Oftamirin belongs to a group of drugs that act on the sense organs; ATC code S03D; international name – Myramistin; synonym – Okomistin.

Indications Oftamyrin

As an antiseptic, Oftamirin is used for ophthalmological diseases that develop as a result of bacterial, viral or fungal infection: conjunctivitis and blepharoconjunctivitis in acute and chronic form, keratitis (inflammation of the cornea), keratouveitis (inflammation of the cornea and vascular membrane of the eyeball), as well as for eye injuries (mechanical, thermal, chemical).

In ophthalmological surgical interventions, Oftamirin can be used to prevent infection.

Pharmacodynamics

The antiseptic effect of Oftamirin is provided by the quaternary ammonium base compound included in its composition – miramistin. This is a cationic detergent (surface-active substance) with a strong antiseptic effect.

Due to the presence of strongly basic groups in the molecules of miramistin, the drug acts on bacteria as a toxin: it oxidizes proteins and thereby destroys the integrity of the cytoplasmic membranes of microbial cells. As a result, this leads to a violation of the osmotic balance of the microbial cell and its death.

Okomistin has a bactericidal effect against all gram-negative and gram-positive, anaerobic and aerobic, asporogenous and spore-forming bacteria and bacteroids, as well as the Herpes simplex virus.

The drug exhibits antifungal action on fungi of the genus Candida, Aspergillus, Penicillium, Rhodotorula rubra, Torulopsis gabrata, Malassezia furfur), as well as on dermatophytes of the genus Trichophyton and Microsporum.

Pharmacokinetics

Oftamirin is applied locally – in the form of instillation, and its active substance is not absorbed by either the mucous membranes or the skin and, therefore, does not enter the bloodstream.

Dosing and administration

Oftamirin drops should be instilled into the eyes - directly into the conjunctival sac (the space between the eyeball and the eyelid), without touching the eye, eyelashes and eyelid with the tip of the dropper bottle. If the patient has contact lenses, they should be removed before instillation and put back on 15-20 minutes after the procedure.

The standard dose for infectious and inflammatory eye diseases is 1-2 drops in each eye at least four times a day. The duration of the course of treatment is determined by an ophthalmologist.

In case of thermal or chemical burns, before using Oftamirin, the eyes should be washed, and then 1-2 drops should be instilled into each eye every 10 minutes for 60-120 minutes.

Use Oftamyrin during pregnancy

Use during pregnancy and lactation is possible, but it should only be prescribed by the attending physician - in case of extreme necessity.

Contraindications

This product is contraindicated for use in the presence of individual intolerance to miramistin; drops are not prescribed to children.

Side effects Oftamyrin

Despite the generally good tolerability of Oftamirin, some patients may experience local irritation in the form of itching and redness of the eyes immediately after using the drops.

Overdose

Overdose with local use of Oftamirin has not been recorded.

Interactions with other drugs

Oftamirin can be used in combination with other antimicrobial agents for local use, provided that a 15-minute break is observed between their instillations.

Storage conditions

Storage conditions of Oftamirin: at temperatures up to +30°C, freezing is not allowed.

Shelf life

The shelf life of an unopened bottle is 3 years; from the moment the bottle is opened, the shelf life of the drug is 25 days.
