Psoriasis is a disease that is quite difficult to treat, as many medications at best bring only temporary relief. However, therapeutic procedures must be carried out to at least prevent the process from worsening and spreading.
One of the drugs of choice for this pathology may be Daivobet, a combined antipsoriatic agent from the glucocorticosteroid group.

Release form
Daivobet can be produced in several medical forms:
- external cream (consists of 50 mcg calcipotriol and various excipients);
- external solution (consists of 52.2 mcg calcipotriol hydrate and additional substances);
- external ointment, tube of 15 g, 30 g or 60 g (the ointment consists of 0.05 mg calcipotriol, 0.5 mg betamethasone and some additional substances).
The most commonly used is the ointment, which is a homogeneous substance of white or cream color, without a specific odor.

Pharmacodynamics
The active ingredient of the ointment, calcipotriol, is a synthetic preparation of the active product of the metabolism of natural vitamin D. It helps to inhibit the proliferation of cells in the outer layer of the skin, while their morphological differentiation is significantly accelerated. The active substance also has a small effect on calcium metabolism in the body.
The properties of betamethasone are aimed at reducing signs of inflammation, relieving itching, increasing local immunity, and strengthening the vascular network at the site of the lesion. The exact mechanism of action of the drug has not yet been clarified.

Pharmacokinetics
The absorption of the active ingredients of the drug through the skin is 1-5% of the applied dosage. Due to the limited absorption of this drug, its effect on the body is exclusively local. The percentage of absorption of the drug can be increased by applying an occlusive dressing over the ointment. The drug is retained in the tissues for several days.
The metabolites formed in the liver are pharmacologically inactive, the partial elimination time is short. The active substances as metabolites are excreted from the body through the urinary system and with feces.
Dosing and administration
Daivobet ointment is a preparation for external use. In case of psoriasis, it is enough to apply it to the affected skin in a thin layer once a day. It is not recommended to use more than fifteen grams of the preparation per day.
The therapeutic course should last at least 1 month. Further use of the drug should be agreed with the doctor.
In case of extensive psoriatic lesions of the skin (more than 30%), it is recommended to refrain from using Daivobet in favor of another drug that can be prescribed for the treatment of total forms of psoriasis.
Use Dyvobet during pregnancy
Psoriasis does not increase the risk of miscarriage or early labor, and does not provoke the development of congenital anomalies in the fetus. However, the use of certain antipsoriatic drugs during pregnancy, including Daivobeta, may pose a certain degree of risk to the process of bearing a child.
There is no reliable information about the danger of this drug for pregnant women, so the advisability of taking the drug should be determined by a doctor.
It should be noted that most experts are of the opinion that psoriasis treatment should not be prescribed during pregnancy unless absolutely necessary: it is better to wait with therapeutic measures until the end of the lactation period.
Contraindications
Among the contraindications to the use of this drug, the following can be distinguished:
- metabolic disorders, tissue mineralization disorders, in particular, calcium metabolism;
- allergic sensitivity to any components of the drug;
- disorders of liver and kidney function;
- other associated skin lesions caused by viruses, fungal infections, bacteria and parasites;
- acne, dermatitis, tuberculous and syphilitic skin lesions, ulcerative diseases of the outer skin, increased capillary fragility, ichthyosis;
- other forms of psoriasis, except vulgaris;
- pregnancy and breastfeeding;
- the patient's childhood.
It is also not recommended to apply the drug to the skin of the face and mucous membranes.
Side effects Dyvobet
The most common side effects are the appearance of an itchy rash on the body, accompanied by a burning sensation and pain.
Sometimes, at the initial stages of treatment, the disease may worsen, but if treatment is not stopped, the symptoms gradually begin to subside.
Signs of skin irritation, redness, follicle formation and swelling may appear at the site of application of the ointment.
Excess calcium may be present in the urine.
Large doses of the drug can provoke a decrease in the functionality of the adrenal glands, as well as the development of Itsenko-Cushing syndrome.
Overdose
Overdose of Daivobet can be caused by long-term and uncontrolled treatment, often carried out independently, without the knowledge of a doctor.
Symptoms of overdose include increased calcium concentration in the circulating blood, suppression of the adrenal glands and pituitary system.
After discontinuing the drug, the symptoms usually disappear and the imbalance of systems and organs stabilizes.
Chronic drug intoxication requires gradual and gentle withdrawal of the drug.
Storage conditions
When storing the drug, it is strictly forbidden to heat it above 25 o C.
