Agen 10 medication refers to cardiovascular drugs - calcium ion antagonists. It is able to block calcium channels, mainly acting on the vascular wall.

Indications of the agena 10

 The medication is used in the treatment of the following diseases and conditions:

  • arterial pressure increase of the second and third stages;
  • different course of ischemic heart disease (eg, stable angina, vasospastic angina);
  • chronic course of heart failure in the stage of decompensation.

Release form

 The medication Agen 10 is produced in a tablet form with a light, almost white hue. The form of the tablet is elongated, it has a dividing notch on one side, the letter A (on the first letter of the name of the preparation) and the figure 10 (denotes the dosage of the active component). The active substance of the medication is amlodipine, which is presented as amlodipine besylate. Of the additional substances are MCC, dicalcium phosphate, sodium starch-glycolate, magnesium stearate. 

Pharmacodynamics

Medicinal antianginal and hypotensive, synthetic derivative of dihydropyrin, representing a group of calcium channel blockers of the second generation. Reduces signs of cardiac muscle ischemia, enlarging the lumen in the coronary arteries and distal arterioles, thereby reducing myocardial oxygen consumption, smoothing cardiac preload. Under the influence of the drug decreases the intensity of attacks of pain in heart failure, the need for nitroglycerin decreases.

 The drug gently expands the lumen of the vessels, which also contributes to a decrease in pressure. The drug, taken once, ensures the stabilization of blood pressure during the day. With IHD, the drug exhibits antisclerotic and cardioprotective effects, and also reduces hypertrophic signs in the left ventricular myocardium tissues that occur with constantly inflated blood pressure values. During a severe chronic course of heart failure reduces the likelihood of cardiac arrest.

 Agen 10 does not affect the conduction of the heart and contractility of the heart muscle, but it can normalize the frequency of heart beat, lower platelet aggregation and reduce the risk of blood clots. The drug helps increase glomerular renal filtration and remove excess sodium from the bloodstream.

 Agen 10 does not affect the metabolic processes in the body.

Pharmacokinetics

 When ingested, the preparation is well absorbed, with the maximum amount of active ingredient in the serum being detected within 6-12 hours after the drug is administered.

 The drug becomes bioavailable at 64-80%.

 The distribution percentage can be about 21 l / kg. The amount of food consumed and the presence of it in the stomach do not affect the degree of absorption of the drug.

 Due to scientific research, it is reliably determined that on average 97% of the distributed active substance binds to plasma proteins.

 The half-life of the drug can be from 35 to 50 hours. Stability in the amount of the substance in the blood serum is reached on the 7-8th day of constant use of the drug.

 Biotransformation of the active component occurs in the liver, where conversion into an inactive form of metabolites is observed.

 The active component of the drug is removed from the body through urination: up to 10% of the total amount - in unchanged form, up to 60% - inactive metabolites.

 The active component of the drug is not able to give in to hemodialysis.

Dosing and administration

Agen 10 should be taken orally, half the tablet (5 mg) per day at a time. For 1-2 weeks, the dosage of the medication is gradually increased to one tablet (10 mg) once a day.

 With prolonged therapy, hypertension is prescribed a maintenance dosage of not more than ¼-½ tablets daily.

 Angina strain requires the appointment of a ½ or whole tablet daily at a time.

 Patients with low height and underweight, elderly, patients with liver pathology and chronic ischemic heart disease should be prescribed in a dosage of ¼ tablets daily, if necessary gradually increasing the dose to ½ and the whole tablet a day.

 The tablet is taken without chewing, regardless of food intake, washed down with water, tea or juice.

 The completion of the course of therapy is carried out by gradually lowering the dosage to prevent the formation of withdrawal syndrome. It is not recommended to stop sharply the use of medication.

Use of the agena 10 during pregnancy

Since there is no scientifically valid data on the use of this medication in the production and lactation, the use of Agen 10 during these periods is not recommended. The decision to prescribe the drug to pregnant and lactating women can be made by a specialist individually, based on an assessment of possible risks and the benefits of treatment. 

