Indications of the uursomax
The medicine of Ursomax can be used:
- if it is necessary to eliminate the biliary cholestherin etiology, which are undetermined by the x-ray method, the diametrical size of which does not exceed 1.5 cm (an obligatory condition is a functioning gallbladder, regardless of the presence of concrements);
- as a remedy for gastritis with a reverse reflux of bile (reflux);

- for the stabilization of liver function and biliary system in children suffering from cystic fibrosis (age - 6-18 years).

Release form
Drugs of Ursomaks are produced in the form of capsules with granular or powdered contents based on ursodeoxycholic acid.
Capsules dense (№0), white, sealed in blister plates of 10 pieces.
A cardboard package may contain one, five or ten blister plates (10, 50 or 100 capsules, respectively).
Pharmacodynamics
The active ingredient Ursomax is ursodeoxycholic acid - a substance that is present in a small amount in the composition of human bile.
After consuming Ursomax, the medication reduces the concentration of the cholesterol component in the bile fluid, limiting its absorption in the digestive system and blocking the ingress of cholesterol into the bile. In all likelihood, due to the breakdown of cholesterol and the formation of liquid crystalline forms, a slow decomposition of bile calculi occurs.
It is believed that the medical properties of ursomax in liver pathologies and bile stagnation are due to a partial substitution of lipophilic toxic bile acids with hydrophilic non-toxic acid, which serves as an active ingredient in Ursomax. This leads to an acceleration of the excretory activity of liver cells and the activation of immune regulation mechanisms.
Pharmacokinetics
After using the drug Ursomax, the active substance is well absorbed in the intestine by the way of active and passive transportation. High rates of assimilation are estimated at 60-80%.
After the absorption processes, there is an absolute conjugation of bile acid with a number of amino acids - in particular, with such as glycine and taurine. Excretion is carried out by means of bile.
The clearance for I passage can be 60%.
Depending on the daily amount of Uursomax and the degree of disorder of the hepatic function, the active ingredient accumulates in the bile secretions. Also, a relative decrease in the level of other, more lipophilic, acids is found.
Under the influence of microbial flora in the intestine there is an incomplete decomposition of the active ingredient. One of the degradation products is considered to be hepatotoxic and can lead to changes in the hepatic parenchyma, which has been confirmed by experiments on animals. People only digest a small amount of the toxic component, which is then completely inactivated in the liver.
The biological duration of the half-life of the active ingredient Ursomax can be 3.5-5.8 days.
Dosing and administration
Ursomax appoints a doctor if there are appropriate indications. Dosage and duration of therapy depend primarily on the weight of the patient and on the characteristics of the disease.
- To eliminate bile-cholesterol concrements, the dosage is determined from the calculation of 10 mg of Ursomax per kg of weight of the patient. The required number of capsules is swallowed whole, daily, at bedtime, regularly. Duration of admission can be from six months to a year. If after 12 months of therapy, no positive dynamics are detected, then Ursomax is stopped. The dynamics of treatment is important to monitor once every six months, using the technique of ultrasound and radiography. At the same time, the probability of calcification of concrements should be evaluated. If signs of calcification show up, the therapy is completed.
- When inflammation of the mucous tissues of the stomach with reflux bile is enough to take 1 capsule of Ursomax at night, for 10-14 days. The therapy regimen can be adjusted at the discretion of the treating doctor.
- In primary biliary cirrhosis, the daily amount of Ursomax should be 12-16 mg per kg of weight of the patient. During the first three months of therapy, Ursomax is taken three times a day. After improving the patient's condition, they switch to a standard procedure - once a day, at night.
Capsules are swallowed whole, with liquid. Reception is conducted daily at the same time.
With the primary form of biliary cirrhosis, at first, there may be a worsening of clinical symptoms - for example, itching. With such signs, therapy is continued, limiting the intake of Ursomax once a day. After normalization of the patient's condition, the number of capsules gradually increases (weekly add one capsule until the desired therapeutic dosage is reached).

Use of the uursomax during pregnancy
Informative data on the possible use of Ursomax for the treatment of pregnant patients are not sufficient. When investigating the effects of Ursomax on rodents, a reproductive toxicity of the drug was detected in the initial stages of pregnancy. With this in mind, specialists can not recommend Ursomax as a drug for receiving pregnant women. Moreover, before the start of treatment, all women of childbearing age should be excluded from pregnancy, and also resort to the use of reliable contraception (it is preferable to use non-hormonal oral medications or products with a minimum estrogen content).
The intake of Ursomax during the period of breastfeeding is considered acceptable, since the medicine is found in insignificant quantities in breast milk and can not have any negative effect on the baby's body.
Contraindications

- allergic mood of the organism to individual ingredients of Ursomax;
- acute inflammatory process in the system of biliary excretion;
- absence of patency in the bile ducts or bladder;
- frequent hepatic colic;

- disorders of contractility of the gallbladder;
- the absence of a positive result after portoenterostomy, an incorrect outflow of bile in children of children with atresia of bile ducts.
Side effects of the uursomax
Usually, Ursomax is well tolerated by patients of all ages. Only in some cases it is possible to develop unwanted side-effects, such as:
- diarrhea or semisolid paste-like stools;
- severe pain in the projection of the liver;
- calcification of concrements of bile (very rarely);
- transition of biliary cirrhosis to a decompensated state, with partial regression after stopping the intake of Ursomax;
- allergic manifestations in the form of skin rashes.
Overdose
In case of an overdose, diarrhea usually occurs: other manifestations are considered unlikely, because an excessive increase in dosage leads to an impairment of Ursomax absorption. As a result, the surplus of the drug is simply removed from the calves.
If a patient develops diarrhea, then the dosage is reduced, and with the appearance of persistent diarrhea, treatment is stopped completely.
Special measures and drugs with an overdose of Uromax are not carried out. It is enough to observe the patient's condition and control the stability of water-electrolyte balance in the body.

Interactions with other drugs
It is contraindicated to take Ursomax in combination with anti-acid medicines, with drugs Kolestyramin, Kolestipol, Smecta, Fosfalugel or Almagel, since such medications worsen the absorption of ursodeoxycholic acid in the intestinal cavity. If the combination of taking drugs can not be avoided, then between their use should withstand a time interval of 120 minutes.
Ursomax can affect the absorption of Ciclosporin in the intestinal cavity. With this combination, you should monitor the level of Cyclosporine in the bloodstream and, if necessary, change the dosage.
In some patients, taking Ursomax may impair the absorption of ciprofloxacin.
The combination of Ursomax + rosuvastatin may lead to an increase in the concentration of rosuvastatin in the blood serum.
It is proved that Ursomax lowers the maximum possible serum concentration of Nitrendipine in patients with adequate liver and kidney function.
It is necessary to monitor the condition of patients with simultaneous treatment of Ursomax and Nifedipine. In some cases, you may need to increase the dosage of Nifedipine.
There is evidence of a weakening of the therapeutic effect of Dapsone in combination with Ursomax.
Medications based on estrogen, as well as means for normalizing the level of cholesterol in the circulatory system can enhance the release of cholesterol in the liver and increase the risk of formation of concrements in the system of bile formation.

It is recommended to keep Ursomaks in a packed form, away from children, in the temperature range from +18 to + 25 ° C.
