Sidnofarm is an active vasodilator (peripheral) antianginal drug. It is a donor of the NO element secreted by the drug during metabolism; it helps stimulate the cGMP component.

With an increase in cGMP values, there is a relaxation of the smooth muscles of the cells of the vascular membranes (mainly inside the venous bed), which leads to a decrease in preload, as a result of which the proportion between oxygen intake and the need for it is restored (it decreases by 26%). [ 1 ]

Indications Sydnopharm

It is used for the following disorders:

  • prevention and elimination of angina attacks;
  • left ventricular failure in the active phase;
  • myocardial infarction (acute) in the phase of normalization of hemodynamic values;
  • pulmonary heart disease (chronic form);
  • Chronic heart failure (in combination with CG and diuretics);
  • increased blood pressure within the pulmonary circulation.

Release form

The drug is produced in the form of tablets with a volume of 2 or 4 mg, as well as in the form of prolonged-release tablets (volume of 8 mg); 10 pieces are contained in blister packs. There are 3 such packs in a box.

Pharmacodynamics

Narrowed vessels that can expand are subject to the vasodilatory effect of the drug. This leads to improved blood flow within the collaterals, increased tolerance to stress, and a decrease in the number of angina attacks during physical exertion.

Sidnofarm weakens the binding and secretion of proaggregants (thromboxane with serotonin), which leads to the suppression of the early stage of platelet aggregation, due to which blood clotting decreases. [ 2 ]

At the same time, in people with CHF, the drug reduces the size of the heart chambers and lowers blood pressure inside the pulmonary artery, which reduces the volume of blood that fills the left ventricle and reduces the tension of the myocardial wall. [ 3 ]

Pharmacokinetics

The medicine is well and quickly absorbed inside the digestive tract. When taken orally, its effect develops after 20 minutes, and after sublingual use - after 5-10 minutes. The maximum effect is achieved by 0.5-1 hour after administration; the duration of action is approximately 6 hours.

The drug is almost not synthesized with intraplasmic protein; metabolic processes are realized inside the liver, and excretion occurs mainly through the kidneys.

Long-term use of the drug does not lead to the development of resistance, which is how it differs from nitrates.

Dosing and administration

To prevent an angina attack, you need to take a tablet of the medicine orally (swallow it without chewing) and wash it down with plain water. You need to take 1-4 mg of the medicine 2-3 times a day.

Extended-release tablets (8 mg) are taken 1-2 times a day (3 times if necessary). To stop an angina attack, you need to put such a tablet under your tongue.

  • Application for children

Prescribing medication in pediatrics is contraindicated.

Use Sydnopharm during pregnancy

Sidnofarm cannot be used in the 1st trimester. In the 2nd and 3rd trimesters, it is prescribed only in cases where the need for therapy significantly exceeds the risks of complications for the fetus.

The medication is excreted in breast milk, which is why breastfeeding should be discontinued during treatment.

Contraindications

Main contraindications:

  • active phase of myocardial infarction;
  • glaucoma, especially closed-angle glaucoma;
  • problems with cerebral blood flow;
  • increase in intracranial pressure;
  • reduced blood pressure values or a tendency to such a disorder;
  • vascular collapse;
  • elderly people;
  • strong personal sensitivity to the elements of the drug.

Side effects Sydnopharm

The use of the drug may cause a mild headache, which disappears if you continue taking the pills.

A decrease in blood pressure may be observed, sometimes reaching collapse. Moreover, the higher the initial blood pressure, the more intense its fall will be. In addition, a slowdown in motor and mental reactions may occur.

The development of bronchial spasms, dizziness, itching, epidermal rashes and other signs of allergy is possible.

Overdose

Poisoning with the drug causes vomiting, dizziness, nausea and severe headache. There is also a sharp drop in blood pressure, which is life-threatening.

In such disorders, symptomatic actions are carried out.

Interactions with other drugs

Sidnofarm potentiates the antihypertensive effect of antihypertensive drugs and vasodilators, and in addition, the antiplatelet effect exerted by aspirin.

Storage conditions

Sidnofarm must be stored in a place protected from moisture penetration. Temperature – within the range of 15-25оC.

Shelf life

Sidnofarm can be used for a period of 36 months from the date of production of the therapeutic substance.

Analogues of the drug are Corvaton with Molsidomine.
