Veroshpiron is a competitive aldosterone antagonist and a potassium-sparing diuretic.

The active component of the drug is spironolactone. In the distal nephron sections, it prevents the retention of Na and water by aldosterone, and at the same time weakens the potassium-excreting activity of aldosterone and reduces the binding of permeases. A decrease in blood pressure occurs with the development of a diuretic effect; its manifestation begins after 2-5 weeks of drug use.

Indications Verospirona

It is used in the complex treatment of primary hypertension. In addition, it is used when swelling occurs in the case of CHF, as well as in ascites, nephrotic syndrome and liver cirrhosis.

It is also prescribed as an auxiliary component in the prevention of the development of hypokalemia or -magnesemia during therapy with diuretic drugs.

The drug is also used for aldosterone during a short preoperative treatment cycle or for diagnosing Conn's syndrome.

The substance is released in tablets and capsules.

Pharmacodynamics

The Cmax values of the drug are determined after 7 hours from the moment of capsule administration, and the therapeutic effect lasts for 24 hours. The active element is converted into metabolic components with activity. Excretion is realized through the kidneys.

Dosing and administration

In case of primary hypertension, it is necessary to use 0.05-0.1 g of the substance once. The portion can be gradually increased to 0.2 g (such adjustments are made once over a 2-week period).

In case of idiopathic hyperaldosteronism: 0.1-0.4 g per day.

In case of hypokalemia or hyperaldosteronism of a pronounced nature: it is necessary to take 0.3 g of the drug in 2-3 doses per day. Gradually, the dose is reduced to 25 mg per day.

For hypokalemia or -magnesemia caused by the influence of diuretic drugs: 0.025-0.1 g per day (1 time or in several doses).

Treatment or diagnosis of Conn's syndrome: 0.4 g per day for a 4-day period. The daily dose should be divided into several uses.

Edemas that occur in case of nephrotic syndrome: use 0.1-0.2 g of the drug per day. Veroshpiron should be used only in situations where other methods of therapy have not brought the desired result.

Edema syndromes arising in case of CHF: 0.1-0.2 g of the drug per day (in several doses), over a 5-day period, in combination with loop or thiazide diuretics. The daily dosage can be reduced to 25 mg.

Edemas resulting from cirrhosis: if the Na+/K+ ratio in urine exceeds 1.0, 0.1 g of the drug should be taken. If this ratio is below 1.0, the daily dosage is 0.2-0.4 g.

Edemas occurring in children: initially, 1-3.3 mg/kg or 30-90 mg/m2 per day, in 1-4 doses. After a 5-day interval, the dosage is adjusted.

Often the treatment cycle lasts 20 days, and then a break of at least 6 months should be taken. The maximum permissible duration of use is 2 months. Long-term use of Veroshpiron can cause the development of acute renal failure.

The medication should be taken after meals.

Use Verospirona during pregnancy

Veroshpiron is not prescribed to pregnant or breastfeeding women.

Contraindications

Main contraindications:

  • severe intolerance to the components of the drug;
  • hyponatremia or hyperkalemia;
  • hypocorticism;
  • anuria;
  • renal failure of severe severity (creatinine clearance level below 10 ml per minute).

The medication should be used with extreme caution in the following situations:

  • metabolic acidosis;
  • hypercalcemia;
  • diabetes mellitus or diabetic nephropathy;
  • AV block;
  • menstrual cycle disorders;
  • local or general anesthesia procedures;
  • diseases affecting the kidneys or liver;
  • old age.

Side effects Verospirona

Side effects include:

  • gastrointestinal tract lesions: vomiting, constipation, nausea or diarrhea, as well as intestinal colic, gastritis and liver problems;
  • CNS disorders: dizziness, muscle spasms, feeling of inhibition, headaches, lethargy and ataxia, as well as drowsiness, cramps affecting the calf muscles, and confusion;
  • disorders of hematopoietic function: agranulocytosis, thrombocytopenia or megaloblastosis;
  • problems with metabolic processes: hyperkalemia, -uricemia or -creatininemia, as well as hyponatremia, increased urea levels and acidosis or alkalosis of a metabolic nature of the hyperchloremic form;
  • disorders of the endocrine system: impotence or decreased potency, gynecomastia (in men). Dysmenorrhea, amenorrhea, as well as terrorrhea, hirsutism and breast carcinoma (in women);
  • signs of allergy: urticaria, itching, drug fever, as well as maculopapular or erythematous rashes;
  • dermatological disorders: alopecia or hypertrichosis;
  • urinary tract lesions: acute renal failure.

Overdose

Signs of poisoning: dizziness, vomiting, diarrhea, nausea and epidermal rash.

Gastric lavage is performed, and measures are taken to increase blood pressure values.

Interactions with other drugs

Veroshpiron weakens the effect of anticoagulants and mitotane, and also enhances the activity of diuretics and hypotensive substances.

Potentiates the effects of triptorelin with buserelin, as well as gonadorelin.

Combination with potassium-containing substances increases the likelihood of hyperkalemia.

The drug reduces the toxicity of SG.

Salicylates with indomethacin reduce the diuretic activity of the drug.

The drug enhances the metabolic processes of antipyrine and also prolongs the half-life of digoxin.