Contraindications

 Use of the drug is not recommended in the presence of hypersensitivity to the components of the drug or to other medications dihydropyridine group.

 Do not prescribe Agen 10 under reduced pressure, during pregnancy and breastfeeding.

 With caution use the drug in the acquired form of heart disease (mitral and aortic stenosis), with a progressive chronic form of cardiac insufficiency, in the acute phase of myocardial infarction, as well as in elderly patients.

 The drug is not recommended for use with any other medicines without the approval of the treating doctor.

 During treatment with Agen 10 medication, the weight of the patient should be monitored, since this drug can provoke the development of obesity. It is also necessary to observe the condition of the gums, regularly visiting the dentist.

 The drug is used only by adult patients, since its use in children has not been studied.

Side effects of the agena 10

With the use of Agen 10, there may be some side effects:

  • insomnia or drowsiness, irritability, tearfulness, depressive or anxious condition;
  • increased fatigue, headaches, the appearance of a tendency to a sharp change of mood;
  • convulsions, disorders of consciousness, sensitivity disorders in the limbs, trembling in the hands, asthenia;
  • pain in the stomach, nausea, thirst, weight change, stool disorders, inflammatory diseases of the digestive system;
  • rapid or slow heart rate, puffiness of the lower part of the trunk, extrasystole, chest pain, persistent lowering of pressure, migraine;
  • rapid and painful act of urination, violation of sexual possibilities and libido;
  • pain in muscles, joints, bones;
  • dermatitis, alopecia, allergic manifestations, itching dermatosis;
  • deterioration of visual functions, conjunctivitis, double vision and soreness in the eyes, tearing disorder;
  • noise in the ears, nosebleeds, hyperhidrosis;
  • gynecomastia male type, pain in the spine, obesity;
  • a feeling of heat, redness of the face.

 Seldom there can be disorders of coordination, memory disorders, signs of agitation (motor anxiety).

 A blood test can show bilirubinemia, high activity of blood enzymes.

Overdose

Signs of an overdose Agen 10 can be considered the development of rapid heart rate, lowering blood pressure due to excessive relaxation and vasodilation.

 If signs of an overdose are found, gastric lavage should be carried out, a suspension of sorbent agents (sorbex, activated charcoal) should be taken. If necessary, you can inject injections of drugs to support the heart and blood vessels, carry out activities for symptomatic therapy. Measures in case of overdose should include mandatory control over diuresis and volume of circulating blood, assessment of the pulmonary system and heart condition.

Interactions with other drugs

The following medicines may interfere with the action of the drug aimed at lowering blood pressure:

  • non-steroidal anti-inflammatory drugs (aspirin, ibuprofen, diclofenac);
  • hormonal preparations-estrogens (in connection with the delay of sodium ions);
  • sympathomimetic drugs (epinephrine, ephedrine, salbutamol);
  • α-adrenostimulators (norepinephrine, phenylephrine, metameninol, mefentermin, methoxamine).

 Drugs that inhibit microsomal oxidation (ketoconazole, erythromycin, cyclosporine) increase the concentration of Agen 10 in the blood serum, which can affect the increase in side effects of the drug. At the same time, drugs that induce microsomal liver enzymes (rifampicin, phenobarbital, phenytoin) can reduce the amount of Agen 10 in the blood.

 Diuretics (hydrochlorothiazide, indapamide, furosemide, spironolactone), β-adrenoblockers (timolol, metoprolol, labetalol), ACE inhibitors (captopril, enalapril, fosinopril) contribute to the intensification of the hypotensive and antianginal action of Agen 10. Also, the antihypertensive effect is enhanced by α-adrenoblockers (tropafen , prazosin), amiodarone, quinidine, antipsychotics (aminazine, haloperidol, zeldox).

 The drug has no effect on the effects of digitoxin and warfarin.

 The use of the drug together with medicines containing lithium (carbonate, oxybutyrate, lithium nitinate) can lead to the appearance of signs of neurotoxicosis, which is characterized by the development of dyspeptic phenomena, coordination disorders.

 The effects of quinidine and procainamide together with Agen 10 can provoke an elongation of the QT interval in ECG.
