Pharmadex is an eye drop that has anti-allergic and anti-inflammatory properties.
Indications Pharmadex
Treatment of steroid-sensitive non-infectious inflammatory and allergic conditions of the conjunctiva, cornea and anterior segment of the eye, including inflammatory reactions in the postoperative period.
Pharmacodynamics
Corticosteroids have anti-inflammatory properties, acting by inhibiting vascular adhesion molecules of endothelial cells, cyclooxygenase type I or II, and the formation of cytokines. This helps suppress the process of leukocyte adhesion to the vascular endothelium, and also prevents the formation of provocateurs of the inflammatory process - all this blocks the possibility of their penetration into the inflamed eye tissue.
Dexamethasone has a strong anti-inflammatory effect, but at the same time, compared to other steroids, it has a weaker mineralocorticoid effect. This substance is considered one of the most powerful anti-inflammatory drugs.
Pharmacokinetics
Dexamethasone is excreted by splitting. About 60% of the substance is excreted in the urine as 6-β-hydrodexamethasone. No unchanged substance remains in the urine. The half-life is only 3-4 hours. Binding to plasma proteins of blood serum is about 77-84%. The clearance is within 0.111-0.225 l/h/kg, and the distribution volume has values of 0.576-1.15 l/kg. The bioavailability of dexamethasone in case of oral administration is approximately 70%.
Dosing and administration
The medicine can only be used by adults and only for ophthalmological use.
If the disease is in an acute or severe stage, the inflamed eye should be instilled with 1-2 drops every hour. If the condition has improved, the frequency of the procedure can be reduced to instilling the same dose every 2-4 hours. Later, the dose is reduced to 1 drop 3-4 times a day.
If a chronic form of inflammation is observed, the medicine should be instilled 1-2 drops every 3-6 hours to achieve the desired effect.
To eliminate an allergic reaction, you need to drip 1-2 drops every 3-4 hours until the desired effect is achieved.
The duration of the treatment course is determined individually for each patient. On average, the duration is 2-9 days, and the maximum treatment can last 2-3 weeks.
Use Pharmadex during pregnancy
This medicine is not recommended for use by pregnant women.
Contraindications
Pharmadex may be contraindicated in the following cases:
- In case of hypersensitivity to the active substance and other components of the drug;
- Chronic bacterial infections;
- Acute form of superficial herpetic keratitis;
- Chickenpox or cowpox, as well as other viral diseases of the conjunctiva and cornea (except keratitis caused by herpes zoster);
- Diseases of the eye structures caused by fungi;
- Infectious diseases of the eye caused by mycobacteria.

Side effects Pharmadex
Possible side effects from the eyes:
- short-term burning sensation and discomfort in the eyes;
- tingling and itching;
- redness of the mucous membrane, as well as lacrimation;
- allergy;
- itching and swelling of the eyelids;
- deterioration of vision;
- narrowing of the visual field;
- increased IOP (intraocular pressure);
- perforation of the cornea as a result of its thinning;
- pupil dilation
- ptosis and keratitis;
- photophobia;
- occurrence of posterior cup-shaped cataract;
- clouding of the eye with possible destruction of the optic nerve.
GCS therapy can cause viral, bacterial or fungal infections, conjunctivitis or blepharitis, as these drugs suppress the body's immune response to the infection.

Interactions with other drugs
Long-term use of the drug in combination with iodoxuridine may increase the process of destruction of the corneal epithelium.
In case of using other local ophthalmological drugs (as concomitant treatment in combination with Pharmadex), it is necessary to maintain a small interval between procedures – approximately 10-15 minutes.
Storage conditions
The medicine should be kept in a place out of reach of children and away from sunlight. The room temperature should be between 15-25 °C.
Shelf life
Pharmadex is permitted to be used for 3 years from the date of manufacture of the medicine.
