Revmoxicam is a selective NSAID. The active ingredient of the drug is meloxicam.

Indications Rheumoxicam

It is used to eliminate pain symptoms in people with diseases affecting the musculoskeletal system and having a degenerative-inflammatory form.

It is used for osteoarthrosis and arthrosis. In addition, it is used in the combined treatment of rheumatoid arthritis and Bechterew's disease.

Release form

The drug is available in tablet form (7.5 or 15 mg), as well as in injection liquid and suppositories.

The tablets are distributed in blister strips, in the amount of 10 pieces. Inside the box there are 1-2 strips.

Rectal suppositories are packaged with 5 pieces in a blister, 1 blister in a pack.

Injection liquid – in ampoules with a capacity of 1.5 mg. Inside the box there are 5 such ampoules.

Pharmacodynamics

The drug has a strong antipyretic, anti-inflammatory and analgesic activity. The effect of the drug is due to the fact that its active element is able to suppress the function of COX-2. Due to this, there is a decrease in the binding of proinflammatory cytokines, which act as mediators of inflammation processes. There is almost no effect on the activity of COX-1. Suppression of the binding of cytoprotective PG and thromboxane is also not recorded.

Taking the drug relieves pain and reduces the intensity of inflammation in diseases associated with the functioning of the musculoskeletal system. At the same time, Revmoxicam does not affect the function of chondrocytes (for example, the processes of binding the proteoglycan element).

Pharmacokinetics

When taken orally, meloxicam is well absorbed in the gastrointestinal tract, demonstrating a bioavailability rate of 89%. Food intake does not affect the degree of absorption of the drug.

On the 3rd-5th day from the start of therapy, a stable meloxicam indicator is recorded inside the body (it does not matter what form of the drug is used). Synthesis with intraplasmic protein is 99%, and the meloxicam values inside the synovium are 50% of its plasma indicators.

The component undergoes complete metabolism in the liver, transforming into inactive decay products. The half-life is 20 hours. The substance is excreted by the intestines and kidneys (in approximately equal proportions). Meloxicam can pass through the BBB and histiocytic barrier.

Dosing and administration

Scheme of use of tablet form.

The medicine is swallowed whole, without chewing, with plenty of plain water. The tablets should be taken with food to reduce the risk of negative symptoms in the gastrointestinal tract. The size of the dosage portions is selected by the doctor individually. Usually, people with osteoarthritis should take 7.5 mg of the substance per day. If necessary, the portion can be doubled - up to 15 mg.

People with rheumatoid arthritis or Bechterew's disease need to take 15 mg of the drug per day. Then, after achieving the desired effect, it is necessary to use a maintenance dose of 7.5 mg/day.

People who are prone to developing negative symptoms are allowed to use the drug in dosages not exceeding 7.5 mg.

Mode of administration of injections of medicinal substance.

The medication should be administered intramuscularly, parenterally. The injection should be deep and administered into the gluteal region. The duration of the therapeutic cycle is selected by the doctor for each patient individually.

About 0.75-1.5 ml of the drug is usually administered per day. Injections are given once per day, and they can be given no more than 5 days in a row. If necessary, the drug is used in other medicinal forms. No more than 15 mg of the drug can be administered per day.

Scheme of use of rectal suppositories.

The medication is administered rectally. Before the procedure, all necessary hygiene measures must be taken. The duration of the course and the portion size are selected by the attending physician. Usually, the administration of 1 suppository is prescribed once a day. It is allowed to use Revmoksikam for a maximum of 7 days in a row.

For gynecological procedures, suppositories are used exclusively as prescribed by a doctor, according to a regimen selected by a gynecologist.

Use Rheumoxicam during pregnancy

It is forbidden to use the medicine (in any form) during pregnancy or lactation. It is important to completely exclude the possibility of pregnancy before starting therapy. During the course of treatment, you should use reliable contraception. You cannot use Revmoksikam when planning conception.

