Ibuprom max is a non-steroidal drug with analgesic properties and belongs to the group of drugs that have a pronounced anti-inflammatory effect. In addition to acting as an analgesic and contributing to the removal of inflammation, due to its use, a decrease in the hyperthermia accompanying fever is achieved. Also, the anti-exudative effect of the drug is noted.
Indications Ibuprom Max
Indications for use Ibuprom max provide the possibility of its use as an analgesic and anti-inflammatory agent in diseases of the musculoskeletal system having degenerative and inflammatory character. To such belong, for example, arthritis of various types: chronic, psoriatic, rheumatoid, juvenile. In addition, the drug can be included in complex therapy of arthritis in systemic lupus erythematosus. It is also justified to use it in high-speed medicinal forms, when gout arthritis has an acute attack of gout.
It is expedient to prescribe Ibuprom max for patients with osteochondrosis, for patients with Persononej-Turner's disease (neuralgic amyotrophy), and also for ankylosing spondylitis (Bekhterev's disease).
High efficiency is characterized by the use of the drug as an analgesic for pains having a different etiology, including: pain syndrome accompanying arthritis and arthralgia, myalgia, ossalgia, sciatica. Ibuprom max helps to reduce the intensity of the painful sensations of migraine, the pain of the dental and headaches, as well as those that accompany the menstrual syndrome. This medication also acts as an effective analgesic used in cancer diseases. Indications are also the patient's presence of neuralgia and neuralgic amyotrophy, also known as Persononeja-Turner's disease; bursitis, tendinitis and tenosynovitis; Pain syndrome in combination with inflammation in the postoperative period.
Thus, the indications for the use of Ibuprom max are mainly in its inclusion in symptomatic therapy, as well as in the need to reduce the intensity of pain and reduce the intensity of inflammation at the time in which it is used. The drug does not have any significant effect on the course and prognosis of the disease.

In one Ibuprom max coated tablets there is a main active ingredient, ibuprofen 200 mg, and a number of auxiliary substances. Tablets with a coating of sugar shell differ in that in each of them ibuprofen is contained in 400-milligram quantity.
Thus, the most widely available dosage form of this drug is coated tablets. They can be found in different packages and in different quantities depending on the packaging.
Pharmacodynamics
The main active ingredient of this medication is ibuprofen, a substance synthesized from propionic acid. The effect that the preparation produces is due to the fact that its action causes a violation of the metabolism of arachidonic acid, as a result of the inhibition of the activity of the enzyme cyclooxygenase. The cyclooxygenase itself acts as a catalyst for the process in which endogenous biologically active substances are synthesized in arachidonic acid. The effect of the drug in the human body is, in particular, inhibition of the processes, that they synthesize thromboxane, as well as prostaglandins E, F. As for the latter, there is a decrease in their number, both in the focus of the inflammatory process and in the tissues of the central nervous system. The use of medication is characterized by the effect of nonselective inhibition of cyclooxygenase, which is due to its inhibitory effect on the activity of this enzyme in its two isoforms, cyclooxygenase-1 and cyclooxygenase-2, respectively.
When prostaglandins tend to decrease their presence in the inflammatory focus, the receptors become less sensitive to the effects of chemical stimuli. This in turn means that suppression of the synthesis of prostaglandins in the central nervous system provides a systemic effect of anesthesia. And by inhibiting prostaglandins in the hypothalamus, or more precisely - in its center, which is responsible for the thermoregulation of the body, the drug is effective as an antipyretic agent in case of fever.
Finally, acting as an inhibitor for processes in which thromboxane is synthesized from arachidonic acid, Ibuprom max produces an antiaggregant effect.
Pharmacodynamics Ibuprom max is characterized by good suction performance in the gastrointestinal tract. The degree of absorption of the drug reaches a maximum in the stomach and small intestine. The level of bioavailability is about 80 percent. However, it is necessary to note the following: in spite of the fact that with absorption of food, the absorption changes insignificantly, and there may be some slowdown in the absorption of the main pharmacologically active substance.
In the blood plasma, ibuprofen is 99%, that is, almost completely exposed to protein binding, among which forms such bonds with it mainly albumin. In order for a drug to be taken orally to come to its highest concentration in plasma it takes from 45 minutes to one and a half hours. The synovial fluid begins to differ by the presence of the drug at its maximum concentration 2-3 hours after taking. There is a relationship between the maximum concentration in the synovial fluid and changes in the maximum plasma concentration of the drug. The justification for this is the fact that biological fluids differ in the concentration of albumin.
