Peptika kombipek is a medicine used for ulcers and GERD. It is a complex compound that helps to destroy Helicobacter pylori.
Omeprazole creates a suitable environment for the development of antimicrobial activity, and at the same time directly acts against H. Pylori. [1]
Clarithromycin is a macrolide antibiotic; it inhibits protein binding inside the bacterial cell, interacting with the 50S microbial ribosome subunit. [2]
Tinidazole demonstrates the effect of relatively simple bacteria, and in addition to most of the anaerobes. [3]
Omeprazole, in combination with antibiotic treatment, results in a rapid reduction in ulcer symptoms and healing.

It is used in the treatment of ulcers in the gastrointestinal tract or gastritis , which has a chronic form (associated with the influence exerted by Helicobacter pylori), in people with some sensitivity to tinidazole.

Pharmacodynamics
Omeprazole is an antiulcer drug with antisecretory activity. It weakens the secretion of gastric hydrochloric acid and inhibits the effect of H + / K + - ATPase, preventing the development of the final stage of the release of hydrochloric acid.
Clarithromycin has been shown to act against streptococci agalactia, campylobacter jeuni, Staphylococcus aureus, chlamydia pneumoniae with haemophilus influenzae, mycoplasma pneumoniae with listeria monocytogenes, as well as gonococci and moraxella catarrhalis. In addition, the list includes ureaplasma urealiticum, legionella pneumophila, Mycobacterium avium, Helicobacter pylori with Hansen's rods and pertussis, and in addition propionibacterium acne and Toxoplasma gondii.
Clostridia, gardnerella, peptostreptococci with bacteroids, fusobacteria and peptococci with eubacteria, intestinal lamblia, vaginal Trichomonas and dysentery amoebae are sensitive to tinidazole.
Pharmacokinetics
Omeprazole.
When administered orally at high speed, it is fully absorbed. About 90-95% of the element is synthesized with proteins.
The half-life of a substance is 0.5-4.5 hours, but anti-secretory activity persists for a period of 24+ hours.
The main part (77%) of omeprazole is excreted in the urine. Among the isolated elements, hydroxyomeprazole with the corresponding carboxylic acid was identified. The remainder of the portion is excreted in the faeces.
Clarithromycin.
After oral administration, clarithromycin is involved in metabolic processes with the formation of the main metabolic element - 14-hydroxyclarithromycin. It has an anti-microbial effect similar to the active element in severity (or 1-2 times weaker; depending on the type of bacteria).
The half-life is about 3-4 hours. Approximately 20% of the substance is excreted unchanged through the kidneys, and another 15% is in the form of 14-hydroxyclarithromycin.
Tinidazole.
When taken orally, the absorption of tinidazole is complete and at high speed. Protein synthesis is rather weak; the indicator of the active element is 88%.
Tinidazole passes without complications into the inflamed areas and various tissues. Comparison of the plasma values of substances inside the cerebrospinal fluid made it possible to determine that tinidazole penetrates into it more actively than metronidazole (the indicators are equal, respectively, 88% and 43%). Tinidazole in large volumes is determined inside breast milk.
Over a period of 24 hours, 20-25% of tinidazole is excreted in the urine.
Dosing and administration
The 1st combination of the drug contains 2 capsules of omeprazole, and in addition, 2 tablets of tinidazole with clarithromycin. One specified kit is assigned for a 1-day intake.
It is required to use 1 capsule of omeprazole, as well as 1 tablet of tinidazole with clarithromycin in the morning, and then in the same amount in the evening. In addition, an additional intake of 0.25 g of clarithromycin in the morning and in the evening must be prescribed.
The duration of the treatment cycle is 7 days.

Peptika kombipek is not prescribed to persons under the age of 14.
Use Peptika kombipek during pregnancy
It is forbidden to use the medication in pregnant women or with hepatitis B.
Contraindications
Use is contraindicated in case of diagnosed intolerance to clarithromycin with tinidazole or omeprazole, and in addition in the case of pathological blood changes. Also not prescribed in combination with ergot alkaloids.

Side effects include nausea, metal taste, headaches and vomiting.
Overdose
Poisoning if taken together with Mg and Al substances can provoke thirst, symptoms of neurotoxicity, hypotension and redness of the skin on the face.
Requires gastric lavage and supportive action.
Interactions with other drugs
The introduction of clarithromycin with theophylline may increase the plasma values of the latter.
The use of clarithromycin with terfenadine increases the plasma level of the latter, which can lead to prolongation of the QT-interval and the development of a heart rhythm disorder.
The combination of clarithromycin with oral anticoagulants (including warfarin) leads to an increase in the therapeutic effect of the latter.
The combination of clarithromycin with cyclosporine, cisapride, phenytoin and carbazepine, as well as with lovastatin, digoxin, disopyramide, astemizole and valproate with pimozide, can provoke an increase in the plasma level of these substances.
Due to the intense suppression of the processes of hydrochloric acid release, the omeprazole element is able to affect the absorption of ampicillin with ketoconazole and Fe salts.
Omeprazole reduces the rate of elimination of diazepam with phenytoin and warfarin.
The medication cannot be used together with substances of ergot alkaloids.
Storage conditions
Peptika kombipek must be stored in a dry place, closed from the penetration of children. The temperature level is within 15-25 ° С.
Shelf life
Combipec peptika can be used for a period of 36 months from the date of manufacture of the pharmaceutical element.
