Pharmacodynamics

Omeprazole is a substance in the group of inhibitors of the proton pump, it slows the activity of the enzyme H + - K + - ATPase in the gastric lining cells. Thus, it blocks the final stage of hydrochloric acid evolution. As a result, the basal and stimulated release of pepsinogen and gastric juice, both day and night, is inhibited. Omeprazole after a one-time procedure begins to work in the first hour, and its effect lasts for a day. The maximal efficiency reaches 2 hours. In patients with duodenal ulcer, 20 mg of substance allow for maintaining the level of intragastric pH at the level of 3.0. With the termination of taking the drug, the release of hydrochloric acid by parietal cells is restored to the normal level after 3-5 days.

Dosing and administration

Intravenous injection of the drug with GERD or development of peptic ulcer having a duodenal or gastric location is administered in an amount of 40 mg / day. For an adult.

The initial dose of the drug in the syndrome of Zollinger-Ellison is 60 mg / d, but this index may increase. If the daily dose of omeprazole is more than 60 mg, it is recommended to divide it into 2 doses.

The initial dose of the drug in peptic ulcers, accompanied by bleeding in the GI tract, will be 80 mg / day intravenously. Further, starting from the 3rd day of the treatment course and until the 21st day, the drug is taken orally - 20 mg / d.

For treatment of hepatic insufficiency it is possible to prescribe the drug in a dosage of a maximum of 10-20 mg. Intravenous injection should be administered for 20-30 minutes. To make a batch, the powder contained in 1 bottle is added to a 5% injection of dextrose or 0.9% sodium chloride solution (100 ml). Such a solution can be used for 1 day, no more.

Oral administration of Omez capsules. They are consumed whole, not chewing. To treat peptic ulcers with duodenal or gastric localization, as well as reflux esophagitis, a dosage of 20 mg / day before breakfast is prescribed. Therapeutic course lasts 14 days. If the two-week treatment was ineffective (the peptic ulcer did not heal), the course is prolonged for another 2 weeks (until the final scarring occurs). Often, peptic ulcers in the area of the duodenum after the application of Omez (40 mg / day) are cured for 1 month, and reflux esophagitis in erosive form or gastric ulcers - for 2 months.

For effective treatment of Zollinger-Ellison syndrome, the drug for internal use is administered at a dosage of 60 mg / d. The duration of the treatment course will depend on what the clinical picture of the disease is. The maintenance dose can fluctuate within the limits of 20-120 mg / day - exact figures individually for the patient are determined by the attending physician. If the required daily dose of Omez is more than 80 mg, the medication is divided by 2 times.

Interactions with other drugs

Since the enzyme cytochrome P450 is involved in the metabolism of the drug in hepatocytes, it must be used with caution when taking warfarin and phenytoin, as well as diazepam. In such combinations, omeprazole will reduce the rate of excretion of metabolites of these drugs, because of which it is necessary to reduce their dose.

When omeprazole is combined with clarithromycin, an increase in the concentration of both substances in the plasma is observed.
