A visit to the dentist is a real test for many people. Fear of unpleasant sensations drives a person, delaying the moment of treatment to the extreme point. Today, there are many drugs that can relieve pain and make a visit to the dentist no less a pleasant procedure. Ultracaine, a drug intended for oral anesthesia, is especially popular. It is widely used for dental interventions of varying complexity.

Indications Ultracaine

You can relieve pain and completely eliminate it with this remedy. The main indication for the use of Ultracaine is tooth extraction, it can be either single or multiple. From now on, no discomfort or pain, a competent drug will help you cope with all the hardships of visiting a dentist.

The medicine is often used when preparing a tooth for placing a crown. This type of work requires grinding down the tooth enamel. The crown covers the tooth like a frame, protecting it from destruction and excess pressure. Grinding is an unpleasant procedure, requiring the use of special preparations, in particular Ultracaine.

The product is used to prepare the oral cavity for various treatment procedures. It is widely used to treat teeth in people suffering from serious illnesses. The product is well tolerated and has proven its effectiveness over many years.

Release form

The drug is available in ampoules. Usually, they are injected into the area where dental intervention will be performed. The main form of release of the drug is ampoules and cartridges. The ampoule is small, its volume does not exceed 2 ml, one package can contain about 100 pieces. Cartridges have a smaller volume, equal to 1.7 ml. One package, like ampoules, contains 100 pieces.

The Ultracaine solution is transparent. It does not have any foreign impurities or odor. If this is present, the product is most likely spoiled or stored in improper conditions. One ampoule of the medication contains active and auxiliary components.

Thus, 1 ml of the substance includes: articaine hydrochloride 40 mg/ml and epinephrine hydrochloride 6 mcg/ml. The auxiliary components are: sodium bisulfite, sodium chloride, water for injection. This composition is typical for ampoules, in cartridges it is similar. All of the above list falls on 1 ml of the medicine. Only in the ampoule the quantitative content of components is slightly higher than in the cartridge.

Pharmacodynamics

Ultracaine is a local anesthetic. It is widely used as an anesthetic during dental procedures. Ultracaine is a combination drug. It is capable of producing a vasoconstrictor effect and at the same time acting as a local anesthetic. Its main purpose is regional or infiltration anesthesia. This is the pharmacodynamics of Ultracaine.

Atrocaine hydrochloride is an amide anesthetic with a local effect. It is part of the tiaprofen group. Epinephrine hydrochloride has a vasoconstrictor effect and prevents atrocaine from entering the blood. This reduces the likelihood of adverse reactions.

The effect of Ultracaine is noted 1-3 minutes after administration. Ultracaine is characterized by its safety and reliability. After use, the pain-relieving effect lasts for 45 minutes. Due to its unique properties, the product is not capable of causing wound regeneration.

Epinephrine, contained in a small amount, is intended to prevent a possible increase in blood pressure and tachycardia. Ultracaine can be safely used by people at risk for changes in blood pressure.

Pharmacokinetics

After the drug is administered, its maximum amount in the blood plasma is recorded after 15 minutes. The average value is 400 mcg/l. The drug's effectiveness lasts for 45 minutes, the average value of the drug in this case is 2000 mcg/l. In children, the pharmacokinetic data are similar. Differences are observed only in the plasma concentrations of articaine and articaic acid. Articaine enters the systemic bloodstream as an inactive metabolite.

After administration of the drug, its concentration in the dental alveoli is significantly higher than in the systemic bloodstream. Binding of the active component to plasma proteins is 95%.

All agents with a local analgesic effect are metabolized in the liver. The main component is inactivated by plasma esterases. This occurs due to hydrolysis in the carboxyl group. Due to the fact that hydrolysis occurs relatively quickly, almost 90% of the main component is inactivated in this way. As a result of this process, the main metabolite is formed - articaic acid. It does not have a local analgesic effect, but is not toxic either.

