Pharmacodynamics

The active substance of Dalfaz retard is alfuzosin, which shows activity when taken orally.

Alfuzosin blocks adrenoreceptors located in the triangle of the bladder, the prostate gland, the urethra.

With benign formation in the prostate gland, a smooth muscle contraction occurs, which prevents the flow of urine and leads to disruption of the bladder.

Dalfaz retard directly affects the smooth muscles of the prostate gland and improves the outflow of urine, reduces pressure in the urethra, eases urination. 

Pharmacokinetics

Dalfaz retard reaches the highest concentration in the body 3 hours after ingestion. The half-life of the drug in the body is eight hours.

The therapeutic effect of the drug is not dependent on food intake.

The active substance (alfuzosin) of the drug disintegrates mainly in the liver, 11% of the substance is excreted by the kidneys in an unchanged state, 80-90% - in the form of metabolites with feces.

At the age of more than 75 years, the absorption period of the drug is reduced, and the highest concentration in the body is higher.

In case of serious violations of the liver, the half-life and bioavailability are increased in comparison with healthy patients.

If the kidneys are disturbed, the process of cleansing the body and the level of distribution of the drug are increased. 

Dosing and administration

Dalfaz retard tablet is taken whole by mouth, it is recommended to drink one glass of water. In day you need to take 2 tablets (morning and evening).

Adult patients are prescribed 5 mg of the drug (one tablet) twice a day.

In the elderly, with reduced blood pressure, when taking medications to reduce blood pressure, one tablet is prescribed in the first days (preferably in the evening), then, taking into account the clinical response, the specialist can increase the dosage, but not more than two tablets per day.

With renal failure, dose adjustments are not required.

When liver failure is prescribed, 1/2 tablet (2.5 mg) once a day, then the specialist can increase the dose (not more than 1 tablet per day).

Contraindications

Overdose

Dalfaz retard when taken in an elevated dose causes a decrease in blood pressure, rapid heart rate.

In case of an overdose, hospitalization of the patient and treatment in hospital with vasoconstrictive drugs and plasma substitutes is necessary, after improvement of the condition it is recommended to observe the patient for a minimum of 24 hours.

Purification of the blood in this case is ineffective, since the active substance binds to proteins in a high degree. 

Interactions with other drugs

Dalfaz retard can not be taken concomitantly with urapidil, prosazin and other alpha1-adrenoreceptor blockers.

Caution should be taken Dalfaz retard with drugs to lower blood pressure, ketoconazole, itraconazole, ritonavir.

Simultaneous reception of nitrates can provoke a rapid rhythm of cardiac contractions, lowering blood pressure.

Anesthesia during medication treatment can cause instability of blood pressure.
