Captopress is a combination antihypertensive medication.

Indications Captopres

It is used for various types of hypertension with arterial character.

Release form

Pharmacodynamics

The drug contains 2 active elements - hydrochlorothiazide with captopril. It has powerful diuretic and antihypertensive properties. Its drug activity and principle of action are based on the properties of its constituents.

Substance captopril is included in the category of ACE inhibitors. The component helps to reduce blood pressure values, weakens the systemic resistance exerted by peripheral vessels, and the preload relative to the myocardium, and at the same time increases the cardiac volume and stability regarding physical activities. It acts by slowing down the activity of the ACE element, as a result of which the process of transformation of angiotensin-1 into angiotensin-2 is suppressed, which has a strong vasoconstrictor effect.

Hydrochlorothiazide is a diuretic substance of thiazide nature. It potentiates the excretion of potassium, sodium and chloride ions, as well as liquids. Enhances the hypotensive activity of captopril.

Pharmacokinetics

After ingestion, the active elements of the drug are well absorbed inside the gastrointestinal tract. Plasma Cmax values of captopril are recorded after the expiration of the 1st hour from the moment of oral administration, and hydrochlorothiazide - after 1-2.5 hours. Approximately 30% of captopril undergoes intraplasma protein synthesis.

Excretion of active substances to a greater extent flows with urine (unchanged elements and metabolic products). The half-life of captopril is 45–120 minutes, and that of hydrochlorothiazide, is 5.5–15 hours.

Hydrochlorothiazide is able to overcome the hemato-placental barrier and may be excreted in mother's milk.

Dosing and administration

The medicine is taken orally, swallowing the pill whole, (no need to grind or chew). It is necessary to wash down the drug with plain water. If necessary, it is allowed to divide the tablet in half. Drug use is not tied to food intake. The duration of the therapeutic cycle is selected by the attending doctor, individually for each patient.

Adults with healthy renal activity should first use 25 mg of the drug 2 times a day. Later, when needed, the portion is increased to 50 mg of the substance 2 times a day.

Per day, a maximum of 0.1 g of captopril and 50 mg of hydrochlorothiazide is allowed.

Contraindications

  • liver and gastrointestinal lesions: vomiting, upset stool, loss of appetite, epigastric pain, nausea, dry oral mucous membranes and stomatitis. In addition, jaundice, peptic ulcer, hyperbilirubinemia and hepatitis;
  • disorders of the hematopoietic processes and the work of the cardiovascular system: angina pectoris, Raynaud's disease, decrease in blood pressure, tachycardia, pallor of the epidermis or hyperemia, thrombocyto, neutro or pancytopenia, and in addition, cardiogenic shock, anemia of aplastic or hemolytic nature and agranulocytosis;

  • problems with the work of the respiratory system: bronchial spasms, cough, respiratory distress and runny nose allergic nature;
  • urinary disorders: polyuria or nephrotic syndrome, severe renal impairment and oliguria;
  • signs of allergy: urticaria, erythema, angioedema, rash, photosensitivity, itching and Stevens-Johnson syndrome;
  • changes in test results: an increase in urea nitrogen with bilirubin, and in addition, creatinine with calcium inside the blood plasma, a decrease in blood glucose and sodium, decreased hematocrit with platelets, and also leukocytes with hemoglobin, as well as the development of proteinuria;

Overdose

Using too large portions of Captopres leads to tachycardia, nausea, headaches, stool disorders, neutropenia, vomiting and anorexia, and in addition to a decrease in blood pressure values. The subsequent increase in dosage causes the appearance of signs of hydrochlorothiazide intoxication, a disorder of EBV balance and a state of coma (as a result of the effects of the drug on the central nervous system).

Interactions with other drugs

The combination with MAOIs, vasodilators, diazoxide, ganglioblokatorami, diuretic drugs and adrenergic blockers causes an increase in the antihypertensive activity of the drug.

Diuretic, with potassium-sparing ability, and potassium drugs when used together with Captopress increase the values of potassium inside the plasma and increase the likelihood of hyperkalemia.

There is a decrease in the effectiveness of the drug in the case of its combination with methenamine or NSAIDs.

The combination of a substance with lithium means leads to an increase in the plasma values of the latter.

Simultaneous use with barbiturates, ethanol and narcotic drugs increases the likelihood of orthostatic collapse.

The combination with corticosteroids, amphotericin B, and in addition with adrenocorticotropic hormone can lead to the appearance of electrolyte metabolism disorders.

Captopress can potentiate plasma calcium levels.

The drug potentiates the toxic activity of SG when combined with them.

Use with medication may require correction of a portion of hypoglycemic substances, anti-gouty drugs, sulfinpyrazone, probenecid, as well as oral anti-coagulants.

Colestipol with colestiramine weaken the absorption of hydrochlorothiazide.
