Sydnopharm is an active vasodilator (peripheral) antianginal drug. It is a donor of the NO element secreted by the drug during metabolism; it helps to stimulate the cGMP component.
With an increase in cGMP values, the smooth muscles of the cells of the vascular membranes (mainly inside the vein bed) relax, which leads to a decrease in preload, as a result of which the proportion between oxygen intake and the need for it is restored (it decreases by 26%). [1]
Indications Sydnofarm
It is used for such violations:
- prevention and elimination of angina attacks;
- active left ventricular failure;
- myocardial infarction (acute) in the phase of normalization of hemodynamic values;
- cor pulmonale (chronic form);
- Chronic heart failure (in combination with SG and diuretics);
- increased blood pressure values within the pulmonary blood flow.
Release form
The release of drugs is realized in the form of tablets with a volume of 2 or 4 mg, as well as in the form of prolonged-release tablets (volume 8 mg); contains 10 pieces inside cell packs. There are 3 such packages inside the box.
Pharmacodynamics
The narrowed vessels, which can expand, are exposed to the vasodilating effect of the drug. This leads to improved blood flow within the collaterals, increased exercise tolerance, and a decrease in the number of angina attacks during exercise.
Sydnopharm weakens the binding and secretion of proagregants (thromboxane with serotonin), which leads to inhibition of the early stage of platelet aggregation, which reduces blood clotting. [2]
At the same time, in people with CHF, the medication reduces the size of the heart chambers and lowers the blood pressure inside the artery of the lungs, which lowers the volume of blood that fills the left ventricle and weakens the tension of the myocardial wall. [3]
Pharmacokinetics
The medicine is well and at high speed absorbed inside the digestive tract. When administered orally, its effect develops after 20 minutes, and after use under the tongue - after 5-10 minutes. The maximum effect of exposure reaches 0.5-1 hour after administration; the duration of action is approximately 6 hours.
With intraplasmic protein, the drug is almost not synthesized; metabolic processes are carried out inside the liver, and excretion is carried out mainly through the kidneys.
Long-term use of drugs does not lead to the emergence of resistance, which is what differs from nitrates.
Dosing and administration
To prevent angina pectoris, you need to take a pill of medicine inside (swallow without chewing) and drink it with plain water. It is necessary to take 1-4 mg of the medication 2-3 times a day.
Prolonged tablets (8 mg) are used 1-2 times a day (if necessary, 3 times). To stop an angina attack, you need to put such a pill under the tongue.
- Application for children
Prescribing a medicine in pediatrics is contraindicated.
Use Sydnofarm during pregnancy
You cannot use Sydnopharm in the 1st trimester. In the 2nd and 3rd trimesters, it is prescribed only in cases where the need for therapy significantly exceeds the risks of complications for the fetus.
The medication is excreted in breast milk, which is why it is necessary to stop breastfeeding for the period of treatment.
Contraindications
The main contraindications:
- active phase of myocardial infarction;
- glaucoma, especially angle-closure;
- problems with intracerebral blood flow;
- an increase in ICP indicators;
- reduced blood pressure values or a tendency to such a violation;
- collapse of blood vessels;
- elderly people;
- strong personal sensitivity to the elements of the drug.
Side effects Sydnofarm
The use of drugs can provoke a mild headache that disappears if you continue to take the pills.
There may be a decrease in blood pressure, sometimes reaching a collapse. Moreover, the higher the initial blood pressure, the more intense its fall will be. In addition, there may be a slowdown in motor and mental reactions.
Development of bronchial spasm, dizziness, itching, epidermal rashes and other signs of allergy is possible.
Overdose
Poisoning with the drug causes vomiting, dizziness, nausea and severe headache. There is also a sharp drop in blood pressure, which is life-threatening.
With such disorders, symptomatic actions are taken.
Interactions with other drugs
Sydnopharm potentiates the antihypertensive effect of antihypertensive drugs and vasodilators, and in addition, the antiplatelet effect of aspirin.
Storage conditions
Sidnopharm must be stored in a place protected from moisture penetration. Temperature - within 15-25 ° C.
Shelf life
Sydnopharm can be used for 36 months from the date of production of the therapeutic substance.
Analogs
