Celanide is a herbal medicine for the treatment of cardiovascular diseases. Let's look at the instructions for this drug and the features of its use.

International name: lanatoside C. The drug belongs to the pharmacological group of cardiac glycosides. Digitalis glycosides.

The active substance is the crystalline glycoside digilanide, which is obtained from Digitalis lanata Ehrh. The drug has cardiotonic properties, does not accumulate and effectively affects the heart. Celanide has a broad therapeutic effect compared to other digitalis glycosides. Its effect is similar to the effectiveness of intravenous administration of Strophanthin. But our drug restores heart rhythm better and prolongs diastole, while cumulating less Digitoxin.

Indications Celanide

The cardiotonic drug is used only as prescribed by a doctor. Let's consider the main indications for the use of Celanide:

  • Chronic circulatory failure grades I–III
  • Acute heart failure
  • Tachyarrhythmic tachysystolic form of atrial fibrillation
  • Paroxysmal tachycardia
  • Other cardiovascular disorders

During treatment, constant medical supervision and strict individual selection of dosage are required. If the patient has pulmonary heart disease, liver, kidney or coronary insufficiency, electrolyte imbalance, the doctor selects the minimum effective dose.

In case of myocardial infarction and angina, the use of the drug is possible only in case of severe heart failure. But in this case it can lead to myocardial ischemia. For pregnant women with heart failure, patients with bradycardia and kidney pathologies, the drug should be used under strict medical supervision.

Release form

The drug is available in several forms, which makes the treatment process convenient for each patient. Celanide is available in the form of:

  • Tablets – each capsule contains 25 mg of the active substance celanide. The tablets are white and disintegrate in water within 10 minutes. Each package contains 30 capsules.
  • Solution 0.05% (drops) - contains 500 mg of celanide, 455.5 ml of ethyl alcohol 95%, 150 g of glycerin and about 1 liter of water for injection. Available in dropper bottles of 15 and 10 ml.
  • Injection solution 0.02% – available in 1 ml ampoules. Contains 200 mg celanide, 148.7 ml ethyl alcohol 95%, 150 g glycerin and water for injection. Each package contains 10 ampoules.

Celanide-kmp

The international and chemical name of the drug is lanatoside C. Pharmacotherapeutic group Celanide-kmp - cardiac glycosides. The drug is available in tablet form, each capsule contains 25 mg of the active substance.

  • Celanide is prescribed for the treatment of cardiovascular diseases. The dosage and method of administration are selected for each patient individually. As a rule, 1-2 tablets 3-4 times a day are enough to achieve a stable therapeutic effect.
  • The tablets are contraindicated for use in cases of severe bradycardia, unstable angina, hypersensitivity to pharmaceuticals containing digitalis and glycosides. Not used for stage II-III atrioventricular block, hypertrophic subaortic stenosis, Morgan-Adams-Stokes syndrome.
  • The medication may cause side effects. In this case, symptoms of severe bradycardia, polytopic extrasystole, and slowing of atrioventricular conduction are observed. Decreased appetite, constipation, diarrhea, anxiety, increased agitation, allergic reactions on the skin, and drowsiness are possible.
  • Failure to comply with the rules of use causes symptoms of overdose. It manifests itself as an increase in side effects. To eliminate adverse symptoms, it is recommended to reduce the dosage or stop taking the drug. In case of intoxication, gastric lavage is performed, absorbents and saline laxatives are taken.

Pharmacodynamics

The active component of Celanide is a primary crystalline glycoside obtained from the leaves of foxglove woolly. The pharmacodynamics of the drug indicate that this substance has a stronger cardiotonic effect than other cardiac glycosides. Celanide slows the heart rate, increases myocardial contractility, reduces venous pressure and increases diuresis.

The drug exhibits a small cumulative effect. The mechanism of action is based on the effect on the potassium-sodium pump of myocardiocytes, the activity of phosphodiesterase and the exchange of calcium ions. The glycoside affects the state of cyclic adenosine monophosphate, which is involved in the energy supply of the contractile process of myocardial cells.

Pharmacokinetics

After oral administration, the drug is quickly absorbed in the intestine. Pharmacokinetics indicate that the cardiotonic effect manifests itself 2 hours after administration. The maximum therapeutic effect occurs after 4-6 hours. Bioavailability is 15-45%.

Plasma protein binding is 25%. The half-life is 28 to 36 hours. The drug is absorbed in the gastrointestinal tract at a level of 40%. It is metabolized in the liver, with about 35% excreted per day. The glycoside is excreted in the urine and feces unchanged and in the form of inactive metabolites.

