Serevent is a bronchodilator medication.

Indications Sereventa
It is used for long-term and continuous therapy of people with treatable airway obstruction associated with asthma (also in situations where night attacks are present), as well as for bronchoconstriction caused by physical activity.
It can also be used in people with obstructive and chronic lung diseases.
The medicine is prescribed to people with chronic bronchitis if there is severe obstruction.
Release form
The medicine is released in the form of a metered aerosol, inside canisters with a special adapter-sprayer, the volume of which is 60 doses. Inside the box - 1 such canister.
Pharmacodynamics
The medicine contains the component salmeterol, a selective β2-adrenergic receptor agonist with a prolonged effect and no effect (when used in therapeutic doses) on the functioning of the cardiovascular system.
The drug causes long-term bronchodilation in people with curable obstruction of the respiratory tract. Its therapeutic effect lasts for a maximum of 12 hours. Regular use of salmeterol helps to achieve a sustainable improvement in pulmonary function and reduce the severity of developing obstructive syndrome, as well as nighttime bronchial constriction in people with asthma.
With constant use of the aerosol, a large number of people with asthma no longer need to take additional bronchodilators. However, it should be noted that Serevent is not suitable for relieving acute bronchial spasms - such people need to constantly keep short-acting bronchodilators with them.
In vitro tests revealed that salmeterol, in addition to its direct bronchodilatory effect, reduced the volumes of histamine, leukotrienes and PG released from labrocytes, which suppressed the early and late stages of the allergic response.
With a single use of the aerosol, a decrease in bronchial hyperreactivity was observed, which demonstrates the anti-inflammatory effect of salmeterol. The mechanism of indirect and direct bronchodilatory effect, and in addition to this suppression of the allergic response and anti-inflammatory effect of the drug is different from the effect of corticosteroids. Because of this, while using Serevent, it is prohibited to cancel the already used corticosteroids (administered through inhalation or orally).
Pharmacokinetics
After inhalation of 50 mcg of the substance 2 times a day, serum values of Cmax of the active element are achieved (a maximum of 200 pg/ml). The concentration of the drug in the serum is quite low, so when administering therapeutic doses, one should not expect the development of a systemic effect. The therapeutic efficacy of the drug is not determined by its indicators in the serum, because the effect of salmeterol is realized in the lung tissue.
Dosing and administration
The aerosol may only be used for inhalation procedures. It must be used continuously, as the medication is ineffective in relieving acute bronchial spasms.
The dosage of the medication, the treatment regimen, and additional therapy are selected by a physician. When treating people with bronchial asthma, it is necessary to constantly monitor the general condition of the patients, and in addition, control pulmonary activity.
Adults often require 2 inhalations of the drug 2 times a day to treat asthma or chronic pulmonary pathologies with obstructive syndrome. For people with severe respiratory tract obstruction, and also if there is no result after using the above doses, the dosage is increased to 4 inhalations 2 times a day. It is prohibited to change the dosage of Serevent on your own, stop therapy, or change the doses of concomitant treatment.
Children over 4 years of age are often prescribed 2 inhalations 2 times a day.

Use Sereventa during pregnancy
No teratogenic effect of the drug was detected during clinical tests, but given the limited amount of existing information regarding testing for this group of patients, Serevent can only be prescribed to pregnant women with the permission of a physician who has carefully analyzed the existing risks.
Since the serum levels of salmeterol are extremely low, its levels in breast milk are also low. Therefore, the likelihood of the drug affecting breastfed infants is also quite low. The decision to use the drug during breastfeeding should be made by the attending physician.
Contraindications
The use of the medication is contraindicated in people with hypersensitivity to salmeterol or auxiliary components contained in its composition.
The aerosol should be administered with great caution to individuals with hyperthyroidism.
Side effects Sereventa
The use of the aerosol may result in the appearance of individual side effects characteristic of most β2-adrenoreceptor agonists - headaches, tremors, dizziness and palpitations. Often these manifestations are curable, and their severity decreases (or they disappear completely) with continued therapy or a reduction in the dosage of the drug.
In some patients, after the administration of the drug, heart rhythm disorders (such as atrial fibrillation, supraventricular tachycardia, and extrasystole) may develop. Such symptoms are observed rarely, and are more likely to occur in people with cardiovascular diseases, and also when inhaling too large portions of the aerosol.
As with other inhalation medications, paradoxical bronchial spasms may occur in some people when using Serevent. In such cases, it is necessary to discontinue the use of the drug and prescribe the use of bronchodilators with a rapid effect, and in addition, with the doctor's prescription, to select an alternative treatment option.
Rarely, when using salmeterol, patients have experienced pain in the joints, cramps affecting the muscles, and signs of intolerance (angioedema, rashes, and anaphylaxis).
The use of an aerosol can cause local negative symptoms, including irritation of the mucous membrane in the mouth and throat.

Overdose
After inhalation of large doses of salmeterol, patients may experience muscle spasms, headaches, dizziness, tremors and tachycardia, and in addition, the risk of other negative symptoms characteristic of the active element of the drug may increase.
In case of poisoning with Serevent, it is necessary to prescribe cardioselective β-blockers, and in addition, if necessary, to carry out other symptomatic procedures. It is necessary to take into account that persons with a history of bronchial spasms should be prescribed β-blockers with extreme caution.
Interactions with other drugs
Combining selective and non-selective β-blockers with Serevent is prohibited.
An increase in the serum level of salmeterol may be observed when using the drug together with ketoconazole and other strong inhibitors of the CYP3A4 component. In addition, when combining these drugs, the risk of adverse cardiovascular events (including increased systolic blood pressure, tachycardia, and prolongation of the QT interval) may increase.

Storage conditions
Serevent should be kept out of reach of children, away from sunlight, open flames and heating devices. Temperature indicators are standard for pharmaceuticals.
Medicinal containers must not be punctured, disassembled, heated or deformed, even after the medicinal substance in them has run out.
Shelf life
Serevent can be used within 24 months from the date of release of the medicinal substance.
Use in children
The drug is not recommended for use in children under 4 years of age (because there have been very few clinical trials conducted for this age group).
