We present to your attention the instructions for use of the drug Yarina Plus – a representative of monophasic oral contraceptives.

The manufacturer of the drug is the German company Bayer Pharma AG.

Indications Yarina plus

  • Contraception for women who have signs of hormonal-dependent tissue swelling.
  • Prevention of unwanted pregnancy and treatment of moderate acne.
  • Prevention of unwanted conception in women with a deficiency of folic acid derivatives and B vitamins.

Release form

The drug Yarina Plus is available in tablet dosage form in two types of tablets:

  • active complex tablets: round, convex on both sides, with a thin orange-colored film on the surface. On one of the surfaces there is a designation "Y+" enclosed in a hexagon;
  • additional multivitamin preparations: round in shape, convex on both sides, with a thin film coating of a light orange hue. On one of the surfaces there is a designation "M+" enclosed in a hexagon.

The composition of the active complex tablet is presented:

  • drospirenone (a derivative of spironolactone) in the amount of 3 mg;
  • ethinyl estradiol (hormonal estrogen agent) 0.03 mg;
  • calcium levomefolate (folate analogue) 0.451 mg;
  • additional components: lactose, microcrystalline cellulose, sodium croscarmellose, magnesium stearate, hyprolose;
  • components of the outer film: varnish coating, or hypromellose, macrogol, talc, dyes (red and yellow iron oxide), titanium dioxide.

The composition of the additional vitamin preparation is presented:

  • sodium levomefolate, lactose, microcrystalline cellulose, magnesium stearate, sodium croscarmellose, hyprolose;
  • components of the outer film: varnish coating, macrogol, talc, dyes (red and yellow iron oxide), titanium dioxide.

The blister pack contains 21 active tablets and 7 additional ones, sealed in aluminum foil. The kit includes a set of stickers for easy maintenance of the tablet intake calendar.

Pharmacodynamics

The contraceptive Yarina Plus is a low-concentration, single-phase complex hormonal drug for internal use. It includes active ingredients and additional tablets containing calcium levomefolate.

The contraceptive ability of Yarina Plus is explained by the suppression of the ovulation process and the increase in the density of cervical mucus. In patients who use complex contraceptive drugs, the monthly cycle is stabilized, the course of menstruation is facilitated, the risk of anemia is eliminated. Experts say that taking such contraceptives can serve as a preventive measure for malignant diseases of the endometrium and appendages.

One of the main components of the drug, drospirenone, exhibits an antimineralocorticoid effect and prevents hormone-dependent tissue swelling. This serves as a preventative measure against weight gain and fluid retention in the body. Another ability of dropirenone is to eliminate acne and reduce the oiliness of the hair and skin. This is explained by the similar action of this substance with the natural property of progesterone.

If the drug is used and taken correctly, the percentage of pregnancy in patients may be less than 1%. If the pills are taken randomly or missed, the probability of pregnancy while taking Yarina Plus increases significantly.

Another component of the drug, levomefolate, can be thought of as a bioactive form of folate. This form allows the substance to be absorbed more fully than folic acid. Levomefolate is usually prescribed to provide the body of a pregnant or lactating woman with the required amount of folates. The presence of this substance in the contraceptive drug is explained by the fact that levomefolate can reduce the risk of developing neural tube pathology in the embryo if the patient wants to become pregnant immediately after the end of the contraceptive course.

Pharmacokinetics

When taken orally, the active component of the drug is quickly and almost completely absorbed into the blood. After a single internal use, the maximum level of drospirenone and ethinyl estradiol is observed after 60-120 minutes and reaches 37 ng / ml and 54-100 pg / ml, respectively. The biological availability of drospirenone is 76-85%, ethinyl estradiol - 20-65%. Simultaneous use of food does not affect the bioavailability of the drug, only in some cases it can decrease to 25%.

The active substance binds to serum protein. Only up to 5% of the total level appears in plasma as a free hormonal substance, while about 95% non-specifically binds to protein. Increased levels of sex hormones do not affect the binding of drospirenone to plasma proteins.

After internal use, drospirenone leaves the body via the digestive system and kidneys. Sex hormone levels do not affect the pharmacokinetic properties of drospirenone.

If the drug is taken according to the scheme, the level of drospirenone in the serum will increase two or even three times daily. A stable level is observed in the second phase of the treatment cycle.

Dosing and administration

How and according to what scheme should Yarina Plus be used?

The drug is used orally, following the instructions attached to the contraceptive. The regimen involves taking the drug at the same time, swallowing whole, with clean water. The tablets are used in the amount of one piece per day, without missing a dose, for 28 days. After the package is exhausted, start using the next one.

