Palixid-Richter (synonyms: Doperezil, Alzepil, Arisept, Yasnal) refers to drugs whose action is aimed at neurotransmitter processes of the peripheral nervous system, in particular, at afferent nerves and cholinergic synapses.

Indications Palixid-Richter

Palixid-Richter is used in the symptomatic treatment of such neurodegenerative diseases as:

  • Alzheimer's disease (moderate severity),
  • presenile and senile dementia of the Alzheimer's type,
  • dementia of vascular (cerebrovascular) etiology,
  • dementia in diffuse Lewy body disease,
  • minor cognitive impairment (thinking, memory, speech) of the amnestic type.

Release form

Palixid-Richter is available in the form of film-coated tablets of 5 and 10 mg (20 pieces per blister).

Pharmacodynamics

The active substance of Palixid-Richter - donepezil hydrochloride - reduces the activity of the enzyme acetylcholinesterase, which affects the biosynthesis and metabolism of acetylcholine - one of the main endogenous neurotransmitters. The breakdown and neutralization of acetylcholine leads to a decrease in neurohumoral and synaptic transmission of excitation in the endings of peripheral nerves.

Donepezil hydrochloride, by inhibiting acetylcholinesterase, stops the destruction of acetylcholine, which increases the level of acetylcholine in the hippocampus and cerebral cortex, activates cholinergic receptors and synapses, and generally improves the transmission of excitation to the central nervous system. Thus, the progression of Alzheimer's disease and dementia slows down, and the condition of patients improves.

Pharmacokinetics

After taking Palixid-Richter, the maximum concentration of donepezil hydrochloride in the blood plasma is observed on average after 3.5 hours. In this case, more than 90% of the drug binds to plasma proteins, and its half-life is about three days.

70% of the drug Palixid-Richter is transformed by the liver (under the action of isoenzymes CYP3A4 and CYP2D6), 30% of the dose taken is not metabolized. Metabolites and unchanged donepezil can remain in the body for more than seven days, excreted in urine (about 60%) and feces.

Dosing and administration

The drug Palixid-Richter is taken orally once a day (in the evening), regardless of food intake. The initial dose is 5 mg. In this dosage, the drug is taken for a month, after which the doctor evaluates its effectiveness. The dosage can be increased to 10 mg per day (one-time).

The method of administration of this drug requires regular monitoring of the patient's condition.

Use Palixid-Richter during pregnancy

The use of Palixid-Richter during pregnancy and lactation has not been studied, since there is no clinical experience of its use in pregnant and lactating women.

Contraindications

Contraindications for the use of this drug include: hypersensitivity to donepezil hydrochloride and age under 18 years.

The use of Palixid-Richter for the treatment of patients with cardiac arrhythmia, bronchial asthma, gastric ulcer and duodenal ulcer requires caution.

Side effects Palixid-Richter

To date, the list of side effects of Palixid-Richter includes: headache, dizziness, fainting, increased fatigue, convulsions, sleep disorders, hallucinations, agitation, aggression; skin rash and itching; nausea, vomiting, gastrointestinal disorders; urinary incontinence.

In addition, treatment with drugs containing donepezil may cause an individual reaction, and it is not possible to predict its nature.

Overdose

There is no information regarding overdose of Palixid-Richter.

Interactions with other drugs

Due to limited experience with the use of this drug, the interaction of Palixid-Richter with other drugs has not been sufficiently studied.

Donepezil hydrochloride is recognized to be able to enhance the action of drugs for the relief of cardiac and muscle spasms, as well as cholinomimetic drugs that interact with cholinergic receptors and activate them, like endogenous acetylcholine.

And the simultaneous use of Palixid-Richter with anticholinergic drugs (for example, Glycopyrrolate) can lead to jumps in blood pressure and an increase in heart rate.
