Indications Haloperidol richtera

The drug is prescribed:

  • in acute and chronic mental disorders occurring against the background of general overexcitement, hallucinatory and delusional states (patients with schizophrenia, affective states, psychosomatic disorders);
  • for inappropriate behavior, paranoia, schizoid personality changes (including in children), for the treatment of autism;
  • with hereditary chorea, nervous tics;
  • in case of persistent, prolonged hiccups of neurological etiology;
  • in case of persistent attacks of vomiting (which cannot be eliminated by conventional means);
  • in preparation for surgery.

Release form

Haloperidol richter is produced as an injection solution or tablets.

Injection solution - colorless or slightly yellow, without signs of turbidity or sediment. Available in 1 ml ampoules (containing 5 mg of the active component), 5 pcs in a package made of thick cardboard.

Tablet form: light tablets of 1.5 mg or 5 mg, flattened, round, with a dosing notch, with the logo “ǀ|ǀ” on one surface.

In addition to the active ingredient, there are a number of additional ingredients, which are represented by starch, silicon dioxide, magnesium stearate, talc and lactose. The package contains 25 tablets.

Pharmacodynamics

The drug Haloperidol Richter is a psycholeptic, a derivative of butyrophenone. It has a pronounced antipsychotic and antiemetic effect. This ability of the drug is explained by the possibility of blocking the central dopamine and α-adrenergic receptors in the brain. Blocking hypothalamic receptors leads to a decrease in overall temperature indicators, to increased prolactin production. The antiemetic effect is associated with the inhibition of dopamine receptors in the trigger area of the vomiting centers. Significant antipsychotic effect is advantageously accompanied by a mild sedative effect.

Haloperidol richter is capable of potentiating the action of barbiturates, narcotic painkillers, anesthetics, as well as some other drugs that can, to varying degrees, depress the activity of the central nervous system.

Pharmacokinetics

The drug is absorbed by passive transport of substances, in non-ionized form, mainly directly from the small intestine. The biological availability of the drug is estimated at approximately 65%. When taken orally, the maximum concentration in the bloodstream is observed after 2-6 hours, when administered by injection - within 20 minutes.

The therapeutic effect is already detected at a drug level in plasma of 20 to 25 mg/l. Binding to plasma proteins is approximately 92%.

The drug is metabolized in the liver, and the metabolite does not have pharmacological activity.

The plasma half-life when taken orally is 24 hours, and when administered by injection, it is 21 hours. Most of the metabolite is excreted through the intestines (up to 60%), the rest through the urinary system. It easily overcomes both the blood-brain and placental barriers, and is also found in breast milk.

Dosing and administration

The methods of application and doses of Haloperidol Richter are determined by the doctor depending on the phase of the pathology and the individual reaction of the patient to the drug. We provide the average statistical dosages of the drug, most often used in medicine.

To stabilize psychomotor reactions at the beginning of treatment, intramuscular injections of the drug from 2.5 to 5 mg three times a day are used. The maximum daily dosage is 60 mg. If the sedative effect is stable, the injection administration of the drug is replaced by oral administration.

Elderly patients can take from 0.5 to 1.5 mg of Haloperidol, which is 0.1-0.3 ml of injection solution. The maximum daily dose is 5 mg.

Children from 3 years of age use from 0.025 to 0.05 mg of the drug per day, divided into two injections. The maximum permitted dosage is 0.15 mg per kilogram of the child's weight per day.

Haloperidol Richter tablets are taken half an hour before meals. They should be washed down with water or milk (to reduce the negative impact on the digestive tract). The initial daily dosage can be from one and a half to five milligrams, divided into 2 or 3 times. The dosage is gradually increased by an average of 2 mg until a stable therapeutic effect is achieved. The maximum daily dose of the drug is 100 mg.

The average duration of the treatment course is from 2 to 3 months. Then the drug can be prescribed in maintenance quantities (after the exacerbation phase), with the dosage gradually reduced over several weeks.

For antiemetic effect, Haloperidol Richter is taken orally from 1.5 to 2.5 mg.

Use Haloperidol richtera during pregnancy

Studies of the use of Haloperidol Richter during pregnancy have not found a significant increase in the number of detected congenital anomalies in fetal development. Meanwhile, there is evidence of the occurrence of congenital anomalies when using Haloperidol in combination with other drugs during pregnancy. For this reason, the appointment of Haloperidol Richter to pregnant women is permissible only in situations where the expected positive effect for the expectant mother is higher than the danger to the fetus.