Contraindications

Main contraindications:

  • the presence of hypersensitivity to the components of the medication;
  • glucose-galactose malabsorption, galactosemia, lactose intolerance (for tablets);
  • ulcerative lesions developing inside the gastrointestinal tract;
  • severe heart failure;
  • kidney or liver diseases that are severe;
  • rehabilitation after CABG surgery;
  • a person has a tendency to bleed or has recently had cerebrovascular bleeding;
  • pathologies occurring inside the rectum (for suppositories).

Intravenous administration of the drug is prohibited.

It should be used with caution by people with bronchial asthma, liver cirrhosis, and also with exhaustion or dehydration of the body. Also, it should be used with caution by elderly people or those who take anticoagulants, as well as people with diseases in the digestive system.

Side effects Rheumoxicam

The use of the drug may cause the occurrence of certain side effects:

  • disorders of the liver and digestive system: problems with stool and digestive function, flatulence and nausea, abdominal pain and vomiting, as well as belching and increased activity of liver enzymes. Gastritis, bleeding, ulcers and erosions in the gastrointestinal mucosa and stomatitis are observed sporadically;
  • dysfunction of the cardiovascular system and hematopoietic processes: swelling in the face or limbs, heart rhythm disorders, increased pressure, hyperemia, as well as thrombocytopenia or leukopenia and anemia. Progression of heart failure and development of hypertension are possible in people with diseases in the cardiovascular system;
  • lesions affecting the functioning of the nervous system: headaches, emotional instability, dizziness and disturbances in the day/night routine;
  • urinary disorders: glomerular nephritis, acute renal failure, tubulointerstitial nephritis and changes in blood urea and creatinine levels, as well as nephrotic syndrome. Necrotic papillitis has developed sporadically;
  • damage to the sense organs: tinnitus, decreased vision or conjunctivitis;
  • allergy symptoms: urticaria, itching, photosensitivity, bronchospasm, rash, or Stevens-Johnson syndrome. Angioedema or anaphylaxis may occur;
  • other signs: the appearance of itching, irritation or burning in the anal area (when using suppositories).

If any negative symptoms occur after using any form of the drug, you must immediately stop treatment and consult your doctor.

Overdose

Taking the medication in large doses can cause abdominal pain, nausea, drowsiness and vomiting, as well as lethargy. In addition, overdosing on the drug increases the risk of negative symptoms and increases their intensity.

A significant excess of the permissible portion size can lead to acute meloxicam intoxication, which causes liver and kidney dysfunction and increased blood pressure. Coma and cardiac arrest can also occur.

The medicine has no antidote. To get rid of the disorders, it is necessary to wash the stomach (if poisoning occurred with tablets) or the rectum (in case of intoxication with rectal suppositories). In addition, symptomatic measures are carried out. If severe poisoning is observed, the victim must be hospitalized.

Interactions with other drugs

The combined use of the drug with other medications from the category of non-narcotic analgesics increases the risk of bleeding inside the gastrointestinal tract and the development of ulcers.

It is impossible to combine Revmoxicam with SSRIs, antiplatelet agents and anticoagulants, as well as with heparin and thrombolytic agents.

Concomitant use with diuretics in dehydrated individuals increases the risk of acute renal failure. If such a combination is necessary, renal function should be monitored regularly.

The drug weakens the effect of diuretics, vasodilators, drugs that slow down the activity of ACE, β-blockers and contraceptives placed inside the uterus.

The drug potentiates the hematotoxic effect of methotrexate, as well as the negative effect of cyclosporine on the kidneys. Therefore, in such interactions, it is important to monitor kidney function and peripheral blood counts.

The drug increases plasma lithium levels.

Combining Rhumoxicam with cholestyramine reduces the half-life of the substance meloxicam.

The effectiveness of oral antidiabetic drugs and insulin may change when combined with the drug. When prescribing the drug to people with diabetes, it is necessary to monitor sugar levels and, if necessary, change the dosage of the antidiabetic drug.

It is prohibited to mix the injection liquid with other medications in the same syringe.

Storage conditions

Reumoksikam in tablet form must be kept in a place protected from moisture and sunlight, at a temperature of 15-25°C.

The solution and candles require temperature readings within the range of 8-15°C.

Shelf life

Reumoksikam is allowed to be used for 24 months from the date of release of the drug.

Application for children

The medicine in any form should not be prescribed to children under 15 years of age.
