Terbized is an antimycotic from the allylamines category and has a large range of antimycotic activity.

In low concentrations, terbinafine has fungicidal properties relative to dermatophytes, individual dimorphic and mold fungi. Influence affecting yeast, taking into account their type, may be fungistatic or fungicidal.

Terbinafine has a specific inhibitory effect on the early phase of sterol biosynthesis inside the fungal cell. As a result, the lack of ergosterol with intracellular cumulation of squalene develops, which is why fungal cells die.

Indications Terbizeda

It is used in case of fungal infections of the epidermis, which are provoked by dermatophytes, including trihofiton (for example, red trihofiton, T.mentagrophytes, wart trihofiton and T.violaceum), fluffy microsporum and flaky epidermofiton.

It is also prescribed for yeast infections of the epidermis (mainly provoked by Candida fungi - for example, Candida albicans).

It is used in the treatment of pityriasis, caused by the activity of Pityrosporum orbiculare.

Release form

The release of the substance produced in the form of a cream - inside a tube with a capacity of 10 g.

Pharmacodynamics

Terbinafine influences by slowing the activity of the enzyme squalene oxidase inside the cell fungal wall. The specified enzyme is not included in the hemoprotein P450 group. Component terbinafine has no effect on metabolic hormonal processes or the metabolism of other drugs.

Pharmacokinetics

After local use, terbinafine is insignificantly absorbed inside the circulatory system (below 5% of the dosage), therefore the systemic effect of the drug is minimal.

Most of the terbinafine is synthesized with intraplasma blood protein (99%). The substance passes quickly through the epidermis, cumulating inside the lipophilic stratum corneum. In addition, terbinafine gets inside the secretions of the sebaceous glands. The medicine gets inside the nail plates in the first weeks after the start of treatment. Metabolic processes develop inside the liver; inactive metabolic elements are formed.

Most of the inactive metabolic components (71%) are excreted along with urine, and a smaller part with feces (22%). The final half-life term is 11-17 hours. Gets inside maternal milk. Data on the accumulation of substances is not available.

In individuals with problems in the liver or kidneys, Terbizeda’s excretion process can be slowed down, which results in elevated levels of terbinafine inside the blood.

Dosing and administration

The cream is used for external treatment - it is applied to the epidermis 1-2 times a day, taking into account the type of pathology. The duration of the course is selected depending on the course of the disease.

Before using the drug you need to dry and clean the infected areas. Apply the cream should be a thin layer, a little rubbing - in the affected area and surrounding areas. In the case of infections in which the skin folds have become diaper (between the fingers, under the breast, in the groin area and in the interglacial area), the areas treated with the medicine should be covered with gauze, especially at night.

On average, therapy usually lasts:

  • in case of an interdigital athlete's athlete (“athlete's foot” is included) 1 times per day in the period of 7 days;
  • with squamous-hyperkeratic or dyshidrotic athlete, they are used 2 times a day for a 14-day period;
  • in case of inguinal or trunk dermatomycosis, it is used 1 time per day within 7 days;
  • with epidermal candidiasis - 1-2 times a day in a 1-2-week period;
  • with a shingles - scaby - 1-2 times a day for 14 days.

The weakening of the intensity of clinical symptoms often develops after several days of therapy. If therapy is carried out irregularly or it is terminated prematurely, the likelihood of a relapse of infection appears. In the absence of any improvement in the patient’s condition, the diagnosis needs to be clarified.

Use Terbizeda during pregnancy

Because there is only limited information regarding the clinical experience of Terbized during pregnancy, it is allowed to prescribe it during this period only in situations where the likely benefits for the woman are more expected than the negative consequences for the fetus.

Terbinafine is excreted with breast milk, which is why breastfeeding is not allowed to use the medicine. Infants should not be in contact with the cream-treated epidermis (for example, skin in the chest area).

Contraindications

It is contraindicated to prescribe medication to persons with strong intolerance against terbinafine or other elements of drugs.
