Ravel SR is an antihypertensive drug used as prescribed by a doctor. Let's consider the indications for its use, medicinal properties and contraindications.

The clinical and pharmacological group of the drug is diuretics and antihypertensive agents. The drug has vasodilatory, i.e. vasodilating, diuretic and hypotensive properties. Its pharmacological action is similar to thiazide diuretics and sulfonamide derivatives. The active substance - indapamide inhibits sodium absorption in the cortical segment of the nephron, increasing urine output and renal excretion of sodium and chloride ions.

The hypotensive effect lasts for 24 hours. The drug affects vascular tone, reduces left ventricular hypertrophy and decreases arteriole resistance. The effectiveness is also maintained in patients with impaired renal function, without affecting the concentration of lipids in the blood plasma.

Release form

Release form – film-coated tablets of prolonged action. One package contains a blister of 20, 30 or 60 pieces. One tablet contains 1.5 mg of the active substance indapamide.

The advantage of the tablet form is that after ingestion, the tablets quickly dissolve and are absorbed, exerting a pharmacological effect on the lesion.

Pharmacodynamics

Pharmacodynamics of Ravel SR indicate the antihypertensive effect of the drug. The tablets have diuretic properties, since the active substance belongs to the sulfonamide derivatives and is similar to thiazide diuretics. Indapamide destroys the reabsorption of sodium ions in the renal tubules, which entails increased excretion of chlorine and sodium ions in the urine, increasing diuresis.

In addition to the diuretic effect, the active component affects the tone of blood vessels. This reduces the overall peripheral and arteriolar resistance. The antihypertensive effect at high doses does not affect the level of blood pressure reduction, but increases diuresis. Therapeutic doses do not affect carbohydrate and lipid metabolism, but, like thiazide diuretics, reduces left ventricular hypertrophy.

Pharmacokinetics

Pharmacokinetics of Ravel SR are the processes that occur with the drug after administration. Indapamide is quickly and completely absorbed from the gastrointestinal tract. Food intake significantly slows down absorption, but does not affect the quality of absorption. The maximum concentration in the blood plasma is achieved 12 hours after taking a single dose. Repeated administration of a double dose causes fluctuations in concentration towards a decrease, but this does not lead to its accumulation. Binding to plasma proteins is at the level of 75-79%.

Ravel is metabolized in the liver, the elimination period is 14-24 hours. 70% is excreted as metabolites, 5% unchanged and 20% with feces as inactive metabolites. If the drug is taken by patients with renal failure, this does not affect the pharmacokinetics.

Dosing and administration

The method of administration and dosage are individual for each patient, so they should be selected by the attending physician. The normal dosage is considered to be 1.5 mg once a day, that is, one tablet per day.

It is advisable to take the medicine in the morning with a sufficient amount of liquid. At higher doses, medical supervision, monitoring of heart rate and blood pressure are required.

Use Ravel SR during pregnancy

The use of Ravel SR during pregnancy is not recommended. The drug may cause fetoplacental ischemia with the risk of delayed fetal development. The tablets are also prohibited for use during breastfeeding, as the active substance is excreted in the mother's milk.

The use of an antihypertensive drug is possible only when the therapeutic benefit for the woman is more important than the potential risk to the fetus. As a rule, instead of dangerous drugs, expectant mothers are prescribed safer drugs that will not harm the fetus and the female body.

Contraindications

Contraindications to the use of Ravel SR are based on the action of the active components of the pharmaceutical product. The tablets are not used for such disorders as:

  • Individual intolerance to the components of the drug
  • Hypersensitivity to sulfonamides
  • Impaired liver function
  • Severe renal failure
  • Hepatic encephalopathy
  • Hypokalemia
  • Childhood age of patients
  • Pregnancy and lactation.

Side effects Ravel SR

Side effects of Ravel SR appear with increased dosages or exceeding the recommended treatment period. The drug can cause arrhythmia, increased heart rate, orthostatic hypotension and hypokalemia (ECG changes). Most often, patients complain of headaches, nervousness, dizziness, asthenia, abdominal pain. In addition, constipation, diarrhea, dyspepsia, hepatic encephalopathy are possible.

Disturbances occur in all organs and systems. In the urinary system, this manifests itself as polyuria, frequent infectious diseases and nocturia. Allergic reactions are possible, i.e. skin itching, urticaria, maculopapular rash and hemorrhagic vasculitis. The tablets provoke cough, sinusitis, metabolic and hematopoietic disorders. In rare cases, exacerbation of lupus erythematosus is possible.

If Ravel is prescribed to patients with impaired liver function, but this may cause encephalopathy and the diuretic should be discontinued. Long-term treatment causes the development of hyponatremia. In case of insufficient nutrition and simultaneous use of several drugs by elderly patients and people with ascites, heart failure or liver cirrhosis, medical supervision and prevention of hypokalemia are required.

Overdose

Overdose is possible with prolonged use of the drug and exceeding the recommended dosage. Symptoms of acute poisoning manifest as disturbances of water-electrolyte balance, i.e. hypokalemia and hyponatremia. In addition, attacks of nausea, vomiting, dizziness, headaches, confusion, convulsions, drowsiness are possible.

Overdose is treated by gastric lavage. Patients should take activated charcoal or any other absorbent. Restoration of water and electrolyte balance is also required. In case of other side effects, symptomatic therapy is carried out as indicated.

Interactions with other drugs

Interaction of Ravel SR with other drugs is possible according to medical indications. Tablets are not used with lithium preparations, as this reduces the excretion of lithium by the kidneys. When taken with amphotericin B for intravenous administration, stimulant laxatives or mineralocorticoids, there is a risk of an additive effect. Digitalis and baclofen are prescribed with special caution, as it enhances the hypotensive effect.

If the drug is used with potassium-sparing diuretics, hyperkalemia may develop in patients with diabetes mellitus and renal failure. Regular monitoring of potassium concentration in blood plasma and ECG parameters is required. Antiarrhythmic drugs can provoke polymorphic ventricular tachycardia and bradycardia. When used simultaneously with neuroleptics and tricyclic antidepressants, there is a risk of additive effects.

Storage conditions

Storage conditions for Ravel SR are similar to the rules for storing other tablet preparations. The storage temperature should not exceed 25°C. The medicine should be kept in a dry place, protected from sunlight and out of reach of children.

The tablets must be in their original packaging. If the above rules are not followed, the medicine loses its medicinal properties and is prohibited for use.

Shelf life

The expiration date is 24 months from the date of manufacture (indicated on the package). After its expiration, the tablets are prohibited from being taken. If the medicine has changed color or acquired an unpleasant odor, it must be disposed of.
