Dalizol is a drug used to reduce or eliminate the toxic effects of antitumor therapy. The international name of the drug is calcium folinate.

Dalizol is effectively used to correct side effects during chemotherapy, is the main addition to treatment with methotrexate. The drug is prescribed as a counteraction to methotrexate to prevent toxic effects and side effects.

Indications Dalisol

Dalizol is prescribed to reduce toxicity and counteract substances that weaken the effect of folic acid (methotrexate) during cancer therapy.

Dalizol is also used as part of combination therapy to improve the quality of life of patients with advanced colorectal cancer.

Release form

Dalizol is available as a clear or light yellow solution that is used for intravenous or intramuscular administration.

Pharmacodynamics

The active ingredient of Dalizol is calcium folinate, which is necessary to support the body during cancer therapy. Calcium folinate is used to reduce the toxic effect of chemotherapy. The most toxic is methotrexate, which also reduces the level of folic acid in the body, which in turn leads to a severe form of megaloblastic anemia.

Calcium folinate is transported in the body along the same pathway as antitumor drugs and competes for cellular transport, stimulating the efflux of cytotoxic drugs.

Dalizol has a protective effect on cells that suffer during chemotherapy, helping to restore the source of various coenzymes of folic acid.

The drug is often used to enhance the toxic effect of 5-FU.

In addition, the drug is used for the prevention and treatment of folate deficiency in cases where oral administration of folic acid does not produce the expected effect.

Dalizol is also used to treat megaloblastic anemia.

Pharmacokinetics

The bioavailability of Dalizol after intramuscular administration of the solution is approximately the same as after intravenous injection. The half-life is about 35 minutes for the active L-form.

Active metabolites are excreted after approximately six hours after injection. About 90% of inactive metabolites are excreted in the urine, 8% in the feces.

Dosing and administration

The medication is used as intravenous or intramuscular injections.

The course of treatment and dosage are prescribed by the attending physician, taking into account the age, severity of the disease, concomitant diseases, as well as the dosage and treatment regimen of cytotoxic drugs, in particular methotrexate.

The usual dosage is 15 mg, administered no later than 24 hours after the start of methotrexate therapy. Further doses of Dilysol are administered every six hours for 72 hours.

During therapy, serum creatinine levels and methotrexate levels should be determined once daily.

Use Dalisol during pregnancy

Dalizol may be used by pregnant women only as prescribed by a doctor, in cases where the benefits of treatment for the woman outweigh the possible risks to the unborn child.

There is no data on whether calcium folinate can penetrate into breast milk, therefore it is recommended to stop breastfeeding during treatment with Dalizol.

Contraindications

Dalizol is contraindicated in case of hypersensitivity of the body to any component of the drug; the drug is also not used for pernicious anemia or other types of anemia caused by a lack of vitamin B 12.

Side effects Dalisol

During treatment, Dalizol may provoke a disruption of the immune system; in rare cases, anaphylactic shock, allergic reactions, sleep disturbances, excitability, depressive states (after treatment in large doses), digestive disorders, and inflammation of the mucous membranes may occur.

Dalizol therapy often causes nausea, vomiting, severe forms of diarrhea (fatal outcome is possible).

Overdose

The drug in elevated doses can reduce the effect of chemotherapy with drugs that inhibit the action of folic acid in the body.

There are no known complications following treatment with Dalizol in high doses.

Interactions with other drugs

Dalizol in combination with drugs that inhibit the effect of folic acid on the body (perimethamine, cotrimoxazole) can reduce or completely neutralize the effect of the latter.

Dalizol may reduce the effect of antiepileptic drugs (phenobarbital, primidone, succinimide) and increase the frequency of epileptic seizures.

Combined treatment with Dalizol and 5-fluorouracil leads to an increase in the toxic effect of the latter.

Storage conditions

The medicine should be stored in a dry place, away from sunlight, at a storage temperature of 2-8°C. When frozen, the effectiveness of the medicine decreases.

Dalizol should be kept out of the reach of children.

Shelf life

The medicine is valid for two years from the date of manufacture; do not use the drug after the expiration date or if the integrity of the ampoules with the solution is damaged.
