Heparsil is included in the category of drugs with hepatoprotective effects. Its composition contains the active element silymarin, which is obtained from the fruits of the spotted milk thistle. The substance is a mixture of 4 different isomers of flavolignans: isosilybinin with silibinin, as well as silicristin with silidanin.
The drug has anti-toxic and hepatoprotective activity. The antihepatotoxic effect of silymarin develops when it competes with endings with respect to the corresponding toxins inside the walls of the hepatocytes. This leads to the development of membrane stabilizing effects. As a result, hepatic fibrosis and steatosis slow down.
Indications Geparsila
It is used in case of liver intoxication, as well as for maintenance therapy in persons with inflammation affecting the liver (of a chronic nature) or with hepatic cirrhosis.
Release form
The release of the drug component is made in capsules - 12 pieces inside a cellular packaging, 5 or 10 packages inside the box. It can also be produced inside containers - 30 capsules or 50 capsules.
Pharmacodynamics
Silymarin has a regulating cell and metabolic effects, inhibits the function of the 5-lipoxygenase pathway (especially the LTB4 component), and also stabilizes cell wall permeability and is synthesized with oxygen free radicals with reactivity.
The active substance also stimulates phospholipid and protein binding (functional and structural proteins) inside the affected liver cells (stabilizing lipid metabolism) and normalizes the activity of their walls, and at the same time synthesizes free radicals (has an antioxidant effect), thereby protecting liver cells from the negative effect helping them recover.
Influenced by flavonoids due to antioxidant activity and the ability to improve microcirculation. The clinical manifestation of these effects is an improvement of objective and subjective signs, as well as stabilization of the values of the hepatic function (decrease in bilirubin, transaminase, and γ-globulin). The result is improved health and amelioration of symptoms associated with digestive activity, and in people with impaired digestion (due to liver damage), appetite is improved.

Pharmacokinetics
When ingested, silymarin is absorbed at low speed inside the gastrointestinal tract. Participates in the processes of circulation within the liver and intestines. Accumulation of substances does not occur.
Exposed to intense distribution within the body. Testing conducted with the use of 14C-labeled silibinin showed that the component is found in large quantities inside the liver; inside the lungs, kidneys, heart and other organs, its indicators are very low.
The exchange processes of silymarin develop inside the liver by conjugation. Sulfates and glucuronides (metabolic components) are found inside the bile.
The term half-life of silymarin is 6 hours. Excretion is realized mostly with bile (approximately 80%) in the form of sulfates with glucuronides. Only a small part of the drug is excreted with urine (about 5%).
Dosing and administration
The drug is taken orally, after meals. Capsules are swallowed whole, without chewing, with simple water.
During the day, it is required to take 1–3 capsules of the drug 3 times (corresponding to 0.07–0.14 g silymarin).
The duration of therapy is chosen personally by the attending physician - taking into account the course of the pathology, as well as its nature.
On average, a therapeutic course usually lasts 3 months.

Information regarding the therapeutic efficacy and safety of Heparsil when it is used during breastfeeding or pregnancy is not available.
Side effects Geparsila
The drug is usually transferred without the appearance of complications. Only occasionally, with a strong personal sensitivity, the patient may experience side effects:
- lesions of the digestive function: vomiting, diarrhea, heartburn, nausea and dyspepsia;
- disorders affecting the subcutaneous tissue and the epidermis: epidermal signs of allergy appear sporadically - rash or itching, as well as potentiation of alopecia;
- disorders of the urethra: a single potentiation of diuresis occurs;
- problems with the work of the respiratory system: dyspnea appears singly;
- other symptoms: a single potentiation of already existing vestibular disorders occurs.
Negative manifestations are temporary in nature and disappear after drug withdrawal, without additional application of special measures.

Interactions with other drugs
Combined use of medication with oral contraception and medications used in estrogen-replacement treatment can lead to a weakening of the therapeutic activity of the latter.
Since silymarin inhibits the activity of the hemoprotein P450 system, it causes the potentiation of the action of individual drugs:
- anticoagulants (among them warfarin with clopidogrel);
- antiallergic medicines (for example, fexofenadine);
- hypocholesterolemic agents (such as lovastatin);
- antipsychotics (diazepam and alprozolam with lorazepam);
- antimycotics (among those ketoconazole);
- separate agents used in carcinoma (for example, vinblastine).
Storage conditions
Heparsil is required to be kept in a place closed to the penetration of small children. Temperature - not above 25 ° С.
Shelf life
Heparsil can be used for a 24-month term from the time a drug is sold.
Application for children
Experience with the use of drugs in children under the age of 14 is limited, which is why this age group is not prescribed.