Metabolization processes occur in the liver, where the drug is hydroxylated and carboxylated with conversion to metabolites which are not characterized by pharmacological activity.
The half-life of ibuprofen in its one-time 200-milligram dose occurs over 120 minutes. In the case of a higher dosage, the duration of the half-life period increases. After repeated use of the drug, it becomes equal to two to two and a half hours.
Pharmacodynamics Ibuprom max with respect to its excretion and the body is that this process is mainly carried out by the kidneys. The drug is excreted as metabolites and only about 1% leaves the human body unchanged.
In a very small amount in the form of metabolites, it is removed together with bile.
Dosing and administration
The method of administration and dose of the drug, as well as for how long a period of time is shown Ibuprom max, is determined by the attending physician, taking into account all the individual characteristics of each individual patient.
As a rule, this medication is prescribed for adults and children older than 12 years in a dose of 200 to 400 mg three times a day.
Each reception should be separated from the previous by a time interval of at least 4 hours. The interval between taking the drug can be 4-6 hours.
Elderly patients do not need a special dosage adjustment. As for patients with acute hepatic or renal failure, the dose for them should be reduced.
Ibuprom max is taken regardless of the meal. The tablet should not be divided into parts, it should be swallowed whole, washed down with water in the required amount.
The peculiarities of the use of the drug in patients with gastrointestinal diseases, and also if there are lesions of the duodenum and stomach with an erosive-ulcerative character, is that it is necessary to take Ibuprom max in the process of eating. This rule applies to all dosage forms of the drug.
In the event that as a side effect of ibuprofen there is a headache, in order to stop it, it is not allowed to increase the dose of this drug.
Use Ibuprom Max during pregnancy
The use of Ibuprom max during pregnancy is recommended to be excluded on the dates with the onset of the third trimester of bearing a child.
With regard to lactation and breastfeeding, it should be taken into account that due to the use of this medication, the presence of ibuprofen and its metabolites is indicated in the composition of breast milk. Breastfeeding can be continued only if the drug is taken in a daily dose of not more than 1200 mg. However, if there is a need for a long therapeutic course using Ibuprom max or its use in large doses, breastfeeding should be stopped.
Contraindications
Contraindications to the use of Ibuprom max have a rationale primarily for the patient's individual sensitivity to the effects of some of the constituent components of the drug. This also includes poor tolerance to patients with other non-steroid medications with anti-inflammatory properties.
Among the diseases in which the use of Ibuprom max is unacceptable is noted: the presence of allergic rhinitis, bronchospasm, urticaria. Both those taking place at the current time, and those present in the anamnesis.
It relates the existence of a predisposition to the occurrence of gastrointestinal bleeding, the presence of ulcerative colitis of a nonspecific type, a gastric ulcer and ulcerative lesions in the duodenum of aggravating anamnesis of the patient.
Ibuprom max is excluded from the list of permissible preparations for use in cases of severe renal as well as liver failure. It is also recommended to refrain from using it when there are any disruptions in the activity of the hematopoietic system. In addition, if there is a pathology of the optic nerve, with a sugar-isomaltous deficiency and when absorption of glucose-galactose is impaired.
Inadmissible is the use of the drug by women during the third trimester of pregnancy, and in addition - the appointment of children younger 12 years of age.
Some contraindications to the use of Ibuprom max are not categorical, but they call for the need to observe great caution. As, for example, in the case of bronchial asthma and systemic lupus erythematosus. Carefully weighed approach to the possibility of using the drug requires diseases that damage the connective tissue, violations of normal kidney and liver, heart failure and hypertension. Under regular supervision of the doctor, this medication should be taken during the first two trimesters of pregnancy, as well as the period when the woman is breastfeeding the baby.
It is necessary that long-term courses of treatment with the drug are accompanied by monitoring the state of blood, liver and kidney function.

Side effects Ibuprom Max
In a number of patients in connection with the use of the drug, the following side effects were noted Ibuprom max.
The response of the gastrointestinal tract, in particular, was the appearance of nausea, vomiting, heartburn, pain in the area "under the spoon", digestive disorders, this medication can cause the phenomenon of flatulence, lead to a violation of the stool. In exceptional cases, gastrointestinal bleeding occurred, gastric ulcer formation and ulcerative duodenal ulcer development occurred, colitis and ulcerative pancreatitis appeared.
