Medexol is an anti-inflammatory medication used to treat ophthalmological diseases.

Indications Medexol
It is used to treat non-infectious lesions of the cornea, conjunctiva and anterior segment of the eye (allergic or inflammatory in nature).
Can be used when inflammation occurs after surgical procedures.
Release form
The drug is released in the form of a suspension for eye drops, inside dropper bottles with a volume of 10 ml. There is 1 such bottle inside the pack.
Pharmacodynamics
Corticosteroids are highly effective in treating inflammations affecting the eyes. GCS have an anti-inflammatory effect by inhibiting vascular adhesion molecules within endothelial cells, as well as COX 1 or 2, and also the process of cytokine secretion. This reduces the production of inflammatory conductors and suppresses leukocyte adhesion to the vascular endothelium, thereby preventing their passage into inflamed eye tissues.
Dexamethasone exhibits pronounced anti-inflammatory activity with reduced mineralocorticoid effects (compared to some other steroids) and is considered one of the most potent anti-inflammatory agents.
Pharmacokinetics
Ophthalmic bioavailability values for dexamethasone following topical ocular treatment were determined in subjects who had previously undergone cataract surgery.
The component reaches peak levels of about 30 ng/ml in the intraocular fluid after 2 hours. After that, a decrease in levels is observed with a half-life of 3 hours.
Dosing and administration
Medexol is used exclusively for ophthalmological procedures. The bottle with drops should be shaken before use.
At the beginning of treatment in acute or severe stages of inflammation, it is necessary to use 1-2 drops of the drug - instill into the conjunctival sac of the infected eye at intervals of 0.5-1 hour.
If a positive reaction develops, the dose is reduced to the same 1-2 drops, but at intervals of 2-4 hours.
Later, the dosage can be reduced to 1 drop, instilled 3-4 times a day (if this dose is sufficient to control inflammation).
Chronic forms of inflammation are treated by administering 1-2 drops of the drug at intervals of 3-6 hours.
In case of allergy or mild inflammation, 1-2 drops of the medicine are instilled at intervals of 3-4 hours until the desired result is achieved.
Treatment should not be terminated prematurely. During therapy, IOP levels should be monitored regularly.
After performing the instillation, you need to carefully close your eyes or perform occlusion of the nasolacrimal area. Such measures help to weaken the systemic absorption of the drug, thereby reducing the risk of general negative symptoms.
When using other local ophthalmic drugs during treatment with Medexol, it is necessary to maintain intervals between their applications of 10-15 minutes.
Use of drops for problems with kidney/liver function.
There have been no studies on the use of the drug in people with such disorders. However, the weak systemic absorption of the substance when used locally allows it to be prescribed to such patients without adjusting the dosage.
Scheme of application of the medicinal product.
Before starting the procedure, wash your hands. Then tilt your head back, pull down your lower eyelid, then bring the tip of the dropper to your eye and drop a drop of the drug into the resulting cavity by gently pressing on the bottle.
Do not touch the dropper to your eyelids, eyes or other surfaces, as this may cause the drops to become infected.
After instillation, you need to release the eyelid, close your eyes, and then gently press your finger on the inner corner of the eye (near the nose).
At the end of the procedure, the bottle with drops is closed with a protective cap.
If the drop was not instilled into the eye the first time, the procedure is repeated. If, on the contrary, too much of the drug was instilled, it is necessary to rinse the eye with warm running water and not instill it until the time of the next dose.
If a dose has been missed, a single dose should be instilled as soon as it is remembered. But if by this time it is already time for a new dose, the previous one should be skipped, instilling only the new one. It is forbidden to administer a double dose of the medicine.

Use Medexol during pregnancy
During pregnancy, the medication can be used only in situations where its use is expected to be of benefit to the mother rather than harm to the fetus.
It is recommended to refrain from breastfeeding during therapy.
Contraindications
Among the contraindications:
- presence of high sensitivity to dexamethasone or other medicinal elements;
- acute superficial keratitis caused by the activity of herpes simplex, chickenpox and cowpox, as well as other diseases in the area of the conjunctiva and cornea caused by the action of viruses;
- untreated bacterial eye infection;
- eye infections of mycobacterial origin, caused, among other things, by acid-fast bacteria (Koch's bacillus, Hansen's bacillus or Mycobacterium avium);
- fungal pathologies in the area of eye structures;
- an untreated purulent eye infection in the acute stage, which, like other diseases caused by bacterial activity, can be masked or potentiated under the influence of GCS.
Corticosteroids should not be used after the removal (without complications) of a foreign object from the eye or if the person has trauma or infection limited to the superficial epithelial zone of the cornea.
Side effects Medexol
The use of drops may provoke the occurrence of some side effects:
- test results: sometimes IOP values increase;
- lesions affecting the functioning of the nervous system: dizziness may occur;
- ophthalmologic disorders: often painful sensations in the eye area and damage affecting the cornea are noted. Sometimes eye irritation may occur. Rarely, corneal perforation occurs, glaucoma or cup-shaped cataracts develop, as well as visual field disorder, visual impairment and eye itching; in addition, allergic symptoms in the eye area appear. Occasionally, eye discomfort, keratitis, mydriasis or ptosis, as well as an unusual feeling inside the eyes may occur. Ocular hyperemia, visual fogging, coloration or erosion of the cornea may occur;
- pathologies of infectious genesis: eye infections occasionally develop (an exacerbation occurs or a complication appears).

Overdose
In case of intoxication with the drug after local application, it is necessary to wash off the excess with plain warm water.
Interactions with other drugs
The use of Medexol may provoke an additional increase in IOP values if it is used in combination with pupil-dilating eye drops (such drugs include atropine or other anticholinergic drugs), which can also increase IOP values.
When used in combination with other local ophthalmic medications, it is necessary to maintain intervals of at least 5 minutes between their applications.

Storage conditions
Medexol should be kept in a place out of reach of small children. Temperature indicators should not exceed 25 o C.
Shelf life
Medexol can be used within 36 months from the date of manufacture of the medicinal product. An opened bottle has a shelf life of 1 month.
Application for children
There is no information regarding the safety and efficacy of Medexol use in pediatrics.
Analogues
Analogues of the drug are Dexamethasone, Ozurdex implant, Pharmadex, Dexapos with Oftan Dexamethasone, and Maxidex.
