Renitek is a medication from the category of antihypertensive drugs. It contains an active element - enalapril maleate.

Inside the body, this component is converted into a form that has a therapeutic effect - into the substance enalaprilat, which has a powerful inhibitory effect relative to the ACE element. As a result of the suppression of the action of ACE, there is a decrease in the conversion of angiotensin-1 into angiotensin-2, as well as an increase in the activity of plasma renin and a weakening of the binding of aldosterone. [1]

It is used in the treatment of individuals with different stages of renovascular or primary hypertension .

Pharmacodynamics

The use of the medication leads to an increase in the values of PG-E and NO, increases the excretion of sodium ions and insignificantly reduces the excretion of potassium ions, and in addition reduces the indices of circulating catecholamines.

Enalaprilat reduces blood pressure, and in people with primary hypertension it leads to a weakening of systemic resistance in the region of peripheral vessels and a slight increase in cardiac output. [2]

The medicine is taken orally by swallowing a whole tablet and drinking it with plain water. It doesn't need to be crushed or chewed. If necessary, dividing the tablet in half is allowed. You can use the drug without being tied to food.

To get the maximum medicinal effect with constant monitoring of blood pressure values, you should take the medication at the same time of the day. The duration of the treatment cycle and the size of the dosages are personally selected by the attending physician.

Adults with primary hypertension often take 10-20 mg of the medication 1 time per day. With insufficient effectiveness of the drug, its dosage is gradually increased. The recommended maintenance serving size is 20 mg. A maximum of 40 mg is allowed per day.

  • lesions affecting the activity of the CVS: a decrease in blood pressure values (sometimes it reaches orthostatic collapse), arrhythmia, severe pain in the sternum, palpitations and angina pectoris.   • violations of hematopoietic function: neutro- or thrombocytopenia, as well as agranulocytosis;
  • symptoms associated with urinary activity: oliguria, acute renal failure and renal dysfunction;
  • change in test readings: an increase in the values of creatinine with urea, bilirubin and intrahepatic enzymes inside the blood plasma. A single increase in blood potassium values or a decrease in blood sodium values may be observed, and in addition, a decrease in hemoglobin with hematocrit;
  • signs of allergy: itching, bronchial spasm, epidermal rashes, Quincke's edema, SS, urticaria and runny nose of allergic genesis;
  • others: pharyngitis, baldness, dry cough, impotence, hyperhidrosis and visual disturbance.

Overdose

The use of excessively high portions of Renitek leads to the development of stupor and a strong decrease in blood pressure.

There is no antidote. In case of poisoning, gastric lavage and the use of enterosorbents are carried out (when less than 120 minutes have passed since the drug was taken). In addition, in the case of intoxication, against the background of which the blood pressure indicator drops sharply, 0.9% NaCl is introduced through infusion and the use of angiotensin-2.

To reduce the plasma level of enalaprilat, hemodialysis procedures can be performed.

Interactions with other drugs

Diuretic and antihypertensive substances in combination with medication lead to potentiation of the antihypertensive effect.

Administration together with potassium-sparing diuretics and potassium medications increases the likelihood of hyperkalemia.

The combination with lithium substances enhances their toxic properties and weakens the excretion of lithium.

The introduction of simultaneously with non-narcotic analgesics increases the likelihood of developing nephrotoxic activity.