Negative manifestations that occur in relation to the liver often represent the onset of liver failure, an increase in enzyme activity in the liver, and the development of hepatitis.
The central nervous system for the application of Ibuprom max can give its answer in the form of headaches and dizziness, a state of excessive drowsiness.
In the cardiovascular system, such negative changes as the occurrence of rapid heart rate, tachycardia are possible. Some cases were characterized by the fact that the use of this drug caused heart failure and the development of hypertensive states. There is also a certain degree of risk of thrombotic arterial events, strokes, myocardial infarction is not excluded.
Infringements can be exposed to the activity of the hematopoietic system, which causes the possibility of anemia, leukopenia, pancytopenia and thrombocytopenia.
The urinary system, exposed to the negative effects of the drug, gave edema, increased urea in the blood with a general decrease in the amount of urine throughout the day.
Ibuprom max can act as a cause of the appearance of various allergic reactions: bronchospasm, rashes on the skin, itching, urticaria, erythema multiforme and epidermal necrosis. And besides this, it can lead to anaphylactic shock and the occurrence of anaphylactoid reactions, among them Quincke's edema.
Side effects Ibuprom max when used for a long time is able to appear as agranulocytosis, in which the mucous membrane of the mouth is affected, there are pains in the throat. He is also accompanied by fever and there is an increased likelihood of bleeding. Patients with autoimmune diseases are at risk of aseptic meningitis, fever-associated headache, nausea, vomiting, impaired ability to navigate in space.
[14]
Overdose
Overdose Ibuprom max is characterized by the appearance of nausea, vomiting, gastralgia, dizziness and headache, lethargy and excessive drowsiness.
In the case of an increase in the applied dose, there are prerequisites in the future that hypotension begins, in the blood, the content of potassium increases. Concomitant phenomena are: fever, arrhythmia, metabolic acidosis, loss of consciousness right up to the coma, disrupted kidneys and respiratory function.
The drug used irregularly in elevated doses for a long time, as noted, causes hemolytic anemia, granulocytopenia and thrombocytopenia.
In view of the fact that there is no specific antidote for this medication, the main therapeutic measures are reduced to washing the patient's stomach and symptomatic therapeutic measures. In addition, enterosorbents are used.
Conducting hemodialysis in this case is unreasonable due to the fact that the drug is bound to a very large extent with proteins in the blood plasma.
It is necessary to monitor the basic functions of the body that are vital until all the symptoms of the negative consequences caused by excessively high doses of the drug are eliminated. Thus, electrocardiography and monitoring of changes in arterial pressure are shown. In addition, it is necessary to take the necessary actions to prevent gastrointestinal bleeding and eliminate disturbances in the central nervous system.
To avoid metabolic acidosis due to the overdose of Ibuprom max, it is necessary to promote the restoration and maintenance of acid-base balance of pH at a level of 7.0 to 7.5.
Interactions with other drugs
The interaction of Ibuprom max with other drugs is such that it makes it necessary to weigh the pros and cons of its use in combination with other non-inflammatory anti-inflammatory agents in view of the increased likelihood of all the negative consequences of affecting blood systems and the gastrointestinal tract. Of the non-steroidal anti-inflammatory drugs, only those with a low level of systemic action are acceptable together with Ibuprom max.
It is unjustified to use the drug if glucocorticosteroids are already prescribed, since this combination of these drugs causes a mutual increase in their toxicity.
Entering into interactions with antihypertensive drugs, among which it is necessary to note, first of all, drugs beta-adrenoblockers and ACE inhibitors, Ibuprom max reduces their effectiveness.
Anticoagulants in the applied sets with this drug require regular monitoring of blood coagulability due to the fact that platelet aggregation decreases from ibuprofen.
Medications with diuretic properties, forming a combination with Ibuprom max, are characterized by a weakening of the effect they produce. This is most typical of thiazide and loop diuretics.
For lithium preparations, this medication acts in such a way that their concentration in the blood increases. For this reason, it is necessary to conduct a regular blood test for lithium content.
The interaction of Ibuprom max with other drugs in some cases may, as evidenced in favor of the expediency of its use in the complex treatment, and cause the unjustified use of it with some of them. As, say in the case of zidovudine, an antiretroviral agent included in HIV therapy.
