Pharmacokinetics

Pharmacokinetics Zalasta q-tab - tablets that quickly disperse in the oral cavity. The main component of the drug is absorbed immediately after oral administration. Its maximum concentration in plasma is achieved within 5-8 hours. Food intake does not affect absorption in any way.

Absolute oral bioavailability relative to bioavailability after administration has not been determined. The drug is metabolized in the liver via conjugative and oxidative pathways. The main circulating metabolite is 1 0-N-glucuronide. It does not cross the blood-brain barrier. Cytochromes P450-CYP1A2 and P450-CYP2D6 are involved in the formation of N-desmethyl and 2-hydroxymethyl metabolites.

As for clearance, it is significantly lower in elderly people compared to young people. A similar situation is observed in women compared to men, and in those who smoke compared to those who have given up this harmful habit.

The active component binds to plasma proteins by approximately 93% over the entire concentration range from approximately 7 to approximately 1000 ng/ml. In general, the main binding occurs with albumin and α1-acid glycoprotein. This is the pharmacokinetics of the drug Zalasta q-tab.

Dosing and administration

The method of application and dosage are discussed individually with the attending physician. The tablets are able to disperse quickly in the oral cavity. Therefore, they are fragile in their physical properties. This indicates that the product must be carefully removed from the blister and immediately placed in the oral cavity. Do not use water, the tablet quickly disintegrates.

The recommended starting dose for people with schizophrenia is 10 mg per day. For manic syndrome, it is 15 mg in monotherapy and 10 mg in combination therapy. Interactions with other drugs

Interactions of Zalasta q-tab with other drugs are possible, but with special caution. Thus, the metabolism of the drug can be affected by inhibitors or inducers of cytochrome P450 isoforms, especially the activity of CYP1A2.

Smoking and carbamazepine intake may decrease olanzapine concentrations and thus lead to complete ineffectiveness. In some cases, a slight to moderate increase in olanzapine clearance has been noted.

The drug is not able to change the pharmacokinetics of theophylline. Fluoxamine, in turn, can reduce the ability of olanzapine to metabolize. This can lead to an average increase in the maximum concentration in the plasma of fluoxamine by 54%, and in male smokers by 77%. Patients taking fluoxamine should use olanzapine in a reduced dose.

Activated carbon reduces the oral bioavailability of olanzapine by 50-60%. Therefore, it should be taken 2 hours before or after olanzapine.

CYP2D6 inhibitors. Fluoxetine causes a mean increase in olanzapine Cmax by 16% and a mean decrease in olanzapine clearance by 16%. No interactions with lithium or biperiden have been observed.

Antihypertensive drugs. The drug can significantly enhance the effects of certain medications. Therefore, it should be taken with extreme caution. All this indicates that Zalasta q-tab should be used only with the permission of a doctor.
