Terapin is a drug from the NSAID category and has a pronounced analgesic, anti-inflammatory and antipyretic effect.

Indications Terapina

It is used to treat the following diseases:

  • rheumatism (without reference to the form or stage of pathology);
  • rheumatoid arthritis;
  • myocarditis of infectious-allergic genesis;
  • inflammation in the area of the serous membrane of the heart (pericarditis);
  • Sydenham's chorea;
  • feverish state;
  • pain syndrome (this includes pain caused by gum and dental diseases, migraines, and also after dental procedures);
  • neuralgia (this includes maxillofacial type);
  • having a variety of etiologies of myalgia;
  • thrombophlebitis;
  • arthralgia or lumbago;
  • Kawasaki syndrome;
  • atrial fibrillation;
  • heart valve defect;
  • prolapse of the 2-leaf valve;
  • post-infarction syndrome.

Along with this, it is used as an antiplatelet drug (analog of aspirin) and is prescribed for the treatment or prevention of angina pectoris, myocardial infarction, cerebral or myocardial ischemia (without pain), and in addition to this, during rehabilitation after coronary angioplasty, as well as heart valve replacement.

After surgical procedures, the medication reduces the likelihood of unexpected bleeding and other complications.

Pharmacodynamics

The therapeutic result is achieved by selectively slowing down the activity of the elements COX-1 and COX-2, which regulate the binding of Pg. The antipyretic effect develops with a decrease in Pg values, as a result of which the epidermal vessels expand and sweating increases.

Aspirin has a powerful analgesic effect. It provides a prolonged antiaggregatory effect by inhibiting the processes of thromboxane type A2 binding inside platelets. Antiaggregatory activity persists for 7 days after taking the drug.

Terapin prolongs the PT and reduces blood clotting, resulting in a reduced risk of breakthrough bleeding during anticoagulant therapy and surgical procedures.

Pharmacokinetics

Aspirin in a dose of 1-10 mg/kg per day is absorbed inside the gastrointestinal tract at a good level and high speed. Combined use with food leads to a slowdown in this process. During absorption, presystemic elimination of the drug is noted inside the intestinal wall and liver, where deacetylation occurs. Plasma values of Cmax are noted after 10-20 minutes. The synthesis rate with protein is determined by the level of drug concentration and is approximately 49-70%.

The metabolic processes of Terapin occur inside the liver, where salicylic acid is formed together with its glycine conjugate, and in addition 2,5-dihydroxybenzoic acid (also with its glycine conjugate).

Aspirin's metabolic products are excreted mainly through the kidneys. The half-life is approximately 20 minutes. The excretion rate is determined by the portion size. If it is increased, the half-life can reach 15-30 hours, especially in people with kidney problems.

Dosing and administration

The medication is prescribed for oral use. The portion size and duration of therapy are selected by the physician individually.

The daily dosage for an adult is 0.5-1 g of the drug (in case of urgent need, it can be increased to 3 g). The medicine is taken once or divided into 3 doses per day.

Teenagers (over 12 years of age) should take a maximum of 0.25 g of the medicine per day. It is used twice a day (in the morning and then before bedtime).

Children aged 7-11 years – 0.3 g per day. Children aged 4-6 years – 0.2 g per day. Children aged 2-3 years – 0.1 g per day.

Approximate mode of use for various pathologies.

In case of myocardial infarction, the medication is taken once in a dose of 325 mg.

For circulatory disorders of a dynamic nature (mainly in men), the initial daily dosage is 325 mg, and then increases to 1 g.

To prevent the development of acute thrombosis or occlusion of the aortic bypass graft, the substance should be administered intranasally - in a dose of 325 mg at 7-hour intervals, and then taken orally, 3 times a day, in a dosage of 325 mg.

Use Terapina during pregnancy

The drug should not be used during pregnancy.

Contraindications

Main contraindications:

  • "aspirin triad";
  • allergy symptoms associated with the action of the drug's components;
  • gastric ulcer or erosive-ulcerative gastritis;
  • thrombocytopenia;
  • bleeding inside the gastrointestinal tract;
  • hemorrhagic form of diathesis;
  • aortic aneurysm;
  • breastfeeding period;
  • vitamin K deficiency;
  • portal hypertension;
  • severe liver or kidney dysfunction.

Caution is required when using in the following conditions:

  • urate urolithiasis;
  • having a decompensated form of CHF;
  • gastric ulcer of decompensated nature;
  • kidney or liver disease;
  • duodenitis, gout or gastritis.

Side effects Terapina

The use of a pharmaceutical product may provoke the appearance of the following side effects:

  • dyspeptic disorders (loss of appetite, abdominal pain, bloating, diarrhea, vomiting, constipation and nausea);
  • allergy symptoms (urticaria, angioedema, dermatological itching, as well as exacerbation of eczema or dermatitis);
  • aspirin asthma;
  • problems with kidney function;
  • an increase in blood platelet counts (may lead to acute thrombocytopenia);
  • anemia;
  • an increase in the blood white blood cell count (sometimes severe leukopenia develops);
  • white liver disease;
  • CNS dysfunction (feeling of severe fatigue or anxiety, dizziness, sleep disorders, headaches and apathy);
  • hypercalcemia;
  • runny nose;
  • gastritis in the acute phase;
  • transient visual disturbances;
  • transient deterioration of hearing acuity;
  • nephrotic syndrome;
  • tinnitus;
  • acute bronchospasm;
  • fluid retention in the body.

Overdose

During therapy, it is necessary to carefully follow the recommended treatment regimen. The use of large doses can cause dizziness, tinnitus, nausea, paralysis, headaches, anxiety, and in addition vomiting, fever, pulmonary hyperventilation, and loss of consciousness.

If signs of poisoning develop, you should immediately contact a doctor to receive emergency assistance.

Symptomatic procedures are performed – gastric lavage, use of activated charcoal, monitoring of acid-base balance and electrolyte balance indicators, etc.

Interactions with other drugs

Aspirin potentiates the toxic properties of methotrexate and also reduces its renal clearance, which increases the likelihood of developing renal failure or exacerbation of nephritis.

The drug weakens the effect of opiates, thrombolytics, indirect anticoagulants, reserpine, substances that slow down platelet aggregation, antidiabetic and antihypertensive drugs, and also uricosuric drugs.

The combination with ethyl alcohol increases the likelihood of damage to the walls of the gastric mucosa and the risk of bleeding inside the gastrointestinal tract.

Aspirin increases the levels of lithium salts, digoxin and barbiturates in the blood plasma, which can cause poisoning.

When magnesium supplements and antacids are combined, the absorption of aspirin is slowed down.

The hematotoxic properties of Terapin are enhanced when used together with myelotoxic drugs.

Storage conditions

Terapin should be kept in a dark and dry place, out of reach of children. Temperature level should be within 15-25°C.
