Indications for the use of impotence tablets are the treatment of erectile (sexual) dysfunction in men, except for cases where this dysfunction is caused by mechanical reasons (including injuries to the external genitalia), as well as in cases of damage to the sexual centers in the spinal cord and inflammation of the prostate gland or urethra.
The main names of impotence pills are: Sildenafil (Viagra), Tadalafil (Cialis), Vardenafil (Levitra, Staxyn), Impaza, Tribestan.
Pharmacodynamics
The mechanism of action of impotence pills Sildenafil (Viagra) has the active substance phenylsulfonyl-4-methylpiperazine, which acts by selectively inhibiting the enzyme phosphodiesterase-5, which ensures the breakdown of cyclic guanosine monophosphate (cGMP) - a nucleotide that controls the hemodynamics of the external genitalia. The result of this effect is an increase in the content of nitric oxide in the blood (secreted by the cells of the inner surface of blood vessels) and relaxation of the smooth muscles of the cavernous sinuses, which leads to filling of the arteries and penis with blood and its influx into the spongy tissue of the penis.
The same pharmacodynamics are found in the tablets Tadalafil (Cialis) and Vardenafil (active substance - vardenafil monohydrochloride trihydrate).
Impaza is a homeopathic remedy, and its effect on erectile function occurs due to the activation of NO synthase, an endothelial enzyme that promotes the release of nitric oxide and increases the level of cGMP in smooth muscle fibers. And then everything happens according to the same scheme as when using Sildenafil or Cialis.
The pharmacological action of Tribestan tablets that increases potency is based on the properties of the medicinal plant Tribulus terrestris, which contains furostanol steroid glycosides that have anabolic properties. This drug increases the synthesis of testosterone in the male body, which has a positive effect on erection.
Pharmacokinetics
Tablets for impotence Sildenafil, Tadalafil, Vardenafil after oral administration are absorbed in the gastrointestinal tract, enter the bloodstream and bind to proteins; the maximum content in plasma is noted after 2-5.5 hours.
The bioavailability of Sildenafil does not exceed 40%, Tadalafil and Vardenafil varies depending on age and functional state of the liver.
Biotransformation of active substances of these drugs is carried out by cytochrome enzymes of the liver, metabolites are largely excreted by the intestine with feces, and only slightly more than 12% - by the kidneys with urine. The half-life varies from 3 to 4 hours, for Tadalafil - almost 18 hours.
The pharmacokinetics of Impaza and Tribestan are not presented in the official instructions.
Method of administration and dosage
Tablets for impotence should be taken orally: Sildenafil 0.25 g once every two days; Tadalafil - 1 tablet (the desired effect appears after 20 minutes and lasts more than 30 hours). The initial single dose of Vardenafil is 10 mg (taken 30-60 minutes before the expected intimacy), it is permissible to increase the dosage to 15-20 mg.
It is recommended to take Tribestan one tablet three times a day (during meals), and Impaza tablets are not swallowed, but kept in the mouth until dissolved: one tablet 120 minutes before expected sexual contact and the second 60 minutes.
Overdose of Tadalafil and Vardenafil leads to more intense manifestation of side effects.
Contraindications for use
The main contraindications for use indicated in the instructions are the following:
Sildenafil - cardiovascular diseases (including history of heart attack or stroke), low blood pressure, severe liver failure, congenital retinal degeneration.