Dosing and administration

The use of the drug depends on the doctor's recommendations, indications and its form of release. The method of administration and doses of Celanide are individual for each patient. As a rule, Celanide is prescribed for oral use in tablets of 25 mg and drops of 0.05% solution. Intravenous administration of a 0.02% solution for injection is also possible. To achieve a quick therapeutic effect, 1-2 ml of 0.02% solution is administered intravenously 1-2 times a day. Tablets are prescribed 1-2 pieces 2-3 times a day, drops 10-25 drops of 0.05% solution 3-4 times a day.

After achieving the therapeutic effect, the daily dose is reduced. The maintenance dose when taking tablets is 1-2 pcs. per day, with intravenous administration 2-1 ml of 0.02% solution intravenously or 10 drops of 0.05% solution orally. For long-term maintenance therapy, take ½ tablet 2 times a day.

Use Celanide during pregnancy

Treatment of cardiovascular diseases in expectant mothers is complicated by the fact that many drugs are contraindicated. The use of Celanide during pregnancy is possible only with appropriate indications and strict medical supervision. Digitalis glycoside is not recommended for use during lactation, since the active substance can be excreted in breast milk and affect the development of the child.

Contraindications

Celanide is not used in case of individual intolerance to the plant glycoside of foxglove. The main contraindications to the use of the product are:

  • Acute myocarditis (inflammatory disease of the heart muscles)
  • Disease of the internal cavities of the heart
  • Thyroid diseases
  • Sharp organic changes in blood vessels and heart
  • Severe cardiosclerosis
  • Heart rhythm disturbances

The above-described pathologies are an absolute contraindication to the use of the drug. Safer drugs are selected for treatment.

Side effects Celanide

If the doctor's recommendations for using the product are not followed, a number of side effects occur. Celanide can cause the following adverse symptoms:

  • Heart rhythm disturbances
  • Extrasystole
  • Change (dissociation) of the source of the heart rhythm

If the above-described effects occur, it is necessary to reduce the dosage of the drug or stop taking it. In between uses, the patient is prescribed potassium preparations. In case of nausea and vomiting, absorbents are indicated, and the pulse may slow down.

Overdose

Taking high doses and treatment for a long period of time cause adverse reactions. Overdose manifests itself as an increase in the severity of side effects. To eliminate them, it is recommended to reduce the dose/number of doses or stop using the drug.

If overdose symptoms are pronounced, it is recommended to perform gastric lavage, take absorbents or saline laxatives. Oxygen therapy has therapeutic properties. If overdose has caused sinus bradycardia, bradyarrhythmia, ventricular arrhythmia or AV block II-III degree, then the use of Lidocaine, Phenytoin or an artificial cardiac pacemaker is indicated.

Interactions with other drugs

For the treatment of cardiovascular diseases, several medications are usually prescribed. This speeds up the recovery process. The interaction of Celanide with other drugs is possible with the appropriate medical permission. It is the doctor who selects a set of drugs, focusing on the possibility of their simultaneous use and the individual characteristics of the patient's body.

  • Calcium preparations increase the risk of glycoside intoxication, while potassium reduces the toxic effect.
  • Lithium, psychotropic and sympathomimetics increase the risk of arrhythmia.
  • Corticosteroids and thiazide diuretics cause electrolyte imbalance and increase the toxicity of Celanide.
  • Phenobarbital reduces the effect of Celanide due to acceleration of metabolism and induction of liver enzymes.
  • The use of saluretics increases side effects, while potassium-sparing agents reduce them.
  • Antacids reduce the absorption of the drug in the gastrointestinal tract when taken orally.
  • Aminazine increases blood clotting and weakens the effect of the glycoside.
  • Quinidine, Amiodarone, Verapamil increase the concentration of the drug in blood plasma.
  • Lincomycin increases the risk of developing pseudomembranous colitis.

Storage conditions

To preserve the medicinal properties of the drug, it is necessary to observe its storage conditions. All forms of cardiac glycoside are recommended to be stored in the original packaging, in a dry place protected from moisture, sunlight and inaccessible to children. The storage temperature should be between 15°C and 25°C.

Shelf life

Celanide must be used within 48 months from the date of manufacture. This expiration date corresponds to all forms of the drug. Expired medication is prohibited to take, as it can cause uncontrolled side effects from many organs and systems.