The active substance of the drug is found in breast milk. Therefore, many people prefer to temporarily stop breastfeeding in cases where treatment is unavoidable. There are known cases when a breastfed child developed extrapyramidal symptoms while the nursing mother was taking Haloperidol.

Contraindications

There are a number of known contraindications to the use of Haloperidol Richter:

  • depressed state of the central nervous system, comatose states;
  • diseases of the central nervous system occurring against the background of extrapyramidal disorders (parkinsonism);
  • basal ganglia disorders;
  • children under 3 years old;
  • depressive states;
  • allergic hypersensitivity of the body to the active or additional ingredients of the drug.

Haloperidol Richter is prescribed with extreme caution in cases of cardiac decompensation, arrhythmia, severe kidney or liver damage, a tendency to epilepsy, increased thyroid function, active prostatitis, and increased intraocular pressure.

Side effects Haloperidol richtera

Standard dosages of the drug are usually tolerated without problems. Higher doses may be accompanied by side effects:

  • extrapyramidal disorders (dystonia, muscle rigidity, hypo- and hyperkinesis, athetosis, akathisia, etc.);
  • anxiety, depressive syndrome, epileptic seizures;
  • fatigue, sleep disturbances;
  • cardiac disorders, drop in blood pressure, abnormal cardiographic parameters;
  • allergies, skin rashes, disorders of the sebaceous and sweat glands;
  • dyspeptic disorders, weight loss or gain, increased salivation;
  • menstrual irregularities, sexual desire disorders, impotence;
  • swelling of the extremities, deterioration of urine flow.

Overdose

Overdose of Haloperidol Richter may be accompanied by the following reactions:

  • muscle weakness, trembling in the limbs, feeling of fatigue;
  • blood pressure instability;
  • A large overdose may cause coma, respiratory dysfunction, abnormal heart rhythms, and convulsions;
  • an increase in temperature as a sign of the development of neuroleptic syndrome.

In case of overdose, stop taking the drug. If the drug was taken orally, perform gastric lavage, after which use sorbent preparations (activated carbon tablets, sorbex).

If respiratory dysfunction develops, artificial ventilation may be used. In case of a sharp drop in blood pressure, circulatory support may be required by administering plasma or albumin preparations, dopamine. It is unacceptable to use epinephrine in such situations!

According to indications, it is possible to resort to prescribing antiparkinsonian medications (benztropine mesilate). Diazepam, glucose, vitamin and nootropic drugs are used intravenously.

Hemodialysis is not performed due to its ineffectiveness. There is no specific antidote.

Interactions with other drugs

The drug Haloperidol Richter may interact with some other medications, which must be taken into account when using them in combination.

Haloperidol richter is able to suppress the effect on the central nervous system of narcotic painkillers, sleeping pills, tricyclic antidepressants (amitriptyline), anesthetics and ethyl alcohol.

A decrease in the therapeutic effect was observed with the combined use of Haloperidol and antiparkinsonian drugs.

Haloperidol reduces the effect of adrenaline and also helps to lower blood pressure and increase heart rate when combined.

Potentiates the effect of medications that lower blood pressure.

When combined with anticonvulsants, their dosage must be increased, as the threshold of seizure activity decreases.

The effect of Haloperidol is reduced by drinks such as tea or coffee.

The effect of indirect coagulants may be reduced, and the toxicity of tricyclic antidepressants and drugs that inhibit MAO may be increased.

Combination with the atypical antidepressant bupropion increases the risk of epileptic seizures.

The neurotoxic effect is enhanced when used simultaneously with lithium preparations, and this effect is irreversible.

The effect of bromocriptine is reduced.

The antipsychotic effect of Haloperidol is reduced and its side effects are aggravated when combined with anticholinergic, antiparkinsonian and antihistamine medications.

It is not prescribed with thyroxine, as this increases the toxic effect of Haloperidol.

Use with anticholinergics may provoke the development of glaucoma.

Storage conditions

Haloperidol Richter in tablet form or as an injection solution is recommended to be stored at temperatures from +15° to +30° C, in a place inaccessible to children. The drug should be protected from exposure to sunlight and other bright lighting.
