The combined drug Celestoderm-B with garamycin, according to its pharmacological action, belongs to external dermatological agents with antibacterial and anti-inflammatory properties (ATC code D07CC01).
Other trade names: Belogent, Betaderm, Akriderm Genta, Diprogent, Kuterid G.
Indications Celestoderm-B with garamycin
Celestoderm-B with garamycin is intended for the treatment of atopic, seborrheic, allergic, contact, exfoliative, radiation, and infectious dermatitis; microbial eczema (including nummular); itchy dermatoses complicated by infection.
Release form
Ointment, cream (in tubes of 15 and 30 g).
Pharmacodynamics
The active ingredients of the drug Celestoderm-B with garamycin are the synthetic glucocorticosteroid (adrenal cortex hormone) betamethasone and the aminoglycoside antibiotic garamycin (gentamicin sulfate).
Betamethasone relieves inflammation by inhibiting the release of arachidonic acid and the synthesis of inflammation mediators from it, slowing the movement of leukocytes to the site of inflammation, blocking the activity of lysosomal enzymes and other biochemical processes. Itching is reduced by stabilizing the membranes of immune cells and reducing the release of histamine into the blood. And a decrease in the synthesis of tissue immunoglobulins provides the anti-allergic effect of the corticosteroid in the composition of Celestoderm-B ointment with garamycin.
Gentamicin sulfate (Geramycin) has a bactericidal effect by disrupting the cell membranes of gram-positive and gram-negative microorganisms, which leads to disruption of protein synthesis and death of bacteria.
Pharmacokinetics
Betamethasone in the ointment Celestoderm-B with garamycin is adsorbed into the bloodstream in small quantities (about 14%), binds to plasma proteins, undergoes metabolism in the liver and is eliminated through the kidneys and intestines.
There is no information regarding the pharmacokinetics of Garamycin in the official instructions. Although, it is noted that when applying the ointment to large areas and with prolonged use, the level of systemic absorption of this agent increases.
Dosing and administration
Celestoderm-B with garamycin is used locally, the ointment (cream) should be applied to the affected areas of the skin in a thin layer - once or twice a day.
Use Celestoderm-B with garamycin during pregnancy
Use during pregnancy and breastfeeding is prohibited.
Contraindications
Celestoderm-B with garamycin is contraindicated in tuberculosis of the skin, mycosis, candidiasis, lesions of the herpes simplex virus and chickenpox, syphilitic rash. The ointment is not used in the treatment of children under 6 months, as well as after vaccination.
Side effects Celestoderm-B with garamycin
The use of Celestoderm-B with Garamycin may be accompanied by urticaria, acne, dermatitis, inflammation of the hair follicles and excessive hair growth, increased dryness of the skin and burning.
Long-term use of this ointment (cream) may cause systemic side effects such as weight gain, bone fragility, increased blood pressure and blood sugar levels. And in children, there may be dysfunction of the adrenal cortex, increased cerebral pressure (with bulging fontanelle) and development of edema of the optic nerve.
In addition, gramicidin can cause paresthesia, convulsions, headaches, fever, angioedema and the development of superinfection (or the addition of a fungal infection).
Overdose
May increase side effects.
