Triderm ointment for psoriasis

Indications Triderma for psoriasis
According to the official instructions, Triderm is not prescribed for psoriasis, and the indications for the use of this drug are: various types of dermatoses - when complicated by a bacterial infection; pustular skin lesions (impetigo); microbial and mycotic eczemas; dermatomycosis (including on the feet); superficial pseudomycosis (erythrasma), as well as lichen of fungal origin.
Does Triderm treat psoriasis? Triderm contains the fungicidal (antifungal) agent Clotrimazole, the content of which in 1 g of the drug is 10 mg or 1%; the antibiotic Gentamicin (1 mg or 0.1%) and the fluorinated glucocorticosteroid (GCS) Betamethasone dipropionate, which accounts for only 0.05% (0.5 mg in 1 g).
Psoriasis is an autoimmune disease, not an infectious one, and in its treatment, of the components included in Triderm, only Betamethasone is used externally, and this GCS is not used for the main form of psoriasis - extensive vulgar (plaque).
Therefore, Triderm for psoriasis, like all ointments with antibiotics or fungicides, is prescribed by dermatologists only in cases of infection, which significantly complicates the disease and worsens the condition of patients. Most often, this occurs due to a secondary infection - the development of dermatomycosis or streptococcal pyoderma - with psoriatic rashes on the inner thighs, in the groin folds or under the arms.

Pharmacodynamics
The mechanism of pharmacological action of Triderm is provided by its active substances. Thus, the anti-inflammatory effect of Betamethasone (a strong third-class GSK) is due to the blocking of the synthesis of proinflammatory mediators - cytokines of the interleukin and chemokine group - by immunocompetent cells (B- and T-lymphocytes), keratinocytes, dendritic cells and macrophages of the epidermis. And the antipruritic effect and reduction of swelling of inflamed tissues are the result of the activation of lipocortin, which suppresses the production of COX, prostaglandins, leukotrienes and thromboxanes.
An antifungal drug from the group of imidazole derivatives, Clotrimazole, disrupts the biosynthesis of membrane sterols in dermatophyte cells, yeast-like and mold fungi (Microsporum, Trichophyton, Epidermophyton, Pityrosporum, Candida, Torulopsis, Cryptococcus, Asppergillus).
The aminoglycoside antibiotic Gentamicin, penetrating into microbial cells, has a bactericidal and bacteriostatic effect on pathogens of many infections by disrupting the processes of protein synthesis of bacteria, in particular, Staphylococcus spp. Pseudomonas aeruginosa, E. coli, Shigella spp., Klebsiella spp. and some others. However, this antibiotic does not act on many strains of Streptococcus spp.
Pharmacokinetics
There are no data on the pharmacokinetics of Triderm ointment and cream in the instructions. However, there is a warning about the increased likelihood of transdermal absorption and systemic adsorption of the drug's ingredients (Gentamicin and Betamethasone) when applied to areas with damaged epidermis or to a large area of skin (<10% of the body surface).
It is known that corticosteroids can be absorbed through intact skin, after which they, like systemic GCS, bind to plasma proteins to varying degrees. Corticosteroids are metabolized mainly in the liver and then excreted by the kidneys or through the intestines - with urine and bile.
Dosing and administration
Triderm ointment or cream should be applied in a very thin layer to the affected areas of the skin - twice a day (morning and before bed). It is not recommended to apply bandages.
The maximum duration of use of this product is three weeks.
The instructions state that the use of Triderm for children under two years of age is prohibited, although Gentamicin is used in pediatrics only for children over three years of age.
Use Triderma for psoriasis during pregnancy
Experimental studies involving animals have established the teratogenic effect of topical GCS. Betamethasone is contraindicated in the first three months of pregnancy and throughout the entire lactation period. Its use at later stages is also questionable.
There is insufficient data regarding the topical use of aminoglycosides during pregnancy, in particular Gentamicin, but systemic use of this antibiotic is prohibited due to its ototoxic effect.
Clotrimazole is not used in the treatment of pregnant women during the first trimester.
Contraindications
In addition to hypersensitivity to the components of Triderm ointment or cream, contraindications to their use include rashes associated with syphilis and any form of tuberculosis of the skin; herpes and chickenpox; open wounds, ulcers and skin erosions.
Side effects Triderma for psoriasis
Triderm for psoriasis and infected dermatitis can cause side effects that include local irritation, hyperemia, burning and itching of the skin; dehydration of the epidermis with desquamation; inflammation of the hair follicles; the appearance of hypopigmented spots; development of steroid acne.
Long-term use of Triderm may lead to systemic side effects of glucocorticosteroids in the form of swelling; increased blood pressure, body weight and blood glucose levels; decreased bone strength and gastrointestinal problems. Such side effects are most likely, as well as the development of hypercorticism syndrome in children.

According to reviews from dermatologists, the use of glucocorticosteroid drugs for psoriasis is second only to those that have a keratolytic effect (due to the content of salicylic acid, urea, zinc, hydroxyvitamin D3, solidol or anthracene).
