Omez is an effective antiulcer drug from the group of proton pump inhibitors. The main component of the drug is omeprazole.

Indications Omez

Among the main indications for prescribing the drug:

  • Erosive-ulcerative form of esophagitis;
  • Peptic ulcers in the duodenum or stomach;
  • Ulcers of the above organs that have arisen as a result of taking non-steroidal anti-inflammatory drugs;
  • Stress-induced ulcer;
  • Pancreatitis in acute and chronic form;
  • A recurrent peptic ulcer that is located in the duodenum or stomach; the patient is prone to complications (treatment is aimed at preventing relapse);
  • Zollinger-Ellison syndrome;
  • In the process of complex treatment for eradication of Helicobacter pylori;
  • GERD (treatment to prevent recurrence);
  • Acute mast cell leukemia.

Release form

Omez is available in capsules (each containing 20 mg of the main component (omeprazole)). One pack contains 10 or 30 capsules.

Omez 40 is a dried powder from which a solution for injections is prepared. One bottle contains 40 mg of the main substance.

Pharmacodynamics

Omeprazole is a substance that belongs to the group of proton pump inhibitors, it slows down the activity of the enzyme H+-K+-ATPase in the parietal cells of the stomach. Thus, it blocks the final stage of hydrochloric acid secretion. As a result, the level of basal and stimulated secretion of pepsinogen and gastric juice, both day and night, is suppressed. Omeprazole begins to act within the first hour after a single dose, and its effect lasts for 24 hours. It reaches its maximum effectiveness after 2 hours. In patients with a duodenal ulcer, 20 mg of the substance allows maintaining intragastric pH at 3.0 for 17 hours. After stopping the drug, the secretion of hydrochloric acid by parietal cells is restored to normal levels after 3-5 days.

Dosing and administration

Intravenous injections of the drug for GERD or the development of a peptic ulcer located in the duodenum or gastric region are administered in an amount of 40 mg/day for an adult.

The initial dose of the drug for Zollinger-Ellison syndrome is 60 mg/day, but this figure can be increased. If the daily dose of omeprazole is more than 60 mg, it is recommended to divide it into 2 doses.

The initial dose of the drug for peptic ulcer accompanied by bleeding in the gastrointestinal tract will be 80 mg/day intravenously. Then, starting from the 3rd day of the treatment course and up to the 21st day, the drug is taken orally - 20 mg/day.

For the treatment of liver failure, the drug can be prescribed in a maximum dosage of 10-20 mg. The intravenous injection should be administered over 20-30 minutes. To prepare a portion, the powder contained in 1 bottle is added to a 5% dextrose injection solution or 0.9% sodium chloride solution (100 ml). This solution can be used for 1 day, no more.

Oral use of Omeza capsules. They are taken whole, without chewing. For the treatment of peptic ulcers with duodenal or gastric localization, as well as reflux esophagitis, a dosage of 20 mg / day before breakfast is prescribed. The therapeutic course lasts 14 days. If two-week treatment is ineffective (the peptic ulcer has not healed), the course is extended for another 2 weeks (until final scarring occurs). Often, peptic ulcers in the duodenum after using Omeza (40 mg / day) are cured in 1 month, and erosive reflux esophagitis or gastric ulcers - in 2 months.

For effective treatment of Zollinger-Ellison syndrome, the drug for internal use is prescribed at a dosage of 60 mg/day. The duration of the treatment course will depend on the clinical picture of the disease. The maintenance dose can range from 20-120 mg/day - the exact figures are determined individually for the patient by the attending physician. If the required daily dose of Omeza is more than 80 mg, the drug is taken in 2 doses.

Use Omez during pregnancy

It is prohibited to prescribe Omez to pregnant or nursing mothers.

Contraindications

The drug is contraindicated if the patient is allergic to omeprazole or other active ingredients of the drug.

Side effects Omez

Side effects of the drug include:

Nervous system: increased sweating, dizziness and severe headaches, frequent drowsiness or insomnia, increased excitability, paresthesia, vision problems, depression and the occurrence of hallucinations in patients predisposed to them.

Gastrointestinal tract: diarrhea, stomatitis, abdominal pain, taste bud disturbances, dry mouth, constipation, temporary increase in serum transaminases, as well as liver function tests.

Musculoskeletal system: muscle pain and weakness, joint pain.

Skin: appearance of skin rash or urticaria, development of erythema multiforme.

Others: occurrence of peripheral edema, fever.

Overdose

Overdose may result in symptoms such as arrhythmia, nausea, confusion, and an increase in the above side effects.

Interactions with other drugs

Since the cytochrome P450 enzyme system is involved in the process of drug metabolism in hepatocytes, it should be used with caution when taken simultaneously with warfarin and phenytoin, as well as diazepam. In such combinations, omeprazole will reduce the rate of excretion of metabolites of these drugs, which is why their dose should be reduced.

When omeprazole is combined with clarithromycin, an increase in the concentration of both substances in plasma is observed.
