Indications Pantogama
It is used for the following problems (in children from 3 years old, and also in adults):
- cognitive disorders that develop in neurotic diseases, as well as organic injuries affecting the brain (this includes complications after neuroinfections, as well as TBI);
- extrapyramidal diseases (such as hepatolenticular dystrophy, shaking palsy, familial myoclonus, Huntington's syndrome, etc.);
- CVD, which was caused by atherosclerotic abnormalities that arose in the cerebral vascular structure;
- extrapyramidal disorder (for treatment or prevention of its development) of a hyperkinetic or akinetic nature, developing due to the use of neuroleptics;
- epilepsy, against the background of which a slowdown in mental functions is observed (the drug is used in combination with anticonvulsants);
- excessively high psycho-emotional stress, deterioration of mental performance, and also physical activity - to increase concentration, as well as a positive effect on memory;
- urinary disorders of a neurogenic nature (such as increased frequency of urination, development of enuresis, imperative urinary incontinence, and also urgency);
- cerebral insufficiency in organic form, developing against the background of schizophrenia;
- Children over 3 years of age should take pills for the treatment of PEP, mental retardation (of varying severity), and also speech, motor or mental development (or a combination of these). In addition, it is used for various types of cerebral palsy, neurosis-like diseases (such as stuttering or tics) and hyperkinetic symptoms (such as ADHD).
The medicine can also be prescribed to newborns, immediately after birth – taking syrup.

Release form
It is produced in tablets of 250 or 500 mg, 50 pieces per pack, and also in syrup - in 100 ml bottles, 1 bottle per pack.
Pharmacodynamics
Pantogam has a range of action, which is due to the presence of the neurotransmitter GABA inside its structure. It directly affects the GABA (subcategory B) complex of canals and receptors. It has nootropic and anticonvulsant properties, increases the resistance of the brain to hypoxia, and also toxic effects. Inside neurons, the drug stimulates the activity of anabolic processes, weakens motor excitability, increases physical activity along with mental performance. In addition, the drug combines a moderate sedative effect with a mild stimulating effect.
In chronic alcohol poisoning, and also in case of refusal from alcoholic beverages, the medicine increases GABA metabolism. It has the ability to suppress acetylation processes involved in the mechanisms that inactivate novocaine with sulfonamides, due to which their effect is prolonged. Prevents pathological increase of the bladder reflex, and also increases detrusor tone.
Pharmacokinetics
After oral administration of the drug, hopantenic acid is absorbed from the gastrointestinal tract fairly quickly. The active substance penetrates the BBB and forms the highest concentrations inside the liver and stomach walls, as well as in the kidneys and skin.
The drug is not metabolized and is excreted unchanged within 2 days. About 28.5% of the drug is excreted in feces, and another 67.5% in urine.
Dosing and administration
The drug in the form of tablets and syrup is taken orally, after meals (this should be done after 15-30 minutes). It is recommended to take the drug before 4 p.m.
Use of the drug in tablet form.
The recommended single dose of the drug in tablets (for adults) is within 250-1000 mg, and the total daily dose is within 1500-3000 mg.
Children over 3 years of age should take the drug in a single dose of 250-500 mg; the daily dose is in the range of 750-3000 mg.
Treatment usually lasts for 30-120 days, but sometimes it can last up to 6 months. Therapy can be repeated 3 or 6 months after the end of the previous course.
During complex treatment of epilepsy, the daily dosage of the drug is 750-1000 mg, which should be taken for a period of 1 year or more. To eliminate (combination therapy) extrapyramidal disorder of a neuroleptic nature, it is necessary to take up to 3 g of the drug for several months.
When conducting treatment (complex) for hyperkinesis of the extrapyramidal type, people with hereditary nervous system diseases need to take 0.5-3 g of the medicinal substance for a period of up to 4 months or more.
To eliminate the consequences of neuroinfection or TBI, you should take 250 mg of the drug 3-4 times a day.
To restore the body's performance, which has deteriorated due to heavy loads or asthenia, it is necessary to take 250 mg of the drug three times a day.
To treat extrapyramidal disorder caused by taking neuroleptics, adults should take a dose of 0.5-1 g of the drug three times a day. Children should take 0.25-0.5 g of the drug 3-4 times a day. Such therapy should continue for 1-3 months.
To treat tics in children, the drug should be taken 3-6 times a day at a dosage of 0.25-0.5 g for 1-4 months.
