Serdolekt has neuroleptic activity.

trusted-source[1], [2], [3]

Indications Serdolekta

It is used in schizophrenia. It is forbidden to use in emergency situations - to stop violations.

Release form

The release is made in the form of tablets with a volume of 4, 12, as well as 16 or 20 mg.

Pharmacodynamics

The drug is an atypical neuroleptic, selectively affecting the structures of the visceral brain. The antipsychotic effect develops due to the blocking of 5HT2-terminations of serotonin and the central D2-terminations of dopamine (the degree of effect is the same). It inhibits the productive manifestations of the disease (delirium, a sense of aggressiveness or psychotic arousal, as well as behavioral disturbances, hallucinations and thinking disorders).

A moderate adrenolytic effect leads to the development of antihypertensive reactions. The drug has no effect on muscarinic and histamine endings, because it does not have a sedative and cholinolytic effect. At the same time, it does not affect the indicators of prolactin - in people who took Serdolekt for a long time (12+ months), these values remained within the normal range.

Pharmacokinetics

After oral administration, it is well absorbed, reaching blood values of Cmax after 10 hours. Eating does not affect the process of drug absorption.

The degree of synthesis with blood protein is 99%. The drug passes through the BBB, and also undergoes metabolism inside the liver (with the participation of CYP2D6 and CYP3A isoenzymes). Metabolic products do not have a neuroleptic effect.

Half-life is not more than 3 days. The main part of the drug is excreted in feces, and the remainder is excreted in the urine.

Dosing and administration

To use orally, 1-fold in a day. The size of the initial portion is 4 mg, and then it is increased by the same 4 mg with 5-day intervals. Often the optimally suitable portion per day is within 12-20 mg of the substance. Only a physician should select a serving at the hospital. If absolutely necessary, a maximum dosage of 24 mg can be used.

Older people need to increase the portion slowly, using titration. In view of the likely development of cardiotoxicity, it is imperative that ECG testing be performed.

Contraindications

Main contraindications:

  • arrhythmias;
  • Hypokalemia or Magnesia;
  • a disease affecting the cardiovascular system and having a severe degree of severity;
  • heart failure;
  • hypertrophy of the myocardium;
  • prolongation syndrome QT values (having a congenital or acquired character);
  • bradycardia;
  • liver failure;
  • the presence of intolerance regarding the elements of drugs;
  • combination with medications that prolong QT indices (terfenadine, erythromycin, as well as antiarrhythmics with gatifloxacin, thioridazine, lithium medications, and astemizole);
  • combination with diltiazem, verapamil or cimetidine.

Caution is required when appointing people who have a history of seizures convulsions.

  • convulsive seizures, late form of dyskinesia (occasionally);
  • orthostatic hypotension;
  • swelling in the legs;

Overdose

During intoxication, there is tachycardia, prolongation of the QT interval, drowsiness, lowering of the level of blood pressure and slurred speech. Sometimes also marked ventricular tachycardia, which has a paroxysmal character.

To eliminate the disorders, it is necessary to cancel the medication, perform gastric lavage, and in addition, prescribe laxatives and sorbents to the patient. The drug does not have an antidote, therefore symptomatic measures are taken, and the condition of the injured at the hospital is monitored - until the overdose symptoms completely disappear.

Interactions with other drugs

It is forbidden to combine the drug with substances prolonging the QT-interval values.

Drugs that slow down the activity of the element CYP2D6 (among those quinidine with paroxetine, as well as fluoxetine), increase blood indicators of the active component of drugs, because of what is required to perform regular examinations on an ECG.

The combination with verapamil, erythromycin or diltiazem causes an increase in blood indices of Serdolekt.

The degree of increase in drug indicators is higher in individuals with reduced activity of the CYP2D6 component.

The use of phenobarbital, rifampicin, carbamazepine or phenytoin can potentiate the metabolism of the active element, due to which the indicators of neuroleptic decrease. In this regard, in order to achieve the desired effect, you may need to increase the dosage.

Serdolekt is used by patients who have schizophrenia or panic attacks. Therapy is always long and continuous. Practically everyone who used the medicine notes that after using it, negative manifestations develop. Among these symptoms in the reviews usually distinguish the blunting of emotional reactions, weight gain and weakening of libido. In case of significant severity of adverse symptoms, it is necessary to consult a doctor regarding the transfer of the patient to another neuroleptic.

Among the advantages of the drug, the absence of an overwhelming influence on cognitive functions is noted - Serdolekt, on the contrary, stimulates their activity.
