Indications Raunatina
It is used to reduce elevated blood pressure (moderate or mild hypertension).
Release form
Released in the form of tablets with a volume of 2 mg. A separate pack contains 50 or 100 tablets of the medicine.
Pharmacodynamics
The medicine is of plant origin, it contains a sum of alkaloids obtained from the roots of Rauvólfia serpentina or the root bark of the emetic rauwolfia (family Kurtov). Among the alkaloids are serpentine with reserpine, as well as ajmaline, etc.
Raunatin has antihypertensive properties, which develop as a result of lowering the level of biogenic amines (such as norepinephrine with serotonin and dopamine) inside the CNS (inside the cortico-hypothalamic systems, especially in the posterior region of the hypothalamus). By preventing the deposition of vesicular conductors inside the presynaptic membrane in the area of the peripheral nerve receptors of the adrenergic type, as well as inside the adrenal medulla and the vascular wall, the drug blocks adrenergic transmission, resulting in a gradual decrease in blood pressure.
The antihypertensive effect develops gradually (in comparison with reserpine), but is not too inferior to it in its expression. The peculiarity of the drug is that it is able to enhance renal filtration of the glomeruli, as a result of which blood circulation inside the kidneys increases.
The drug also has antiarrhythmic properties along with a sedative effect on the central nervous system. Compared to the action of reserpine, the sedative properties of the drug are expressed more weakly.
The medicinal effect of Raunatin begins gradually, approximately 10-14 days after the start of using the drug, and then lasts for 2-3 months.
In some cases, raunatin is better tolerated than reserpine.
Pharmacokinetics
About 40% of the drug is absorbed from the gastrointestinal tract after oral administration. The peak plasma level is observed after 1-3 hours. Approximately 40% of the substance is synthesized with plasma protein.
A small part of the drug undergoes metabolism within the mucous membrane of the small intestine, and then is absorbed into the blood and excreted through the kidneys after 24 hours. The main part of the drug undergoes liver metabolism with the participation of some enzymes (hydrolytic and oxidative). The half-life of the substance is within 50-170 hours.
Excretion is quite slow, occurring in urine, mainly in the form of decay products. Only less than 1% of the drug is excreted unchanged. About 40% of Raunatin is excreted unchanged in 96 hours.
Dosing and administration
Raunatin should be taken orally after meals. On the 1st treatment day, you need to take 1 tablet in the evening. On the 2nd - 1 tablet twice a day. On the 3rd - 1 tablet three times a day. Continue this way until the total amount of tablets taken per day is 4-6 pieces. Upon achieving a stable medicinal effect, the dosage should be gradually reduced to 1-2 tablets per day.
The duration of the therapeutic course is selected by the attending physician individually and depends on the patient's tolerance of the drug, as well as the degree of control of elevated blood pressure. In case of long-term use of the drug (more than 3-4 weeks), the maintenance daily dosage should not exceed 1 tablet.

Use Raunatina during pregnancy
The drug should not be prescribed to pregnant women or during lactation, because alkaloids can pass through the placental barrier and also into breast milk.
Contraindications
Main contraindications:
- intolerance to rauwolfia alkaloids;
- state of depression;
- organic disorders in the myocardium;
- coronary insufficiency;
- low blood pressure;
- aggravated gastric ulcer or ulcerative pathology of the duodenum, as well as ulcerative colitis;
- shaking palsy or nephrosclerosis;
- epileptic seizures;
- cholelithiasis;
- bronchial asthma;
- bradycardia;
- severe degree of cerebral atherosclerosis;
- use before electropulse treatment procedures.
It is prohibited to use in children, since there is no information regarding the use of Raunatin in this age group of patients.
Side effects Raunatina
Taking the tablets may provoke the development of the following side effects: hyperhidrosis, a feeling of weakness, and swelling of the nasal mucosa. In addition, sleep disorders, depression, dizziness, hypersalivation, bradycardia, and orthostatic collapse are possible. Cardialgia, asthenia, ulcerogenic effect, manifestations of allergy, parkinsonism, and dyspepsia may also occur, and in addition, libido may decrease. People with angina may experience increased heart pain.
Reducing the dosage or discontinuing the medication can eliminate the disturbances.
Long-term use of high doses of drugs can provoke functional liver disorder, the occurrence of nightmares, an increase in the frequency of angina attacks, as well as the development of manifestations of Parkinsonism.
Overdose
Signs of overdose: hyperhidrosis, a feeling of general weakness, and a marked decrease in blood pressure. In severe intoxication, after a short period of euphoria, hypodynamia, depression, a feeling of drowsiness, pronounced symptoms of shaking palsy, and a comatose state occur. In addition, loss of consciousness occurs.
To eliminate the disorders, gastric lavage and the use of activated carbon will be required. Vomiting should be induced, and a doctor should be called immediately, who will perform further symptomatic treatment.
In case of development of signs of Parkinsonism, cyclodol is used. Forced diuresis is also indicated. If there is a significant decrease in blood pressure or collapse has begun, it is necessary to lay the victim horizontally, raising his legs. If necessary, administer an intravenous injection of rheopolyglucin or mesaton, adding angiotensinamide or a solution of norepinephrine hydrotartrate (2%).
Sympathomimetics are prescribed with great caution, as they can provoke pulmonary edema. A solution of sodium caffeine benzoate (10%) can be administered subcutaneously. If there is respiratory arrest or a sharp suppression of this process, it is necessary to suck out mucus from the respiratory tract, perform intubation, oxygen therapy, and also perform artificial respiration.
The medicine has no specific antidotes.

Interactions with other drugs
To prevent the possibility of complications, it is necessary to prescribe Raunatin in small doses, and enhance its medicinal effect by combining it with saluretics and antihypertensive drugs.
The drug should not be combined with dopegit, as this may provoke depression.
The drug enhances the properties of barbiturates, as well as other sleeping pills, muscle relaxants, cholinomimetics (including carbachol with acetylcholine and aceclidine), adrenomimetics (such as norepinephrine with adrenaline and mesaton), and inhalation drugs.
Increases the arrhythmogenic effect of digitalis. Enhances the bradycardic and antihypertensive effects of anaprilin. Reduces the analgesic properties of morphine, as well as anticholinergics and anticonvulsants.
When combining antihypertensive drugs containing rauwolfia alkaloids and alcoholic beverages, the depressant effect on the central nervous system may be enhanced.
Raunatin in combination with drugs containing alcohol or A-adrenergic receptor antagonists can provoke orthostatic collapse.
Sequential or simultaneous use with other antihypertensive drugs (for example, hydralazine, as well as hydrochlorothiazide and ganglion blockers) is permitted.
Caution is required when combined with guanethidine, quinidine, as well as β-blockers and cardiac glycosides.
Combining antihypertensive drugs with general anesthetics can provoke a state of collapse.
Storage conditions
Raunatin is stored in a place protected from moisture and sunlight, as well as access of children. Temperature limits are 15-25°C.

Shelf life
