Inspra has a diuretic effect.

Indications Inspra

It is used in myocardial infarction – to reduce the incidence of sudden general and cardiac mortality. It is also prescribed after myocardial infarction in people with left ventricular insufficiency.

Used in standard therapy for CHF.

Release form

The drug is released in tablets of 25 or 50 mg, in the amount of 30 pieces per pack.

Pharmacodynamics

The active element of the drug prevents the synthesis of progesterone and androgen endings with aldosterone, which is involved in the processes of regulating blood pressure and the occurrence of pathologies of the cardiovascular system. When eplerenone is added to treatment in people with CHF, an increase in aldosterone levels is predicted.

The use of Inspra in individuals with acute myocardial infarction significantly reduces the likelihood of mortality in this category of diseases.

The antihypertensive effect develops on average after a 14-day period, and after 1 month of regular use it reaches its maximum values. The intensity of the effect is maintained over an 8-24-week period.

Pharmacokinetics

Eplerenone is rapidly absorbed and distributed throughout the body (Cmax is reached after 2 hours). Bioavailability is 69%. Food intake does not affect absorption.

Intraplasmic protein synthesis of eplerenone is 50%; binding to erythrocytes is not observed.

Excretion is 67% via urine and 32% via faeces. The half-life is approximately 3-5 hours.

Dosing and administration

In case of CHF or myocardial infarction, therapy should be started with a dose of 25 mg, and then increased to 50 mg over the course of 1 month (monitoring blood potassium levels). The medication should be taken once a day.

The use of the drug does not depend on food intake.

Use Inspra during pregnancy

There is no information regarding the use of the drug in pregnant women. The data obtained after animal tests do not demonstrate indirect or direct negative effects on the course of pregnancy, embryonic and postnatal development, or the process of childbirth. The drug is prescribed to pregnant women with great caution.

There is no information regarding whether eplerenone is excreted in human milk after oral administration. However, preclinical testing has shown that eplerenone or its metabolic products are found in the milk of rats, but this does not affect the development of their offspring. Because there is no information regarding the possibility of adverse effects in breastfed infants, a decision must be made whether to discontinue breastfeeding or discontinue the drug, taking into account its importance to the woman.

Contraindications

Among the contraindications:

  • renal or hepatic insufficiency in a severe stage;
  • hyperkalemia, which has noticeable clinical expression;
  • severe personal intolerance associated with the drug;
  • combination with potassium drugs;
  • lactase deficiency or galactosemia.

Side effects Inspra

Taking Inspra may cause side effects:

  • headaches, severe dizziness and fainting;
  • eosinophilia or hyperkalemia;
  • decreased blood pressure, left ventricular failure, insomnia, atrial fibrillation or myocardial infarction;
  • pharyngitis or cough;
  • nausea, bloating, diarrhea, cholecystitis, constipation or vomiting;
  • itching, hyperhidrosis and local manifestations of allergies;
  • feeling of malaise and asthenia.

Overdose

There is no data on the development of overdose with eplerenone. In theory, hyperkalemia and a decrease in blood pressure should be expected.

Interactions with other drugs

Eplerenone should not be combined with potassium-containing agents because they potentiate the effects of antihypertensive medications.

Eplerenone should not be combined with lithium, as this increases the risk of lithium poisoning.

The drug should not be taken together with cyclosporine or tacrolimus, because this increases the risk of hyperkalemia and kidney problems.

Tricyclics, neuroleptics, as well as amifostine with baclofen, when combined with Inspra, potentiate the hypotensive activity of the drug.

The combination of the drug and GCS causes sodium and fluid retention in the body.

When using the drug together with trimethoprim, there is an increased risk of developing hyperkalemia.

Storage conditions

Inspra should be stored at temperatures not exceeding 25°C.

Shelf life

Inspra can be used within a 2-year period from the date of manufacture of the medicinal substance.

Application for children

Contraindicated for use in pediatrics (under 18 years of age).

Analogues of the drug are Dekriz, Spironolactone, Epletor with Renial, as well as Veroshpiron and Espiro.
