Renitek is a medicine from the category of antihypertensive drugs. It contains the active ingredient enalapril maleate.
Inside the body, this component is converted into a therapeutically effective form – the substance enalaprilat, which has a powerful inhibitory effect on the ACE element. As a result of the suppression of ACE action, there is a decrease in the processes of converting angiotensin-1 to angiotensin-2, as well as an increase in the activity of plasma renin and a weakening of the binding of aldosterone. Indications Renitec
It is used in the treatment of individuals with different stages of renovascular or primary hypertension.
It can be prescribed to people with heart failure as a substance that increases the likelihood of survival, reduces the need for hospitalization, and slows the development of the pathology.
It is used in cases of asymptomatic left ventricular dysfunction to prevent the development of severe heart failure.
For people with left ventricular dysfunction, the medication is prescribed to prevent the development of coronary ischemia.
It is used in people with unstable angina to prevent myocardial infarction and reduce the incidence of hospitalization.
Release form
The medicinal substance is released in tablets - 7 pieces inside a cell plate; inside a box - 2 or 4 such plates. Also, tablets can be packed in bottles - 100 pieces (1 bottle inside a pack).
Pharmacodynamics
The use of the drug leads to an increase in the values of PG-E and NO, increases the excretion of sodium ions and slightly reduces the excretion of potassium ions, and in addition reduces the levels of circulating catecholamines.
Enalaprilat helps to reduce blood pressure, and in people with primary hypertension it leads to a decrease in systemic resistance in the peripheral vessels and a slight increase in cardiac output. [ 2 ]
In subjects with proteinuria and renal dysfunction, treatment resulted in a reduction in albuminuria and a decrease in urinary excretion of IgG element and systemic urinary protein.[ 3 ]
Enalaprilat stimulates regression of left ventricular hypertrophy, maintaining systolic activity of this organ; in people with heart failure, the drug reduces the incidence of ventricular arrhythmias.
The medication has a weak effect on glucose and lipoprotein metabolism processes.
Pharmacokinetics
When taken orally, the medicinal effect develops after 1-4 hours, after which it continues for 24 hours.
Dosing and administration
The medicine is taken orally, swallowing the whole tablet and washing it down with plain water. It does not need to be crushed or chewed. If necessary, the tablet can be divided in half. The drug can be taken without reference to food.
To obtain maximum medicinal effect with constant monitoring of blood pressure values, the medication should be taken at the same time of day. The duration of the treatment cycle and dosage sizes are individually selected by the attending physician.
Adults with primary hypertension often take 10-20 mg of the drug once a day. If the drug is not effective enough, its dosage is gradually increased. The recommended maintenance dose is 20 mg. A maximum of 40 mg is allowed per day.
An adult with renovascular hypertension often requires 2.5-5 mg of the substance once a day. If there is no improvement, the dosage can be gradually increased. The maintenance dose is 10-20 mg.
People who have taken diuretics shortly before starting to use Renitec can initially use a maximum of 5 mg of the drug. The dosage can be gradually increased if there is no effect. The use of diuretics should be stopped at least 2-3 days before starting to take Renitec.
People with renal impairment and CC levels within 30-80 ml per minute should take 5-10 mg of the drug once a day. With CC values in the range of 10-30 ml per minute, the medication is taken once a day in a dose of 2.5-5 mg. If CC values are less than 10 ml per minute, on days when dialysis is performed, 2.5 mg should be taken once a day. The rest of the time, the dose is selected individually.
People with asymptomatic left ventricular dysfunction and heart failure should take 2.5 mg of the drug once a day. It can be used in combination therapy for heart failure (for example, in combination with diuretics and digitalis). If the drug is well tolerated (without a decrease in blood pressure) or after correction of hypotension signs, the dose is gradually increased. The maintenance dose is 20 mg once a day or 10 mg twice a day.
People with HF need to constantly monitor blood pressure, plasma potassium levels, and renal function.
The age group under 16 years with elevated blood pressure should take 0.08 mg/kg of the drug once a day. If necessary, the dose can be gradually increased.
- Application for children
Renitek should not be administered to infants under 1 month of age and to children whose CF values are less than 30 ml per minute/1.73 m2.
