Many countries in the world have stopped using analgin because of its side effects. In our country, it is still used for treatment, and various combinations with other drugs are created to enhance its effect. Does a mixture of analgin and paracetamol have the right to exist, and if so, in what cases?
Can paracetamol and analgin be taken together?
This tandem exists, doctors make such appointments according to certain indications. The drugs have different active ingredients, but the same direction of action. Their simultaneous use enhances the therapeutic effect.
What happens if you drink aspirin with analgin? This combination is also used, but not for the treatment of pathologies, but for one-time use. The need for their combination arises when a high temperature persists for several days and does not go down. Aspirin with analgin will instantly reduce it.
Indications a mixture of analgin and paracetamol
Paracetamol is a widely used drug for colds, high fever, and pain relief, but its anti-inflammatory activity is low. [ 1 ], [ 2 ] Indications for taking analgin include pain syndromes of various etiologies: in muscles, joints, toothache, menstrual pain, and in the postoperative period. It has a powerful hypothermic effect and suppresses infectious and inflammatory reactions. [ 3 ], [ 4 ]
Paracetamol and analgin are used as antipyretics for colds, if the fever lasts for a long time, the general state of health is poor, and certain effective drugs such as paracetamol or ibuprofen do not help.
Release form
The most common form of paracetamol is tablets. They come in different dosages of the active substance of the same name: from 200 to 500 mg. In addition, there are capsules of the drug, and syrup, suspension and suppositories are produced specifically for children.
Analgin - tablets, each containing 500 mg of metamizole sodium. They also produce a solution for intramuscular and intravenous injections, with 250 or 500 mg of the active agent in an ampoule.
Pharmacodynamics
Paracetamol is a metabolite of phenacetin, previously widely used to reduce fever and relieve pain. It affects the pain and thermoregulation centers in the brain. It suppresses the production of prostaglandins, physiologically active substances that increase the sensitivity of pain receptors.
The pharmacodynamics of analgin are similar, its therapeutic effect is also associated with the suppression of pain mediators, substances responsible for heat production.
Pharmacokinetics
Paracetamol begins to act an hour after administration, is metabolized by the liver, and is almost completely absorbed in the intestine. It is excreted by the kidneys, the half-life is up to 4 hours.
Metamizole sodium is absorbed in the gastrointestinal tract approximately half an hour after administration, its maximum concentration in the blood is reached after 1-1.5 hours in the amount of 60%, the rest is excreted in the urine.
Dosing and administration
The combination of two drugs is not used for long-term treatment, but only in emergency cases, when, for example, it was not possible to bring down a high temperature with other means, and there is severe headache, joints and muscle pain.
It is very important to know how to give analgin and paracetamol, their proportions. You can take 0.35-0.5 g of paracetamol and 0.25-0.5 g of analgin at a time. Depending on how much active substance is in the tablet, the lower limit is half a tablet of analgin and half a tablet of paracetamol (the whole 500 mg), and the upper limit is 2 tablets of paracetamol (200 mg) and 1 analgin.
The maximum daily dose of analgin is 1 g, paracetamol - 2 g, for the mixture it is legitimate to use half the amount permitted. The course of treatment is no more than 3 days.
- Application for children
Analgin is not recommended for use by children, as it is a non-steroidal anti-inflammatory drug, which is characterized by its toxic effect on the body. Only starting from 12-15 years old can it be used to reduce temperature and relieve pain. In combination with paracetamol, it can be used in health-threatening conditions, but only under the supervision of a pediatrician.
The doctor will calculate the required dose depending on the age and weight of the child. Since both of these drugs have an antipyretic effect, an overdose during fever can lead to a rapid decrease in temperature, which will adversely affect the body's condition.
Use a mixture of analgin and paracetamol during pregnancy
Both drugs penetrate the placenta and into breast milk, although paracetamol gets into milk in small quantities. Since it is less toxic, it is still used if there is a threat to the woman's life. However, it is advisable not to do this in the first trimester of pregnancy.
Analgin is contraindicated both during pregnancy and breastfeeding. Therefore, a mixture of analgin and paracetamol is unacceptable for expectant mothers.
Contraindications
If one of the components of the mixture is contraindicated for use, it cannot be used. First of all, this is increased sensitivity to the components, serious liver and kidney pathologies, blood diseases, alcoholism, erosion and ulcer of the gastric mucosa, bronchial obstruction.
Side effects a mixture of analgin and paracetamol
Like any medications, analgin and paracetamol can cause allergic reactions. With prolonged use, analgin is dangerous due to the occurrence of agronulocytosis, leukopenia - a decrease in the level of leukocytes in the blood, impaired renal and hepatic function, arterial hypotension, thrombus formation, and bronchospasm.
The side effects of paracetamol are not so dangerous, but there have been cases of nausea, stomach pain, hypoglycemia, and anemia.
Overdose
Compliance with the dose will not cause an overdose, failure to comply with the rules, too long use can provoke nausea, vomiting, abdominal pain, abnormalities in the blood formula, tachycardia, decreased blood pressure. From the nervous system, dizziness and impaired coordination of movement may occur. Such symptoms require immediate cessation of treatment and emergency intervention.
Interactions with other drugs
The parallel use of a mixture of analgin and paracetamol with other medications must be agreed upon with a doctor, since their interaction can cause adverse reactions.
Thus, analgin is not used simultaneously with other NSAIDs. It, like paracetamol, reduces the diuretic effect. Tricyclic antidepressants, alcohol increases their toxicity.
There are so-called lytic mixtures, invented in the USSR, - combinations of several drugs, most often prepared in the form of injection solutions. They have a beneficial effect on eliminating fever with a very high temperature lasting several days. Their interaction is considered effective in stabilizing the normal state. Here are some of them:
- paracetamol, analgin and suprastin - the latter has an anti-allergic effect, enhances the effect of analgin;
- analgin, paracetamol and aspirin - a powerful trinity to reduce hyperemia;
- analgin, paracetamol and no-shpa - the antispasmodic component is added to eliminate spasms of the vascular system and restore blood circulation;
- paracetamol, diphenhydramine and analgin - diphenhydramine increases the analgesic effect of analgin and the antipyretic effect of the two components.
The above combinations are made for one-time use.
Combining analgin, paracetamol and warfarin at the same time is not advisable, since the latter is an antithrombotic agent, and its combination with NSAIDs (analgin) can cause dangerous bleeding.
Storage conditions
The preparations are stored in places not exposed to direct sunlight, at a temperature not exceeding +25ºС.
Shelf life
The expiration date is indicated on the packaging and instructions and is 2-3 years, after which the drugs are not used and are disposed of.
Analogues
Analogues of paracetamol and analgin for reducing high temperature are drugs called: panadol, antigrippin, theraflu, fervex, efferalgan, ibuprofen, nurofen, cefekon. As painkillers you can use spazmalgon, tempalgin, simpomaks, dolaren, pentalgin, ibuprofen, aspirin, etc.
Reviews
Although in recent years the Ministry of Health policy has been aimed at minimizing the use of drugs with the active substance metamizole sodium, it is not so easy for people to abandon the established ideas about the effectiveness of analgin, which has been used for many decades. According to reviews, it is still widely used to relieve pain syndromes and cold symptoms.
