Terbisil is an antifungal agent with a wide spectrum of medicinal activity. It belongs to the allylamine category. Its active ingredient is terbinafine.
At low concentrations, terbinafine has a fungicidal effect, with susceptibility to dermatophytes (downy microsporum, red trichophyton, flocculent epidermophyton, etc.), mold fungi, and also fungi from the dimorphic family (such as Pityrosporum orbiculare).
In relation to yeast fungi, the drug has both fungistatic and fungicidal effects.
Indications Terbisil
It is used in cases of nail and epidermal lesions of a mycotic nature, hair and scalp diseases of fungal origin (including microsporia, epidermal candidiasis, onychomycosis, rubromycosis, epidermophytosis with trichophytosis and candidiasis affecting the mucous membranes), as well as in cases of pityriasis versicolor (exclusively forms for local treatment).

Pharmacodynamics
Fungicidal activity develops by inhibiting the synthesis of cellular sterol, which results in the suppression of ergosterol production, which leads to the death of the bacteria.
At the same time, Terbisil helps to slow down the activity of the enzyme squalene epoxidase. It is located inside the cell wall, and the disorder of its function causes the accumulation of squalene inside the cell and the death of the pathogenic fungus.

Dosing and administration
The duration of treatment is mainly selected taking into account the degree of intensity of the pathology and its nature.
Using tablets.
Children over 3 years of age are prescribed a dose of 125 mg once a day.
Adults take the medication in a dose of 0.25 g (it is recommended to take it in 1 dose).
In the case of onychomycosis, the therapeutic cycle usually lasts 1.5-3 months (determined by the speed at which the healthy nail grows).
In addition, there are other factors by which the duration of therapy is selected: the presence of concomitant diseases and age.
The medicine can be prescribed for dermatomycosis affecting the feet (interdigital or plantar, as well as in the form of "socks"). This course lasts 0.5-1.5 months. In the case of dermatomycosis affecting other areas of the epidermis, the treatment is 0.5-1 month. For diseases of mycotic genesis affecting the head - 1 month.
Application of cream.
The cream should be applied 1-2 times a day. The affected areas should be dried and cleaned before using Terbisil. The medicine should be spread in a thin layer, treating not only the infected area, but also the adjacent parts of healthy epidermis.
In case of fungal infections, which are accompanied by diaper rash, the treated area should be covered with a bandage.

Use Terbisil during pregnancy
It is prohibited to use the medication during pregnancy or breastfeeding.
Contraindications
The use of the drug is contraindicated in case of severe intolerance associated with terbinafine.
The drug should be used with great caution in cases of diseases associated with the kidneys and liver, against the background of which there are disorders in their functioning and insufficiency.
Terbisil should also be used with extreme caution if the patient has pathologies affecting the hematopoietic system, oncological, endocrine and vascular diseases.
Side effects Terbisil
Sometimes the following side effects may develop:
- disorders associated with the digestive system: nausea, loss of appetite, pain affecting the epigastrium, and diarrhea;
- epidermal manifestations of allergy: rashes or urticaria;
- pain may occur that affects muscles or joints;
- disorders of the nervous system: dizziness or headaches;
- taste disturbances (may reach its complete loss), congestive symptoms in the hepato-pancreato-duodenal zone (for example, cholestatic jaundice), PEE and bullous dermatitis, sometimes leading to the development of TEN, are occasionally observed;
- suppression of the activity of the hematopoietic organs is possible: neutro-, thrombocyto- or lymphopenia, as well as agranulocytosis;
- Burning, hyperemia or itching may occur in the area where the drug is applied.

Overdose
Intoxication develops only occasionally, symptoms include: nausea, dizziness, sharp stomach pain, tinnitus and headaches.
It is necessary to remove the medicine from the body: perform gastric lavage and prescribe activated charcoal or other sorbents to the victim.

Interactions with other drugs
Terbisil affects the clearance rates of drugs whose metabolism occurs with the help of hemoprotein P450 (including cycloserine, tolbutamide and oral contraception). At the same time, terbinafine is able to both slow down and speed up the metabolic processes of these drugs.
Metabolic processes of the drug itself can be accelerated when used with drugs that potentiate the effect of microsomal enzymes of liver cells (including rifampicin).
Medicines that inhibit the activity of the enzyme hemoprotein P450 inhibit the excretion of terbinafine, which is why the dose of the latter must be adjusted.

Storage conditions
Terbisil should be stored in a place closed to small children. Temperature indicators - not higher than 30°C.
Shelf life
Terbisil can be used for a period of 5 years from the date of production of the therapeutic substance.
Application for children
There is no data confirmed by tests regarding the safety of using the drug in pediatrics, therefore, before the age of 2, the medication can be prescribed only if there are vital indications.
Analogues
Analogues of the drug are the drugs Atifan, Terbinafine, Exifin with Binafin, and in addition to this Lamifast, Mikonorm with Lamisil, Terbinorm and Lamicon. Also on the list are Lamifen, Terbinoks and Mikofin with Fungoterbin.
