Veroshpiron is a competitive aldosterone antagonist, as well as a diuretic substance of potassium-sparing character.
The active ingredient of the drug is spironalactone. Inside the distal nephron regions, it prevents aldosterone from retaining Na and water, and at the same time it weakens the potassium-withdrawing activity of aldosterone and reduces the binding of permeases. The decrease in blood pressure occurs during the development of diuretic effects; its manifestation begins after 2-5 weeks of drug administration.
Indications Veroshpiron
It is used in the complex treatment of primary hypertension. In addition, it is used for the appearance of puffiness in the case of CHF, and also for ascites, nephrotic syndrome and hepatic cirrhosis.
It is also prescribed as an auxiliary component in the prevention of the development of hypokalemia or magnesium-magnesium during therapy with diuretic drugs.
The drug is also used for aldosteroma during a short preoperative treatment cycle or for the diagnosis of Conn syndrome.

Pharmacodynamics
The Cmax values of the drug are determined after 7 hours from the moment of use of the capsule, and the therapeutic effect lasts 24 hours. The active element is converted into active metabolic components. Excretion is realized through the kidneys.
Dosing and administration
In case of primary hypertension, it is required to consume 1-time from 0.05 to 0.1 g of the substance. Gradually, you can increase the portion to 0.2 g (such adjustments are carried out 1-fold for a 2-week period).
In the case of idiopathic form of hyperaldosteronism: 0.1-0.4 g per day.
In case of hypokalemia or hyperaldosteronism of a pronounced nature: it is necessary to take in 2-3 doses per day 0.3 g of drug. Gradually reduce the portion to 25 mg per day.
With hypokalemia or magnesium, which are caused by the influence of diuretic drugs: 0.025-0.1 g per day (1-time or in several uses).
Therapy or diagnosis of Conn syndrome: 0.4 g per day in a 4-day period. The daily portion needs to be distributed on several uses.
Puffiness that appears in the case of nephrotic syndrome: apply 0.1-0.2 g of drug per day. It is necessary to use Veroshpiron only in situations when other methods of therapy have not brought the desired result.
Edematous syndromes arising in the case of CHF: 0.1-0.2 g of the medication per day (in several uses), over a 5-day period, in combination with a diuretic loop or thiazide character. Daily dosage can be reduced to 25 mg.
Puffiness developing as a result of cirrhosis: in the case of the Na + / K + ratio within the urine exceeding 1.0, it is necessary to use 0.1 g of drug. If this figure is below 1.0, the daily dosage is 0.2-0.4 g.
Puffiness that occurs in children: first, use 1-3.3 mg / kg or 30-90 mg / m 2 per day, in 1-4 intake. After a 5-day period, dosage adjustment is carried out.
Often, the treatment cycle lasts 20 days, and then you should take a break for at least 6 months. The maximum allowable duration of use is 2 months. Prolonged reception of Veroshpiron can cause the development of arresters.

Use Veroshpiron during pregnancy
Veroshpiron is not prescribed to pregnant or lactating women.
Contraindications
The main contraindications:
- severe intolerance to the elements of the drug;
- hyponatremia or hyperkalemia;
- hypocorticoidism;
- anuria;
- renal failure, with severe severity (CC level below 10 ml per minute).
Extremely careful medication is used in such situations:
- metabolic acidosis;
- hypercalcemia;
- diabetes mellitus or nephropathy of a diabetic nature;
- AV blockade;
- menstrual cycle disorders;
- local or general anesthesia procedures;
- kidney or liver diseases;
- elderly age.
Side effects Veroshpiron
Among the side effects are:
- lesions associated with the gastrointestinal tract: vomiting, constipation, nausea or diarrhea, and in addition colic in the intestine, gastritis and problems with the work of the liver;

Overdose
Signs of poisoning include dizziness, vomiting, diarrhea, nausea, and epidermal rash.

Interactions with other drugs
Veroshpiron weakens the effect of anticoagulants and mitotane, and also enhances the activity of diuretic and antihypertensive drugs.
Potentiates the effects of triptorelin with buserelin, as well as gonadorelin.
The combination with potassium substances increases the likelihood of hyperkalemia.
The drug reduces the toxicity of SG.
Salicylates with indomethacin reduces the diuretic activity of the drug.
The drug enhances the metabolism of antipyrine, and also prolongs the term half-life of digoxin.