Use Renitec during pregnancy
The use of the drug during pregnancy is prohibited, except in cases where there are strict indications for this. It is necessary to make sure that the woman is not pregnant before prescribing the drug.
During treatment with Renitek, patients of reproductive age should use reliable contraception. If conception does occur, the drug should be discontinued immediately.
Use of the drug in the 2nd and 3rd trimesters may cause death of the fetus or newborn, kidney dysfunction, hyperkalemia, pulmonary and cranial hypoplasia, and, along with this, a decrease in fetal blood pressure. Newborns whose mothers took the drug during pregnancy should be closely monitored.
If the drug needs to be taken during lactation, it is necessary to consult with the attending physician regarding the need to stop breastfeeding.
Contraindications
Contraindicated for use in cases of severe personal intolerance to the components of the drug and other medications from the ACE inhibitor category.
Not used in people with Quincke's edema, which is idiopathic or hereditary.
Also not prescribed to persons with glucose-galactose malabsorption, lactose intolerance or galactosemia.
Do not administer if the patient is undergoing hemodialysis sessions using membranes with a high level of permeability.
The drug is used very carefully before performing operations and in case of disorders of EBV parameters (for example, hyponatremia or -volemia).
It should be used with extreme caution in cases where the patient has coronary heart disease, heart failure, renal dysfunction or cerebrovascular manifestations, and also during treatment with diuretics or hemodialysis sessions.
Caution is required when administering to people with aortic stenosis or stenosis affecting the arteries of one or both kidneys.
Side effects Renitec
Often the medication is tolerated without complications. The side effects that sometimes occur are of low intensity and do not require discontinuing the drug. Among the violations:
- problems with the liver and gastrointestinal tract: bowel disorders, vomiting, loss of appetite, pain in the epigastric region and nausea. Intestinal obstruction, hepatitis, pancreatitis or jaundice are observed sporadically;
- disorders associated with the function of the central nervous system and the peripheral nervous system: tinnitus, headaches, increased fatigue, dizziness, emotional instability, asthenia, paresthesia and circadian rhythm disorder. Confusion and depression are occasionally observed;
- lesions affecting the activity of the cardiovascular system: decreased blood pressure (sometimes leading to orthostatic collapse), arrhythmia, severe pain in the sternum, palpitations and angina. Occasionally (usually in people at risk), a stroke or myocardial infarction develops;
- disorders of hematopoietic function: neutro- or thrombocytopenia, as well as agranulocytosis;
- symptoms associated with urination: oliguria, acute renal failure and renal dysfunction;
- changes in test results: increased values of creatinine with urea, bilirubin and intrahepatic enzymes in blood plasma. An increase in blood potassium levels or a decrease in blood sodium levels may be observed occasionally, as well as a decrease in hemoglobin with hematocrit;
- signs of allergy: itching, bronchial spasm, epidermal rashes, Quincke's edema, SJS, urticaria and allergic rhinitis;
- Others: pharyngitis, alopecia, dry cough, impotence, hyperhidrosis and visual impairment.
Overdose
The use of excessively high doses of Renitec leads to the development of stupor and a strong decrease in blood pressure.
There is no antidote. In case of poisoning, gastric lavage and enterosorbents are used (when less than 120 minutes have passed since the drug was taken). In addition, in case of intoxication, against the background of which the blood pressure indicator drops sharply, 0.9% NaCl is administered via infusion and angiotensin-2 is used.
To reduce plasma levels of enalaprilat, hemodialysis procedures can be performed.
Interactions with other drugs
Diuretics and hypotensive substances in combination with the drug lead to potentiation of the antihypertensive effect.
Administration together with potassium-sparing diuretics and potassium medications increases the likelihood of hyperkalemia.
Combination with lithium substances enhances their toxic properties and weakens lithium excretion.
Administration simultaneously with non-narcotic analgesics increases the likelihood of developing nephrotoxic activity.
Storage conditions
Renitek must be stored in a dry and dark place at temperatures in the range of 15-30°C.
Shelf life
Renitec can be used for a period of 30 months from the date of manufacture of the medicinal product.
Analogues
Analogues of the drug are the substances Enap, Enam, Enalapril with Enalozide, as well as Co-Renitek and Berlipril.
