a slowdown in development and growth processes. In the case of treatment of large areas of the epidermis, cushingoid, adrenal insufficiency and steroid-type diabetes mellitus may occur;
  • immune disorders: symptoms of intolerance or allergies, exacerbation of chronic diseases, a decrease in the rate of reparation processes, weakening of immunity and generalization of infectious lesions;
  • problems associated with the work of the CVS: swelling and an increase in blood pressure values.

In the case of processing the outer surface of the eyelids, there is a possibility of developing glaucoma or cataracts.

Overdose

Intoxication can develop in the case of treatment with the drug of large areas of the epidermis, and in addition, with prolonged therapy or treatment of areas with high sensitivity (for example, skin on the face). In addition, there is an increased likelihood of overdose when using drugs in pediatrics. Possible signs include:

  • swelling;
  • increase in blood pressure;
  • itching or burning in the area of drug treatment;
  • an increase in urinary and blood sugar values;
  • cushingoid.

In case of intoxication, symptomatic procedures are performed. At the same time, the use of Sinaflan is being phased out.

Interactions with other drugs

The medicine interacts with many other medicines; it is used with caution when combined with such drugs:

  • systemic corticosteroids - an increase in the therapeutic effect of Sinaflan and the risk of side symptoms from its use;
  • non-hormonal anti-inflammatory drugs - increasing the likelihood of developing systemic and local negative manifestations of both drugs used;
  • antiarrhythmic or antihypertensive substances, potassium and diuretics - weakening the therapeutic effect of these drugs;
  • diuretics (excluding potassium-sparing) - an increase in the risk of hypokalemia;
  • vaccines - an inadequate immune response may occur due to a weakened immune system, which causes an inability to synthesize the normal amount of the required antibodies;
  • immunosuppressants - potentiation of the action of the drug;
  • immunostimulants - suppression of the effects of these drugs in relation to immunity.

Storage conditions

Sinaflan must be kept at the standard temperature for medicines. Do not freeze the medicine.

Shelf life

Sinaflan in the form of an ointment can be used within a 5-year term from the date of sale of the therapeutic product; shelf life of liniment - 24 months.