Treatment of disorders in the urinary system - adults need to drink 0.5-1 g of the medicine 2-3 times a day. Children in such cases are prescribed 250-500 mg (with the recommended daily dose ranging from 25-50 mg/kg). Such treatment usually lasts for 1-3 months.
Taking into account the age, children with various pathologies in the field of the nervous system need to take 1-3 g of Pantogam tablets. The regimen is usually implemented as follows - the dosage is increased in the first 7-12 days, and then during the next 15-40 days, the maximum permissible dose of the drug is consumed, after which a slow reduction in dosage begins (duration - about 7-8 days) until the complete cancellation of the tablets.
If the patient requires a repeat course of treatment, it is necessary to take a break of 1-3 months before it.
Use of the drug in the form of syrup.
For adults, the single serving size is 0.25-1 g, and the daily serving size is within 1.5-3 g.
Infants from birth can be prescribed a single dose of syrup in the amount of 0.25-0.5 g. The daily dose will be 0.75-3 g.
Treatment lasts for 1-4 months, occasionally reaching half a year. The syrup can be prescribed again after an interval of 3-6 months.
The daily doses of syrup for a child vary within different limits, which are determined by his age, as well as the nature of the disease:
- infants up to 1 year – 0.5-1 g;
- children under 3 years of age – 500-1250 mg;
- age category 3-7 years – 0.75-1.5 g;
- children from 7 years old – 1-2 years.
The treatment regimen involves increasing the daily dosage over a period of 7-12 days, and then taking the maximum dose for 15-40 days or more. After that, it is necessary to gradually reduce the dosage (over 7-8 days), up to the point of discontinuing the drug. This course lasts for 1-3 months (during therapy for certain diseases, the duration can reach six months or more).
During complex treatment of epilepsy (along with anticonvulsants), it is necessary to take syrup in a dose of 0.75-1 g/day. Such treatment continues for a period of 1 year or more.
In complex therapy of schizophrenia (in combination with psychotropic drugs), it is required to take 0.5-3 g of Pantogam per day. This course continues for 30-90 days.
During the elimination of neuroleptic syndrome, against the background of which extrapyramidal symptoms arise, up to 3 g of syrup should be taken per day. Treatment in this case lasts for several months.
To eliminate the consequences of neuroinfection or TBI, it is necessary to take 0.5-3 g of the drug per day.
When treating hyperkinesis of the extrapyramidal type (people with organic diseases of the nervous system), it is necessary to consume 0.5-3 g of syrup per day. The duration of therapy in this case is 4+ months.
The use of syrup to restore performance after heavy loads or asthenia occurs according to the following scheme - 3 doses per day in the range of 0.25-0.5 g.
To treat urinary problems in children, 250-500 mg of syrup should be taken. The daily dose is 25-50 mg/kg. This course lasts for 30-90 days. Adults should take the drug 2-3 times a day, in the amount of 0.5-1 g.
If the patient requires long-term treatment, it is prohibited to use parallel drugs that have a similar effect.
Use Pantogama during pregnancy
Pantogam tablets should not be prescribed to pregnant or lactating mothers.
The syrup should not be taken by pregnant women in the first trimester.
Contraindications
The main contraindications for prescribing tablets: children under 3 years of age.
For syrup: the patient has phenylketonuria (due to the presence of aspartame in the syrup).
General contraindications: hypersensitivity to hopantenic acid or other ingredients of the drug. In addition, it is prohibited to take the drug in severe stages of acute kidney disease.
Side effects Pantogama
Usually, treatment with Pantogam does not cause complications.
Sometimes side effects such as allergic reactions (skin rash, runny nose and conjunctivitis) are observed. In such cases, it is necessary to reduce the dosage of the drug or completely stop the treatment.
In addition, noises in the head, daytime sleepiness or sleep phase disorder were noted. With such signs, discontinuation of therapy is often not required, and the disorders quickly disappear on their own.
Overdose
Due to intoxication with Pantogam, the manifestations of side effects may be potentiated, including a feeling of drowsiness, noises in the head or sleep disorders.
To eliminate these disorders, symptomatic treatment is required, during which the patient is prescribed gastric lavage and activated charcoal.
Interactions with other drugs
When Pantogam is combined with barbiturates, their effect is prolonged.
Combination with anticonvulsants leads to potentiation of their medicinal properties.
The drug prevents the development of side effects of neuroleptics and phenobarbital with carbamazepine.
Enhances the effect of local anesthetics (such as procaine).
The effectiveness of the drug is potentiated by combined use with glycine and etidronic acid.
Storage conditions
Pantogam should be kept in normal conditions for medicines. Temperature values are maximum 25°C.
