Cardimax is a medicinal product with antihypoxic and antianginal effects. Its use leads to stabilization of cellular energy exchange processes (at the same time, the medicine actively affects cells that have been affected by ischemia or hypoxia).
The use of the therapeutic agent reduces the likelihood of arrhythmia, increases the patient's physical endurance and increases the level of coronary reserve. [ 1 ]
Indications Cardimax
It is used as a treatment for coronary heart disease. It is also prescribed to prevent the development of angina attacks - in monotherapy or as part of complex treatment.
The drug can be used in the treatment of cochleovestibular disorders of ischemic etiology, including hearing loss and tinnitus, as well as chorioretinal vascular lesions with an ischemic element.
Release form
The therapeutic element is released in tablets - 10 pieces inside a separate strip. Inside the pack - 3 or 10 strips.
Pharmacodynamics
The medicine prevents the reduction of intracellular ATP reserves inside cardiomyocytes, and at the same time maintains the ACE and ATP indices inside brain cells. Cardimax demonstrates a membrane-protective effect and helps the full functioning of membrane ion channels.
The drug selectively suppresses the mitochondrial enzyme, which results in the activation of phospholipid metabolism, a decrease in acidosis, and accumulation of free radicals. The drug affects the change in metabolic processes within the ischemic myocardium - glucose is oxidized, not fatty acids, which helps prevent intracellular acidosis. [ 2 ]
The drug reduces the frequency of angina attacks without changing the heart rate. Because of this, the dose of nitroglycerin can be reduced during its use. [ 3 ]
Pharmacokinetics
The drug is absorbed at a high rate (Tmax is 1.8±0.7 hours); protein synthesis is approximately 16%; distribution volume is 4.2 l/kg.
Approximately 80% of the administered dose is excreted via the kidneys; 62% of the substance is excreted unchanged. The half-life is approximately 6 hours.
Intrarenal clearance rates of trimetazidine are directly correlated with the level of CC. Intrahepatic clearance decreases with age, due to which the half-life term may increase to 12 hours in individuals over 65 years of age.
Long-term administration of the drug (over a period of more than 15 days, 20 mg, 2 times a day) does not change its pharmacokinetics. Food intake does not affect the absorption of the drug.
Dosing and administration
The medication is taken in a dosage of 20 mg (1 tablet), 2-3 times a day. It is taken with food. A maximum of 60 mg of the therapeutic agent is allowed per day.
The medicine can be used for long-term therapy, but the duration of the treatment cycle should be selected by a doctor, individually for each patient.
- Application for children
The use of the medication by persons under 14 years of age is prohibited.
Use Cardimax during pregnancy
Cardimax should not be prescribed during pregnancy.
Contraindications
Main contraindications:
- severe sensitivity to the components of the drug;
- breastfeeding period.
People with severe renal impairment (with a creatinine clearance level below 30 ml per minute) should use the medicine very carefully.
People with liver failure and the elderly should reduce the daily dosage of the drug or extend the interval between doses, due to the fact that these groups experience a prolongation of the half-life of the drug.
Side effects Cardimax
Side effects include:
- dyspeptic symptoms, nausea and pain in the epigastric region;
- manifestations of allergies;
- sleep disorders – such as insomnia.
Interactions with other drugs
The administration of Cardimax in combination with theophylline, nitrates, digoxin, as well as with Ca antagonists, digitalis substances, β-blockers, lipid-lowering agents and heparin does not lead to the appearance of any drug interactions.
Storage conditions
Cardimax should be stored in a dry and dark place, out of reach of children. Temperature level – in the range of 10-25оC.
Shelf life
Cardimax is approved for use for a period of 36 months from the date of sale of the therapeutic product.

Analogues of the drug are the substances Cardutal, Normex and Cardital with Trimet, as well as Metazidine and Tricard with Cardazine-Health, Kratal and Preductal with Hyperzar. Also on the list are Energoton with Triductan and Advokard.
