Remmax is a medicine that combines 2 separate antacid substances in its composition – Ca carbonate and Mg carbonate. These elements have a local therapeutic effect.

The medicinal effect of the drug develops by neutralizing the gastric pH. It should be noted that the effect of the drug does not depend on the indicators of systemic absorption of the drug.

Indications Remmax

It is used to relieve symptoms associated with increased gastric pH (including gastroesophageal reflux and heartburn ) and causing the development of digestive disorders and dyspepsia.

Release form

The medicinal component is released in tablets - 6 pieces per package. There are 3 packages in a box.

Pharmacodynamics

Ca carbonate has a powerful and long-lasting neutralizing effect. This effect is potentiated by Mg carbonate, which also has a strong neutralizing effect.

The overall neutralizing capacity of the drug when tested in vitro is 16 mEqH+ (with titration to a final pH of 2.5).

Pharmacokinetics

In healthy individuals, the neutralization process begins quickly. Taking 2 tablets of the drug on an empty stomach causes an increase in pH values by 1+ units over a 5-minute period, as well as an increase in the gastric pH level above the standard pH mark; this indicator is noted after 2 minutes.

Inside the stomach, the active elements of the drug react with gastric juice, forming water together with soluble mineral salts.

Elements Mg together with Ca can be absorbed in the form of their own soluble salts. The absorption rate of these components from these compounds is determined by the size of the portion of the medicine. The highest absorption values are 10% (Ca) and 15-20% (Mg).

In a healthy person, a small part of the absorbed substances is excreted through the kidneys. If kidney function is impaired, the indicators of drug components in the blood plasma may increase. In the intestine, insoluble compounds are formed from soluble salts and excreted with feces.

Dosing and administration

The medication must be taken orally. The recommended dose is 1-2 tablets, which should be taken when heartburn or pain occurs. In case of pain, the dosage can be increased to 5 tablets per day for a short time. Therapy can last no more than 10 days.

Use Remmax during pregnancy

There is currently no information on an increased likelihood of fetal developmental abnormalities when using Ca carbonate and Mg carbonate during pregnancy. Remmax may be prescribed to pregnant women, but only with strict adherence to the instructions and without prolonged use of large doses. It is also necessary to take into account that magnesium salts can provoke the development of diarrhea, and large doses of calcium used for a long time increase the likelihood of hypercalcemia, accompanied by calcification of various organs, including the kidneys.

It should also be remembered that during lactation and pregnancy, the above components provide a large amount of calcium in addition to the amounts that a woman receives with food. Because of this, pregnant patients should use Remmax for no more than 7 days, and also not consume milk or any dairy products during therapy. This will prevent an excess of the Ca element, which can cause the development of a rare but quite severe disease - Burnett syndrome.

Contraindications

Main contraindications:

  • severe sensitivity associated with drug components;
  • hypercalcemia or conditions that may cause it;
  • nephrolithiasis associated with the deposition of calcium-containing stones;
  • severe renal failure (CC values less than 30 ml per minute);
  • hypophosphatemia.

Side effects Remmax

Side effects include:

  • immune disorders: manifestations of intolerance, including urticaria, anaphylaxis, rashes and Quincke's edema;
  • nutritional and metabolic disorders: with prolonged use of large portions, hypermagnesemia (with the introduction of magnesium-containing antacids), hypercalcemia or alkalosis with symptoms in the form of muscle weakness and gastric manifestations (especially in people with kidney problems) may develop;
  • symptoms associated with the gastrointestinal tract: vomiting, diarrhea, stomach discomfort and nausea;
  • damage to the function of the musculoskeletal system and connective tissues: muscle weakness.

Symptoms that develop in the event of Burnett syndrome:

  • gastrointestinal dysfunction: ageusia;
  • systemic disorders: asthenia or calcinosis;
  • problems with the functioning of the nervous system: headaches;
  • disorders associated with the functioning of the kidneys and urinary tract: azotemia.

Overdose

With prolonged use of large doses of the drug, especially in people with renal dysfunction, hypercalcemia, -magnesemia, or alkalosis may develop, which manifests itself as mild muscle weakness and gastrointestinal tract damage (vomiting, constipation, and nausea). In case of drug poisoning, diarrhea may also occur.

It is necessary to stop using the drug and provide the patient with sufficient fluid intake. In case of severe intoxication (for example, Burnet syndrome), it is necessary to consult a doctor, because additional rehydration (for example, infusion) may be necessary.

Interactions with other drugs

Other medications should be taken 60-120 minutes before or after Remmax administration.

With the combined use of the drug and antibiotics (quinolones with tetracycline), phosphates, SG (digoxin), iron-containing substances, fluoride compounds, eltrombopag and levothyroxine, the absorption of the latter is weakened.

Thiazide diuretics reduce urinary Ca excretion and increase serum Ca levels. The increased risk of hypercalcemia when combined with such medications requires constant monitoring of serum Ca levels.

At the same time, it is necessary to observe a 2-hour interval between the administration of Remmax and the following drugs: metoprolol, digoxin, indomethacin with atenolol, chloroquine, fexofenadine with propranolol, as well as antihistamines and diflunisal. In addition, the list includes phenothiazide neuroleptics, diphosphonates, penicillamine with ketoconazole, GCS (such as dexamethasone and prednisolone) and thyroxine.

Storage conditions

Remmax should be stored in a place inaccessible to small children. Temperature indicators - no more than 25 ° C.

Shelf life

Remmax can be used within a 2-year period from the date of manufacture of the medicinal substance.

Application for children

Not for use in children under 15 years of age.