The half-life of the drug is 25 minutes. The drug is excreted by the kidneys in the form of articainic acid in the amount of 64.2%, articainic acid glucuronide - 13.4% and unchanged - 1.45%. The total clearance is 235 l/h.

Dosing and administration

It is prohibited to administer the product inside a vessel. To avoid possible intravascular administration, it is worth performing a test aspiration before using the injection. The medication should not be used in inflamed areas. A special syringe is used to administer the injection. The product should be administered in such a way that the pressure corresponds to the sensitivity of the tissues at the injection site. The method of application and the administered dose of Ultracaine depend on the further actions of the dentist.

Preparing a tooth for installing a crown and subsequent treatment. In this case, 0.5-0.7 ml of the medicine is enough. The exact dosage is selected depending on the duration of the dental intervention. The maximum amount of the drug should not exceed 7 mg per kilogram of body weight.

Extraction of teeth. Vestibular administration of the drug in a volume of 1.7 ml is sufficient. This amount of the drug is applied to each tooth, not in general. For maximum pain relief, additional administration in a volume of 1-1.7 ml may be required. If the drug is ineffective, a blockade of the lower jaw is used.

Use Ultracaine during pregnancy

The use of the drug during pregnancy is completely permitted. The drug is not capable of causing stress and negatively affecting the developing fetus. The use of Ultracaine during pregnancy is effective and safe. The drug perfectly relieves pain and allows for both operations and dental interventions.

Artricaine is able to penetrate the hematoplacental barrier in small quantities. This feature is the main advantage of the drug. It can be used by pregnant women without fear for the life of their child.

The component of atricaine hydrochloride quickly disintegrates in the body. In addition, it is quickly excreted, which simply does not allow it to penetrate into breast milk. This is facilitated by rapid elimination. When using the drug during breastfeeding, you should consult with your doctor. It is possible that some amount of the drug may penetrate into the baby's body with breast milk.

Contraindications

Patients suffering from paragroup allergic reactions are strongly advised to use only disposable ampoules or cartridges. The fact is that the composition of the drug, packaged in multiple-use vials, includes the preservative methylparaben. It can lead to a persistent allergic reaction. This is the main contraindication to the use of the drug, but, alas, not the last. Patients with allergies should not interact with the components of atricaine and epinephrine. Therefore, you will have to refuse to use the drug.

Due to the epinephrine content in Ultracaine, the drug should not be used by people suffering from tachycardia and glaucoma. The risk group includes patients with bronchial asthma, cardiovascular insufficiency, and severe arterial hypertension. People using beta-adrenergic receptor blockers should not use Ultracaine.

Contraindicated for use in cases of cardiac arrhythmia, including atrioventricular block grades 2-3. Persons with severe conduction disorders of the heart valve and terminal anesthesia should use Ultracaine with caution to avoid possible negative consequences.

Side effects Ultracaine

Negative reactions from the body are associated with the content of epinephrine. It is responsible for disorders of many organs and systems of the body. The main side effects from taking Ultracaine include: headache, increased blood pressure, strong heartbeat. This can occur even with a slight introduction of the drug into the mucous membrane.

The central nervous system reacts in a peculiar way. Fainting, stupor and even respiratory arrest are possible. Involuntary muscle contractions, even convulsions, are often observed.

Visual organs: blindness and blurred vision. From the digestive system: nausea and vomiting. Cardiovascular system: decreased blood pressure, tachycardia, shock. Immune system: possible occurrence of edema, erythema, anaphylactic shock.

The reaction may be enhanced in people suffering from bronchial asthma. The main symptoms of this phenomenon are: vomiting, diarrhea, shock, and loss of consciousness.

Overdose

After the drug is administered, a person may experience unpleasant symptoms. These include: motor agitation, stupor, and dizziness. It is enough to lay the patient in a horizontal position, and the symptoms will begin to subside. In this case, it is necessary to ensure normal airway patency. If the symptoms are not severe, an infusion of crystalloids should be performed. In case of persistent respiratory failure, it is necessary to resort to artificial respiration. These signs of overdose can occur even in a completely healthy person.