Start taking the contraceptive from the first day of menstruation.

In case of dyspepsia (vomiting and diarrhea), the absorption of the drug may be impaired, so in this situation additional contraceptive measures should be taken.

The drug is used in women of childbearing age, which does not include children and patients who have reached menopause.

The drug is not used in case of liver and kidney dysfunction.

Use Yarina plus during pregnancy

Yarina Plus is not used during pregnancy. In cases where pregnancy is detected during the course of taking the contraceptive, the use of the drug must be stopped immediately.

There is practically no information about how and to what extent the drug affects the fetus and the course of pregnancy. However, during observations, specialists did not find any developmental defects in children born to women who took Yarina Plus before pregnancy or in the first weeks of pregnancy. However, it should be taken into account that special studies on this issue have not been conducted.

Yarina Plus is not taken during breastfeeding. The use of the drug can reduce the volume of milk in a nursing woman, as well as affect its composition. In addition, sex hormones in small quantities can get into milk, although the degree of their effect on the baby has not been determined.

Contraindications

  • Tendency to allergic reactions to any component of the contraceptive Yarina Plus.
  • Tendency to form blood clots, thromboembolism, vascular wall integrity disorders, cerebrovasculitis.
  • Ischemia, angina pectoris and other prethrombotic conditions.
  • Migraine conditions with neurological symptoms.
  • Vascular complications of diabetes mellitus.
  • Hepatitis, signs of liver failure.
  • Insufficient kidney function.
  • Various neoplasms in the liver area.
  • Hormonal-dependent tumor processes, or tumors of unknown genesis.
  • Bleeding from the genitals of unknown etiology.
  • Pregnancy and breastfeeding period.
  • Intolerance to some components of the drug, for example, lactose.

Side effects Yarina plus

Side effects of Yarina Plus can occur with varying frequency and intensity, sometimes not appearing at all. We will tell you about the most common of them.

  • Allergic reactions to the drug.
  • Weight fluctuations in one direction or another, often due to swelling.
  • Depressed state, impaired sexual activity, mood swings.
  • Migraine-like headaches.
  • Contact lens aversion.
  • Development of hearing loss.
  • Vascular circulatory disorders, fluctuations in blood pressure, risk of blood clots.
  • Respiratory diseases, asthmatic conditions.
  • Dyspeptic symptoms.
  • Skin lesions, eczematous rashes, urticaria.
  • Soreness of the mammary glands, thrush, menstrual irregularities, inflammation in the vagina, discharge from the breast, swelling of the glands.

Among the more complex side effects are the following:

  • thromboembolism, both in veins and arteries;
  • disorders of the cerebral vascular system;
  • hypertension;
  • increased levels of potassium and triglycerides in the blood;
  • impaired glucose sensitivity and insulin resistance;
  • neoplasms in the liver area, dysfunction of its function;
  • the appearance of skin hyperpigmentation;
  • angioedema;
  • calculous cholecystitis, systemic lupus erythematosus, herpetic eruptions, deafness (otosclerosis), ulcerative lesion of the intestinal mucosa, cervical cancer.

Overdose

There have been no cases of overdose of the contraceptive Yarina Plus.

Presumably, an overdose may manifest itself in the form of dyspeptic disorders, bleeding from the genital tract, and metrorrhagia.

No special drug has been developed to counteract the effect of the contraceptive.

As a rule, Yarina Plus is well tolerated by patients.

Interactions with other drugs

The combined use of drugs that accelerate the induction of microsomal liver enzymes leads to an increase in the clearance of some hormones. Among such drugs are sleeping pills, carbamazepine, griseofulvin, St. John's wort, and rifampicin. When using the listed drugs in combination, it is necessary to use additional barrier methods of contraception.

Many antibacterial drugs (mainly penicillin and tetracycline series) have the ability to reduce the hepatic-intestinal return circulation of estrogens, which leads to a decrease in the level of ethinyl estradiol. In this regard, it is recommended to use additional external methods of contraception during antibiotic treatment and for a week after the end of the course of treatment.

Antiepileptic drugs, methotrexate, triamterene, cholestyramine can reduce the level of folates in the blood and inhibit the action of one of the active substances of Yarin Plus - levomefolate.

Yarina Plus may affect the metabolism of other drugs, such as antiepileptic drugs, pyremethamine, etc.

Storage conditions

Yarina Plus can be stored at a moderate temperature, no more than 25°C, and always in places where children cannot reach.

Shelf life
