Omnic okas has antidysuric and α-adrenolytic properties.
Indications Omnica Ocasa
It is used to treat urinary disorders that occur against the background of a disease such as prostate adenoma.
Release form
The product is sold in tablets, 10 or 30 pieces per box.
Pharmacodynamics
The active ingredient of the drug is tamsulosin, which belongs to the category of substances that specifically and competitively block the activity of postsynaptic α1-adrenoreceptors, having a special affinity for their α1A, as well as α1D subtypes (they are responsible for the relaxation of the smooth muscle tissue of the prostate, the prostatic region of the urethra, and the neck of the bladder).
An oral dose of 400 mcg of tamsulosin increases the rate of urination and reduces muscle tone in the urethra and prostate, resulting in improved urinary flow and reduced negative symptoms associated with unstable bladder function. This effect reduces signs of obstruction and irritation caused by prostate adenoma, which persists even with prolonged therapy.
Like other α1A-adrenergic receptors, tamsulosin can reduce blood pressure values by weakening peripheral resistance, but it should be noted that daily use of 400 mcg of the substance did not lead to a clinically significant decrease in blood pressure levels.
Since the tablets of the drug have a slow type of release of the active component, tamsulosin is secreted slowly, demonstrating exposure with minimal variation in plasma drug parameters over the next 24 hours.
Pharmacokinetics
Tamsulosin is absorbed in the intestine, where about 57% of the dose taken is absorbed. Food intake does not affect the degree of absorption.
Tamsulosin demonstrates linear pharmacokinetic parameters. After taking the first tablet of the drug orally on an empty stomach, Cmax of the substance is observed on average after 6 hours. By the 4th day of the course, an equilibrium level of the element is recorded with the highest indicators in the serum, observed after 4-6 hours from the moment of taking the drug (without reference to food intake). At the same time, Cmax of tamsulosin in plasma increases to 11 ng / ml (compared to the value of 6 ng / ml, which is observed when using the first portion). The minimum serum level of the drug is 40% of the Cmax values in the plasma.
In people using Omnic Ocas, there are significant individual differences in plasma tamsulosin levels after single and repeated administration.
Plasma synthesis of the substance with protein is about 99%, and Vd values are approximately 0.2 l/kg.
Tamsulosin undergoes liver metabolic processes quite slowly, during which secretion of less active metabolic products of the substance is noted. In the blood plasma, most of the drug remains unchanged. The drug has almost no effect on the function of liver microsomal enzymes.
In case of liver diseases, there is no need to change the dosage of the medicine.
Tamsulosin, together with its metabolic products, is excreted largely through the kidneys (with 4-6% of the drug excreted unchanged).
The half-life of a single dose of the drug after oral administration of tablets is approximately 19 hours, and at equilibrium values – 15 hours.
Dosing and administration
The drug is used in a portion of 1 tablet (400 mcg), with 1-time use per day. Tablets are taken without reference to food intake. Such a therapeutic cycle can last without time restrictions. The drug can be used for continuous treatment courses.
The tablet is swallowed whole - it is forbidden to chew it in order to avoid negative effects due to the slow release of the active ingredient of Omnic Ocasa.
Contraindications
Among the absolute contraindications:
- severe liver failure;
- orthostatic collapse;
- the presence of hypersensitivity to tamsulosin or other components of the drug.
Relative contraindications of the drug:
- decreased blood pressure values;
- liver dysfunction;
- chronic renal failure (CC levels below 10 ml/minute).
Side effects Omnica Ocasa
The use of the medication can provoke the occurrence of various side effects:
- vomiting, diarrhea, constipation, and nausea;
- headaches, asthenia, dizziness, fainting (rarely);
- runny nose, urticaria, itching or rash on the epidermis and Quincke's edema (rarely);
- priapism (single) or ejaculation disorder.
There have also been isolated reports of small pupil syndrome occurring in people who have previously undergone cataract surgery.
Overdose
Poisoning usually manifests itself in the form of a decrease in blood pressure, as well as the development of compensatory tachycardia.
To prevent further absorption of tamsulosin, it is necessary to carry out the procedures required for cleaning the gastrointestinal tract (gastric lavage and enema), and in addition to this, prescribe oral use of sorbents. The patient's blood pressure and heart rate values can be restored independently if he is laid horizontally. If there is no result after using this method, it is necessary to decide on the possible need to take drugs that increase the level of BCC, and in addition, prescribe vasoconstrictors, if necessary.
In the process of eliminating the consequences of an overdose, it is necessary to monitor the state of kidney function. Due to the intensive plasma synthesis of tamsulosin with protein, its significant excretion during hemodialysis is unlikely.
Interactions with other drugs
Combination with warfarin, simvastatin, and also diazepam, amitriptyline, diclofenac and propranolol, as well as trichlormethiazide, glibenclamide and chlormadinone does not lead to changes in the free fraction of the drug in the plasma (in vitro), just as tamsulosin does not affect the values of the free fraction of propranolol, chlormadinone and diazepam with trichlormethiazide.
Combination with other drugs from the category of α1-adrenergic receptor blockers may cause potentiation of the antihypertensive effect.
Combining the drug with cimetidine can slightly increase the values of tamsulosin in plasma, while combining with furosemide causes their decrease. But with such changes, no adjustment of the doses of Omnic Ocasa is required, because the plasma level of its active substance remains within acceptable limits.
In vitro tests of metabolic transformations occurring within the liver did not register the development of interactions between tamsulosin and glibenclamide, finasteride, and also amitriptyline and salbutamol.
Combination with diclofenac or warfarin slightly increases the rate of elimination of tamsulosin.
Concomitant use of the drug with atenolol, enalapril, theophylline or nifedipine does not lead to the development of any therapeutic interactions.
Storage conditions
Omnic Okas must be maintained at temperatures not exceeding 25°C.
Shelf life
Omnic Ocas can be used within 36 months from the date of manufacture of the drug.
Analogues
Analogues of the drug are Alfatam, Omnic, Adenorm with Sonizin, Omsulosin and Revokarin, and in addition to this Proflosin, Urofrin and Omniprost. In addition, the list includes Sonizin, Dalfaz, Alfirum, Fokusin with Avodart, Kornam, Alfuzosin, Alfater, Dalfusin, Setegis, Urorek and others.
Reviews
Omnic Okas receives a large number of positive comments from urologists on medical forums. Due to its dosage form, in which the tablets have a slow release, it demonstrates a more stable effect (if compared with the capsules of simple Omnic), and at the same time much less often leads to the appearance of side effects, which are less pronounced. That is why it is preferable to prescribe it to people with problems with urination.
Reviews from patients treated with this drug are less clear-cut, because in addition to the positive effect on the treatment process, the drug also has a pronounced negative effect - for example, it reduces potency, lowers blood pressure and leads to ejaculation disorders.