If involuntary convulsions occur after using the drug, ultrashort barbiturates should be administered. At the same time, monitoring of the person's cardiac functions continues. If a decrease in blood pressure is observed, it is necessary to provide the patient with a horizontal position with legs raised up. The person is also laid down in this way when shock develops. At the same time, it is necessary to monitor the functional work of the lungs and heart. Electrolyte preparations may be administered.

In the event of tachyarrhythmia or sinus tachycardia, beta-adrenergic receptor blockers should be administered. This is done taking into account the blood circulation parameters. In the case of arterial hypertension, the administration of vasodilators is indicated.

Interactions with other drugs

Medicines that have an analgesic and local analgesic effect cannot be used in combination with monoamine oxidase inhibitors and antidepressants belonging to the tricyclic series. Such interaction with other drugs can cause the development of negative reactions from the body.

The use of adrenaline together with noradrenaline can lead to potentiation of the vasoconstrictor effect. Even if the dosage of the first component is significantly lower, this process cannot be completely excluded. When adrenaline enters the body, it leads to inhibition of insulin secretion by the gland. This leads to a decrease in the effect of oral hypoglycemic agents.

It is prohibited to use Ultracaine together with beta-adrenergic receptor blockers. The interaction of all components included in these drugs can lead to the development of bleeding. If an accidental puncture of a vessel occurs, the bleeding can be serious. Narcotic drugs together with Ultracaine lead to sensitivity of the heart muscle. This can contribute to the development of arrhythmia.

Storage conditions

The injection solution is stored in a dental clinic or hospital. Drugs of this type are rarely found at home. In the hospital, all necessary storage conditions are created for them. How can you "keep" the drug at home if there is an urgent need for it?

First of all, the product needs to be provided with a warm, dry place, where there is no direct sunlight. The negative impact of these factors can lead to the deterioration of the medicine. It is also important to observe the temperature regime. The optimal temperature for storing solutions should not exceed 25 degrees. Some of them require placement in the refrigerator.

If the product is at home, you need to ensure safety for your own children. Keep Ultracaine away from them. The glass bottle that contains Ultracaine is easy to break. This can lead to a cut or serious injury to the baby. In extreme cases, do not exclude the possibility of the child using the drug, which will lead to serious consequences. Proper storage conditions are the key to a long service life for the drug and safety for others.

Special instructions

Ultracain d-s forte

This product has a similar effect. It is widely used in dental practice to treat problem teeth. The product is quite safe and can be used even during pregnancy. Ultracain d-s forte contains similar components in the same amount. These are articana hydrochloride (40 mg) and epinephrine hydrochloride (6 mg).

People suffering from bronchial asthma should use Ultracain d-s forte with caution. Patients with acute heart failure and constant pressure surges are at risk. If the dose is too high or if a person has allergic reactions, the body may react inadequately to the drug.

The optimal dosage directly depends on the scope of the upcoming work and the complexity of the situation. In any case, you should not exceed the maximum permissible amount.

Shelf life

The product can be stored for no more than 30 months. During this time, the drugs need to be created under optimal conditions. Thus, the shelf life of Ultracaine is 2.5 years, but it can be stored in an open form for no more than a month. Optimal conditions for the "life" of the drug are created in the hospital. If it is necessary to store the product at home, it is necessary to take care of their full observance.

Each medicine has an acceptable temperature range, at which it will retain all its useful properties. For Ultracaine it is 25 degrees. The product should be stored in a warm, dry place, without direct sunlight. Failure to comply with these small rules will significantly shorten the life of the medicine.

Throughout the entire expiration date, it is necessary to pay attention to the appearance of the drug. It should not change color, consistency or smell. Otherwise, you will have to get rid of it. You cannot use the product after the expiration date. How Ultracaine will behave in this case is unknown. Following small rules significantly increases the chance of the medicine for a long service life.
