Lindaxa is a subgroup of anorectic drugs with a central type of therapeutic effect. The active component sibutramine leads to blocking of the processes of reuptake of bioactive elements (including norepinephrine with serotonin).
Weight loss occurs due to the pronounced effect that the drug has on the food saturation centers, which ultimately leads to a decrease in the patient's appetite. [ 1 ]
Indications Lindaxa
It is used for obesity in the following cases:
- BMI indicators ≥30 kg/m2;
- BMI values ≥27 kg/m2, when additional negative factors are also present ( dyslipidemia or type II diabetes mellitus).
Release form
The medicinal substance is released in capsules with a volume of 10 mg (30 or 90 pieces per pack) and 15 mg (90 pieces per box).
Pharmacodynamics
The drug does not have a blocking effect on the reuptake of MAO elements, and also does not demonstrate affinity for the main endings (including histamine, dopaminergic, serotonin, benzodiazepine, and adrenergic). [ 2 ]
Pharmacokinetics
Biotransformation of sibutramine occurs within the liver, forming metabolic elements with therapeutic activity.
Plasma Cmax values for a single dose of the drug are recorded after 1-1.5 hours (for the active ingredient), and also after 3 hours (for metabolic components with therapeutic activity).
The half-life of sibutyramine is approximately 60 minutes; for metabolic substances demonstrating medicinal activity, it is within 14-16 hours.
Dosing and administration
At first, the medication is used in a daily dosage of 10 mg (1-time use); the capsules should be washed down with plain water. If during the period of the 1st month of therapy the weight decreases by less than 2 kg, the dose of the drug should be increased to 15 mg. If there is no effect from the introduction of this dosage, the medication must be discontinued. In addition, the medication is stopped if after weight loss it is further gained by 3+ kg.
Treatment with the drug can last for a maximum of 1 year, since there is no reliable information regarding the safe use of Lindaxa for longer periods of time.
During the therapy period, the patient also needs to make lifestyle adjustments to prevent weight gain after treatment is stopped.
- Application for children
The medicine is not prescribed in pediatrics (under 18 years of age).
Use Lindaxa during pregnancy
The medication should not be used during pregnancy. [ 3 ]
Contraindications
Main contraindications:
- personal hypersensitivity to sibutramine or other components of the drug;
- eating disorder (history of bulimia or anorexia);
- obesity caused by organic factors;
- history of or current presence of mental illness;
- IHD;
- use together with MAOIs or during a period of less than 14 days after stopping their use;
- administration with antipsychotics, antidepressants or tryptophan;
- strokes or TIA;
- Tourette's disorder;
- decompensated CHF;
- anomalies in the development of the heart that are congenital in nature;
- tachyarrhythmia or arrhythmia;
- elevated blood pressure (malignant form or poorly controlled);
- thyroid dysfunction, in which increased production of hormones is observed;
- severe renal/hepatic dysfunction;
- hyperplasia in the prostate area;
- persons over 65 years of age.
Side effects Lindaxa
The development of side effects often occurs during the first 3-4 weeks of therapy; they do not require dosage adjustment or drug discontinuation.
Constipation, loss of appetite, xerostomia and sleep disorders often occur.
Rarely observed dyspepsia, increased blood pressure, increased heart rate, signs of vasodilation (heat and flushing of the face), hyperhidrosis, anxiety, headaches, exacerbation of existing hemorrhoids and dizziness.
Vasculitis of the hemorrhagic type or nephritis occurs sporadically, and in addition, the platelet count decreases or the value of liver enzymes transiently increases.
Overdose
Possible signs of poisoning include potentiation of negative effects of drugs.
In case of overdose, it is necessary to excrete the medicine from the body as quickly as possible. Gastric lavage is performed, sorbents are prescribed, and symptomatic actions are carried out. In case of a strong increase in blood pressure or increased heart rate, it is necessary to use β-blockers.
Interactions with other drugs
Drugs that inhibit the action of CYP 3A4 (this includes erythromycin with troleandomycin, ketoconazole and cyclosporine) can cause an increase in the plasma level of sibutyramine - this should be taken into account when prescribing drugs.
Substances capable of inducing the activity of the enzyme CYP 3A4 (including carbamazepine with phenobarbital, rifampicin with dexamethasone and macrolides) increase the rate of Lindaxa metabolism processes.
Combination with medications that affect serotonin levels may provoke serotonin intoxication or serotonin crisis.
It is necessary to combine the drug with zolmitriptan, certain opiates, and also sumatriptan and dihydroergotamine very carefully.
The likelihood of interactions developing when using drugs with substances that increase heart rate and blood pressure, as well as drugs with antitussive activity, has been studied very little.
A single use with alcohol does not cause the development of negative manifestations, but its regular use will have a negative effect, since it contradicts the diet regime (due to the high calorie level).
Combination with MAOI is completely prohibited. If taking the drug is required, the MAOI should be discontinued at least 14 days before prescribing Lindaxa.
Storage conditions
Lindaxa should be stored in a dark and moisture-proof place; the permissible temperature level is a maximum of 30°C.
Shelf life
Lindaxa can be used within a 24-month period from the date of sale of the therapeutic substance.
Analogues
Analogues of the drug are Orsoten, Ponderal with Izolipan, Multisorb and Fepranon with Mirapront, as well as Fenfluramine, Meridia and Xenical.
Reviews
Lindaxa receives mixed reviews from patients. The drug effectively eliminates excess weight, but at the same time has a large number of contraindications and negative effects that do not allow it to be considered completely safe for health.
