Remmax is a drug that combines 2 separate antacid substances - Ca carbonate and along with Mg carbonate. These elements have a local therapeutic effect.

The medicinal effect of the drug is developed by neutralizing the gastric pH. It should be noted that the effect exerted by the drug does not depend on the indicators of systemic absorption of the drug substance.

Indications Remmax

It is used to alleviate the manifestations that are associated with an increase in gastric pH (among those gastroesophageal reflux and heartburn ) and cause the development of digestive disorders and dyspepsia.

Release form

The release of the drug component is produced in tablets - 6 pieces inside the package. In the box - 3 packs.

Pharmacodynamics

Ca carbonate has a powerful and long lasting neutralizing effect. This effect is potentiated under the influence of Mg carbonate, which also has a strong neutralizing effect.

The total indices of the neutralizing capacity of the drug during in vitro testing are 16 mEqH + (with titration to a final pH of 2.5).

Pharmacokinetics

In healthy people, the process of neutralization begins quickly. The use of 2 tablets of drugs on an empty stomach causes an increase in pH by 1 + unit per 5-minute period, as well as an increase in the level of gastric pH above the standard pH; This indicator is marked after 2 minutes.

Inside the stomach, the active elements of the drug react with the gastric juice to form water along with mineral salts of a soluble nature.

The elements Mg together with Ca are able to be absorbed in the form of their own soluble salts. The rate of absorption of these components from these compounds is determined by the size of the portion of the drug. The highest values of absorption are 10% (Ca) and 15-20% (Mg).

In a healthy person, a small part of the absorbed substances is excreted through the kidneys. If the renal function is impaired, the indicators of the drug components inside the blood plasma may increase. Inside the intestine, insoluble compounds are formed from salts of soluble character, excreted in feces.

Dosing and administration

The medication is required to use inside. The size of the recommended serving is 1-2 tablets, which should be taken when heartburn or pain appears. With pain, the dosage can be briefly increased to 5 tablets per day. Therapy can last no more than 10 days.

Use Remmax during pregnancy

Currently, there is no information about the increased likelihood of abnormalities in the development of the fetus when using Ca carbonate and Mg carbonate during pregnancy. Remmax is allowed to be prescribed for pregnant women, but only with strict observance of the instructions and without prolonged use of large portions. It is necessary to take into account the fact that magnesium salts can provoke the development of diarrhea, and large doses of calcium, used for a long time, increase the likelihood of hypercalcemia, accompanied by calcification of various organs, including the kidneys.

It should be remembered that during lactation and pregnancy, these components provide large amounts of calcium in addition to the amount that a woman receives with food. Because of this, pregnant patients should use Remmax for no more than 7 days, and also not use milk and any dairy products during therapy. This will prevent an excess of the Ca element, which can cause the development of a rare, but rather severe disease - Burnet syndrome.

Contraindications

The main contraindications:

  • strong sensitivity associated with medication elements;
  • hypercalcemia or conditions that can cause it;
  • associated with the deposition of calcium calculus nephrolithiasis;
  • renal insufficiency of work, which has a pronounced form (CC values less than 30 ml per minute);
  • gypophosphatemia.

Side effects Remmax

Among the side effects:

  • immune disorders: manifestations of intolerance, including urticaria, anaphylaxis, rashes and angioedema;
  • eating disorders and metabolic processes: with long-term use of large portions hypermagnemia can develop (with the introduction of magnesium-containing antacids), hypercalcemia or alkalosis with symptoms in the form of muscle weakness and gastric manifestations (especially in people with kidney problems);
  • signs associated with the work of the digestive tract: vomiting, diarrhea, gastric discomfort and nausea;
  • damage to the function of musculoskeletal system and connective tissues: muscle weakness.

Symptoms that develop in the event of Burnet syndrome:

  • Gastrointestinal dysfunction: Agevziya;
  • systemic disorders: asthenia or calcification;
  • problems with the work of NA: headaches;
  • disorders associated with the activity of the kidneys and urethra: azotemia.

Overdose

With prolonged use of large portions of the drug, especially in individuals with disorders of renal activity, it is possible to develop hypercalcemia, -magnesium, or alkalosis, which manifest as slight muscle weakness and gastrointestinal lesions (vomiting, nausea and nausea). In case of poisoning of drugs, diarrhea may also occur.

It is necessary to stop using the medication and provide the patient with sufficient fluid intake. In case of severe intoxication (for example, Burnet's syndrome), it is necessary to consult a doctor, because additional rehydration may be necessary (for example, an infusion).

Interactions with other drugs

Other medications should be consumed 60-120 minutes before or after the administration of Remmax.

With the combined use of drugs and antibiotics (quinolones with tetracycline), phosphates, SG (digoxin), iron-containing substances, fluorine compounds, elthrombopag and levothyroxine, the absorption of the latter is weakened.

Thiazide diuretic substances weaken Ca excretion along with urine and increase its serum values. Increased likelihood of hypercalcemia when combined with such drugs requires constant monitoring of serum indicators of Ca.

At the same time, it is necessary to observe the 2-hour interval between the administration of Remmax and such drugs: metoprolol, digoxin, indomethacin with atenolol, chloroquine, fexofenadine with propranolol, and in addition antihistamines and diflunisal. In addition, the list includes phenothiazide neuroleptics, diphosphonates, penicillamine with ketoconazole, GCS (such as dexamethasone and prednisolone) and thyroxin.
