Pharmacodynamics

The active ingredient of this drug is a synthetic analogue of the adrenal cortex hormone, the non-halogenated steroid methylprednisolone aceponate.

Methylprednisolone aceponate has lipophilic molecules and penetrates the lipid membrane of skin cells. When interacting with protein receptors of the cytoplasm, 6α-methylprednisolone-17-propionate is formed by hydrolysis, which enters the cell nucleus and acts as a stimulator of the synthesis of intracellular polypeptides, which, in turn, inhibit the release of arachidonic acid, which is necessary for the formation of prostaglandins and leukotrienes - inflammation mediators.

In addition, there is a temporary decrease in the activity of tissue macrophages, fibroblasts, keratinocytes and lymphocytes, which leads to a blockade of the release of interleukins - specific mediators of the immune system.

Thus, Advantan relieves local inflammation and irritation in the form of itchy rashes, redness and lichenification (thickening of areas of the epidermis) caused by increased cell proliferation.

Pharmacokinetics

After application to the skin in areas of inflammation and rashes, Advantan penetrates through the upper layers of the skin, where it is present in fairly high concentrations for 24 hours. The active substance of the drug is active only in areas of inflammation, and no more than 2.5% of 6α-methylprednisolone-17-propionate enters the systemic bloodstream. It is inactivated by binding to glucuronic acid in the blood plasma.

Advantan does not accumulate in the body and is completely excreted in the urine after 30-32 hours.
